Mark your calendar for Open Enrollment the annual opportunity for you to make health benefits changes and to add or drop coverage for eligible dependents And be sure to bookmark the Cardinal at Work website your new online destination for benefits-related information

October 26 through November 13

Even if yoursquore happy with your elections from last year itrsquos always a good practice to review and confirm that yoursquove signed up for the right plan for you and your family If you choose not to take action coverage will either ldquoroll overrdquo or default into a new plan So take the time to review your options otherwise yoursquoll have to wait until the next Open Enrollment period to make changes unless you experience a qualified life event (such as getting married changing job status etc)

The cost of coverage will reflect the 2016 contribution amounts Also if you are not enrolled in Medicare and have a Health Savings Account (HSA) you may set it up directly with HealthEquity Blue Shieldrsquos financial partner or with a financial institution of your choice to set aside tax-deductible funds for future health care expenses

Health Plan Options

We know having options is important In 2016 if you are a pre-65 retiree and not enrolled in Medicare yoursquoll still have several plans to choose from including the Blue Shield EPO the Blue Shield Healthcare + Savings Plan (formerly the Blue Shield High Deductible Health Plan) as well as the Kaiser HMO and the Stanford Health Care Alliance (SHCA) plan

Dental

Good dental care contributes to your overall health and wellness and will continue to be part of our comprehensive health care benefits

Mental Health A healthy body also needs a healthy mind Yoursquoll continue to receive expanded access to out-of-network outpatient mental health providers for the Blue Shield EPO Blue Shield Healthcare + Savings and SHCA plans

BeWell Incentive ProgramTo encourage healthy lifestyle behaviors the BeWell program will continue to offer retirees the opportunity to participate in limited programs at a reduced cost including the Stanford Health and Lifestyle Assessment (SHALA)

WHATrsquoS CHANGING FOR RETIREES UNDER 65The plan changes for 2016 are comparable to previous annual benefit changes and include increases to health plan costs deductibles and out-of-pocket maximums and co-pays for office visits prescription drugs and other services Here are highlights of the changes

Health Plans

Blue Shield PPO plan is discontinued

bull The PPO Plan is being discontinued so current members will have an opportunity to elect a new plan during Open Enrollment

bull If you choose to do nothing yoursquoll be defaulted into the Blue Shield Healthcare + Savings Plan (formerly the High Deductible Health Plan)

Kaiser HMO

bull This plan is no longer the lowest-price plan

Stanford Health Care Alliance (SHCA)

bull This plan is now the lowest-price plan

bull SHCArsquos administrator is changing from Blue Shield to Aetna

bull Plan members will receive new ID cards by January 1 2016

bull Plan members should check the SHCA website at httpstanfordheathcareallianceorg for a list of in-network providers

Blue Shield Healthcare + Savings Plan (formerly the High Deductible Health Plan)

bull The Blue Shield High Deductible Health Plan (HDHP) is now called the Blue Shield Healthcare + Savings Plan

bull The plan design hasnrsquot changed only the name which better aligns with the Health Savings Account (HSA) You may set up an HSA directly with HealthEquity Blue Shieldrsquos financial partner or with a financial institution of your choice to set aside tax-deductible funds to help offset eligible health care expenses

This plan pays 80 of covered costs in network and 60 out of network after yoursquove paid the deductible

Deductibles $1750 individual $3500 family

Out-of-pocket maximum expenses $3750 individual $7500 family

Covered Service Charge Gender Reassignment

bull Gender reassignment procedure coverage has been extended to the Blue Shield EPO Blue Shield Healthcare + Savings and Stanford Health Care Alliance plans Previously Kaiser was the only plan to offer this coverage

bull Starting October 1 2015 CNA Insurance Company discontinued LTC coverage and is no longer our LTC insurance provider Stanfordrsquos new LTC insurance provider Genworth is not making LTC available to retirees

Retirees that elected LTC coverage by Sept 30 2015 may continue coverage with CNA Stanfordrsquos former LTC provider

For policies purchased by Sept 30 CNA will continue to manage LTC direct billing and customer service Please call 800-523-4582 (5 am - 3 pm ET) to speak to a representative

bull If you are a current active employee planning to retire you should consider getting LTC coverage now as LTC insurance will no longer be offered as a benefit once you retire

Q If Irsquom a current Blue Shield PPO member what steps must I take during Open Enrollment

A Because the Blue Shield PPO plan is being discontinued you have two options

1 Review and elect a new plan (Blue Shield EPO Blue Shield Healthcare + Savings Stanford Health Care Alliance or Kaiser)

2 Should you choose not take any action you will automatically be enrolled in the Blue Shield Healthcare + Savings Plan which is the high deductible plan With this plan you may have a Health Savings Account (HSA) which is an account used to pay for health care costs on a post-tax basis You may sign up for an HSA by contacting HealthEquity Blue Shieldrsquos financial partner or going through another financial institution of your choice NOTE You may only sign up for an HSA if you are not enrolled in Medicare

Q Why was the Blue Shield PPO discontinued

A Stanford elected to remove the PPO plan choice due to cost Over the years the

Blue Shield PPO plan has continued to be the most expensive plan offered by the university for what retirees pay in premiums as well as what the university pays in administrative costs Additionally when looking ahead at the Affordable Care Act (ACA) excise ldquoCadillacrdquo tax to be implemented in 2018 the PPO plan is already too expensive to maintain under the current ACA thresholds

Q Why is Kaiser no longer the lowest-price plan

A In 2015 Kaiser and SHCA were both priced the same For the 2016 plan year Kaiserrsquos

proposed increase was higher than that of SHCA so the Kaiser plan is no longer the lowest-price option

Q What are some things I can do to help me save on health care costs

A Some actions you can take to help defray costs are

Preventive CareTo stay on the healthy track be sure to take advantage of preventive care such as annual exams Preventive care is free under all of the plans as long as you stay in network

Choose generic vs brand name prescriptionsSave money on prescriptions by requesting generic or lower-cost versions of the medicine you need and by taking advantage of mail-order programs

Use urgent care facilities and limit emergency room visitsDonrsquot run to the emergency room for needs that are better suited to a doctorrsquos office or an urgent care clinic Also take advantage of your medical planrsquos advice nurse

If you choose the Blue Shield Healthcare + Savings plan stay in network and sign up for a Health Savings Account (HSA)The HSA allows you to set aside tax-deductible funds for future health care expenses on a post-tax basis The HSA may be set up directly through HealthEquity Blue Shieldrsquos financial partner or an financial institution of your choice

Note You may only sign up for an HSA if you are not enrolled in Medicare

Q As a retiree do I have access to Stanford Coordinated Care (SCC)

A Stanford Coordinated Care (SCC) is designed to provide current pre-65 active employees and their adult dependents support to manage ongoing health conditions SCC is not fully Medicare

approved and therefore is unavailable to retirees through the offered Medicare plans Once you transition to a Medicare plan SCC services are no longer available

2016 MEDICAL CONTRIBUTION RATESRetirees will receive a worksheet that outlines what they are eligible for and what it costs from Stanford Benefits in mid-October

Q If Irsquom a current Blue Shield PPO member what steps must I take during Open Enrollment

A Because the Blue Shield PPO plan is being discontinued you have two options

1 Review and elect a new plan (Blue Shield EPO Blue Shield Healthcare + Savings Stanford Health Care Alliance or Kaiser)

2 Should you choose not take any action you will automatically be enrolled in the Blue Shield Healthcare + Savings Plan which is the high deductible plan With this plan you may have a Health Savings Account (HSA) which is an account used to pay for health care costs on a post-tax basis You may sign up for an HSA by contacting HealthEquity Blue Shieldrsquos financial partner or going through another financial institution of your choice NOTE You may only sign up for an HSA if you are not enrolled in Medicare

Q Why was the Blue Shield PPO discontinued

A Stanford elected to remove the PPO plan choice due to cost Over the years the

Blue Shield PPO plan has continued to be the most expensive plan offered by the university for what retirees pay in premiums as well as what the university pays in administrative costs Additionally when looking ahead at the Affordable Care Act (ACA) excise ldquoCadillacrdquo tax to be implemented in 2018 the PPO plan is already too expensive to maintain under the current ACA thresholds

Q Why is Kaiser no longer the lowest-price plan

A In 2015 Kaiser and SHCA were both priced the same For the 2016 plan year Kaiserrsquos

proposed increase was higher than that of SHCA so the Kaiser plan is no longer the lowest-price option
