This year, we look forward to new ways in which Healthy Rewards can support you – and we can all support each other – on the journey to living well.

Over the past year, we asked you to consider the unique reasons each of you has for living well. Finding that motivation – that reason for continually making healthy choices – is the foundation for building a healthy lifestyle.

But sustaining healthy choices isn’t always easy. That’s why we need support systems to help us maintain our forward progress. Maybe it’s a workout buddy who keeps you on track at the gym; or a coworker who urges you to try new, healthier foods. I personally find support and motivation from my family. My husband, daughters, sisters, and now my beautiful granddaughter, all inspire and support me on my wellness journey.

The Healthy Rewards program offered by Children’s is another support system intended to help us focus on better health. Many of the activities are perfect for sharing with co-workers, family members and friends.

We can accomplish so much more with the support of others. It’s what our value of Collaboration is all about. This year, we look forward to new ways in which Healthy Rewards can support you – and we can all support each other – on the journey to living well. Because “wellness,” as you may have noticed, starts with “we.”

How it worksThe 2016/2017 Healthy Rewards program runs Sept. 1, 2016 to Aug. 31, 2017. Achieve your health goals and earn credits by taking part in wellness activities. Log in to your Healthy Rewards wellness portal at CHWHealthyRewards.org and enter each activity to confirm that it was completed. As credits add up, you become eligible for health plan premium savings or a wellness incentive.

To earn your 2017 Healthy Rewards credits, all activities must be completed by Wednesday, Aug. 31, 2017.

If you are enrolled in a Children’s Hospital health plan:

• Credits earned by Aug. 31, 2017 apply toward the cost of your 2018 health plan premiums (starting Jan. 1, 2018).

• If a spouse is covered under your health plan, he or she is eligible to participate, earn credits and save money. Healthy Rewards eligibility begins the same day as health plan coverage.

• Employees (and enrolled spouses) must complete BOTH (1) annual online Wellness Assessment and (2) a biometric screening to earn a premium reduction or wellness incentive.

• Other activity credits add up and help you earn a Children’s health plan premium reduction or wellness incentive. See the Healthy Rewards wellness incentive offering chart in this brochure for details.

Welcome to Healthy Rewards Healthy Rewards is the employee wellness program offered at Children’s Hospital of Wisconsin. It’s a coordinated approach to wellness activities and programs designed to improve your health and well-being and help you find, reduce or even eliminate preventable chronic diseases. Many of the activities are perfect for sharing with co-workers, family members and friends. The goal of Healthy Rewards is to help you live healthier, stay well and feel good — all while earning Healthy Rewards credits when you complete wellness activities.

Healthy Rewards credits add up and lower the cost of your health insurance. If you’re not enrolled in a Children’s Hospital health plan, credits can be used to earn a gift card if you are benefit-eligible.

Healthy Rewards supports our values of Health and Collaboration. Being healthy helps you to be at your best at home and work. And it’s easier to reach our health goals when we collaborate to support one another. Together in good health, we can work toward our vision of having the healthiest kids in the country.

Start earning and entering your creditsTo earn Healthy Rewards wellness incentive offerings for 2018, employees (and enrolled spouses) must complete an online Wellness Assessment and a biometric screening. We encourage you to strive to earn other credits to achieve Gold Level (500 credits) or Silver Level (250 credits).

Step 1: Complete an online Wellness Assessment (Required) April 1 - May 31, 2017Earn: 25 credits

This 20-minute, confidential online Wellness Assessment asks about your personal health history, nutrition, physical activity, safety habits and other current behaviors. Following survey completion, accurate and easy-to-understand feedback is provided based on your responses. The Wellness Assessment is available online (Apr. 1 through May 31 only) at CHWHealthyRewards.org.

Step 2: Complete a biometric screening (Required)Sept. 1, 2016 - Aug. 15, 2017Earn: 25 credits for completion of screening and up to 375 credits based on your results

This confidential biometric screening measures your HDL cholesterol, triglyceride, blood glucose and blood pressure levels, waist circumference and body mass index (BMI). For best results please fast for 10-12 hours before your appointment.

We offer employees and spouses two options for completing a biometric screening:

Onsite (April – May 2017): Log in at CHWHealthyRewards.org for more details about onsite biometric screening events. If you are unable to attend an onsite biometric screening, you can have your personal physician or provider complete your screening (see Physician Form option below).

Physician Form (Sept. 1, 2016 – Aug. 15, 2017): You have the option to verify that your personal physician completed your biometric screening. Go to CHWHealthyRewards.org to download the Physician Form and instructions.

• The physician must submit the completed form to Accountable Health Solutions by Aug. 15, 2017. There are three different ways to submit the completed form; please read the form for more details.

• Your results will generally be available within 7-10 business days after Accountable Health Solutions has received your results. Results will be posted to your wellness portal for you to confidentially view at CHWHealthyRewards.org.

After you complete both your Wellness Assessment and biometric screening, you will be able to view your customized Personal Wellness Report (PWR). The PWR is a comprehensive health risk analysis designed to communicate your current health status. In the PWR, you will find a summary of your biometric screening results and your lifestyle behaviors reported in your Wellness Assessment. You will also receive your Wellness Score, which is calculated based on your screening results and your lifestyle behaviors.

Step 3: Complete additional wellness activities and events to earn Healthy Rewards creditsReview the wellness activities described on the following pages. All credits must be entered into your wellness portal at CHWHealthyRewards.org by Aug. 31, 2017.

Year-round Healthy Rewards wellness activities and events Throughout the year, you will have the opportunity to participate in various healthy activities including online activities and challenges at CHWHealthyRewards.org. To check for monthly activities, log in and go to the Activities tab.

This year’s Healthy Rewards program features health content and activities that are updated every month!

Visit the Well-being Center in the Healthy Rewards wellness portal for:

• Information on important health and well-being topics

• Fun “put-into-practice” activities to earn credits

• Great recipe and fitness ideas, and more!

You’ll also find a Personal Health Track, which is a series of activities and challenges customized to meet your individual health needs.

Tobacco Status September 2016 - August 2017

Earn up to: 75 credits

You will receive credit for maintaining a healthy lifestyle of never using tobacco, or by being tobacco-free for one year or longer. The American Heart Association supports including e-cigarettes in the definition of tobacco products (or tobacco-derived products) and smoking. Therefore, if you are an e-cigarette user, you are not identified as tobacco-free.

Employees enrolled in a Children’s Hospital Health Plan

You will automatically receive credit for being tobacco-free once you have completed enrollment in the Children’s Hospital Health Plan using Employee Self Service.

Employees not enrolled in a Children’s Hospital Health Plan

If you’re not enrolled in a Children’s Hospital health plan, go to the wellness portal Activities tab and complete the Tobacco Use Affidavit Activity. If you indicate that you are not a tobacco user, you will receive credit.

Health CoachingSeptember 2016 - August 2017

Earn up to: 100 credits

Results from the biometric screening and Wellness Assessment identify whether you may be at risk for developing more serious health conditions in the future. Based on your results, you may be eligible for Health Coaching. Health Coaching is a highly personalized program that provides one-on-one assistance to help you meet your wellness goals.

From diet and exercise to emotional well-being, your health coach will help you identify areas you want to improve and work with you to develop healthy lifestyle behaviors as you work to achieve your goals. If you qualify, you will be contacted by Accountable Health Solutions.

Personal Health TrackSeptember 2016 - August 2017Earn up to 50 credits

Each year you will be offered a Personal Health Track based on your personal health profile. Complete this series of activities and mini-challenges to get inspired and create new health habits while earning credits! Your Personal Health Track will be available on the wellness portal after you have completed your 2017 biometric screening, and will take about 4-6 weeks to complete.

Put-Into-Practice Activities (monthly)September 2016 - August 2017Earn up to 60 credits

Visit the Wellbeing Center on the wellness portal. Each month, you’ll find a fresh new focus. From heart health to stress, you’ll find tips, inspiration and tools to help you be your healthiest you.

Plus, each month you’ll be offered a Put-Into-Practice Activity to complete for credit! Challenge yourself to track your healthy behaviors. See how many you can finish this year. Then, step back and notice the changes in yourself as these fun activities pave the path to a healthy new lifestyle.

Wellness Class SeriesSeptember 2016 – August 2017

Earn up to: 50 credits

Earn Healthy Rewards credits for up to five wellness-related classes taken either at work or in the community. These may include an exercise class such as yoga or Pilates; an educational class series about diabetes or asthma; or a nutrition or weight loss program such as Weight Watchers or Nutrition Counseling at Children’s.

Preventive Care ExamSeptember 2016 – August 2017Earn up to: 75 credits

Preventive care services you receive based on recommended guidelines for your age and gender can earn Healthy Rewards credits. These include dental exams, mammograms, routine physical exams and other events

considered as prevention, not treatment. Talk with your provider to discuss which services are recommended for you.

Healthy Work/Community EventsSeptember 2016 – August 2017

Earn up to: 100 credits

Healthy work or community events may include any physical, spiritual, health education or community volunteering event that takes place for at least one hour and is non-paid. Heart Walk, March of Dimes Walk, marathons, donating blood, soccer coaching or volunteering at church are just a few examples.

R&R SeriesSeptember 2016 – August 2017Earn up to: 80 credits

The R&R Series consists of two distinct programs to help staff learn and participate in self-care practices. Take part in a variety of programs that help reduce stress, renew energy, revitalize health, rejuvenate spiritually and build resiliency, including:

1. The monthly R&R program – Practical self-care video tips (approximately three minutes long)

2. The Creative Expression Studio – Creative writing, journaling, painting and transformative work

Wellness Committee and/or Team CaptainSeptember 2016 – August 2017Earn up to: 50 credits

The wellness committee is composed of employees, staff and leaders from across the organization. This committee meets 6- 7 times per year to plan and evaluate Healthy Rewards programming, anticipate questions or concerns related to the program and make recommendations for changes to enhance programming. Team Captains are employees who volunteer from each department or worksite to enhance participation, clarify programming and promote understanding of Healthy Rewards. They serve as the “go-to” wellness person at the department level.

Boost ChallengesSeptember 2016 - August 2017

Earn up to 40 credits

Throughout the year, you’ll be notified of some extra, quick challenges that will boost your wellness credits and boost your well-being!

Corporate Team ChallengeSeptember 2016 - August 2017Credits to be announced on the wellness portal

Join the challenge! Look for more information on the Healthy Rewards wellness portal so see the fun, inspirational team challenges that will help you to make positive change toward living a healthier life. The challenges are focused on healthy eating habits and increased physical activity, and encourage everyone, no matter what their fitness level, to make positive lifestyle changes through teamwork and support.

Healthy Rewards Satisfaction SurveyJuly – August 2017

Earn up to: 25 credits

Earn Healthy Rewards credits when you complete the annual program satisfaction survey, which will be available in July or August 2017.

Upon leaving employment, discontinuing medical coverage or reducing hours to less than .5 FTE, all rights to the incentives are lost. No payments will be made to employees or their dependents. Program requirements take into account that not all activities are available to all participants. All employees, including casual, PRN and those working less than .5 FTE are eligible for incentives during specific wellness activities. Healthy Rewards is committed to helping you achieve your best health status. Rewards for participating in a wellness program are available to all employees. If you think you might be unable to meet a standard for a reward under this wellness program, you might qualify for an opportunity to earn the same reward by different means.
