Emergency instructions Leave the area by the nearest stairway that is clear of smoke. Take away everyone immediately and close all doors as you leave. Conduct a final search of the building if it is safe to do so. Activate the nearest fire alarm pull station to evacuate the building

and to notify the Fire Department. Use stairways, NOT elevators! If you hear a fire alarm, leave the building immediately. If your nearest exit is blocked by fire, smoke, or heat, go to an

alternate exit. If heavy smoke is present, close the door and stay in the room.

Page 9: Safety training by Mohsin Raza Khan

If you are trapped: Call the rescue . Stop the infiltration with clothes, wet the material first (If possible) Cover your nose and mouth with a wet cloth. Retreat. Close as many doors between you and the fire as possible. Do NOT jump! And Do NOT break open windows! Hang an item out the window to attract attention of rescue teams.

If you must move through flames – hold your breath, move quickly, covering your head and hair.

If your clothes catch fire “stop, drop, and roll” until the fire is out. Assemble outside in a pre-designated area. Do not re-enter the building until notified to do so by the Fire Department. Follow the procedures in place to account for all employees after

evacuating.

What Is Safety Inspections?

An inspection is part of workplace monitoring Checking of substandard acts and conditions Checking the PEPME: people, environment, physical

conditions, machinery/materials and equipment. An audit of the whole health and safety system

Establish a good abatement plan

Page 30: Safety training by Mohsin Raza Khan

Mohsin Raza

Types Of Inspections

Informal Formal

Page 31: Safety training by Mohsin Raza Khan

Mohsin Raza

Informal Inspections

Performed everyday: checking the worksites for hazards prior to beginning work

Walking around by management, supervisors, workers to check/monitor conditions informally (not written down, but noted).

Page 32: Safety training by Mohsin Raza Khan

Mohsin Raza

Formal Inspections This is an example of regular inspections. Inspections can be conducted periodically with no defined timelines so

people cannot prepare also known as surprise inspections

Always inform someone higher up that you will be conducting the inspections so you do not create a hazard for yourself or the inspection team

Page 33: Safety training by Mohsin Raza Khan

Mohsin Raza

Planning an Inspection Determine what you wish to inspect with your team Who will be on the team Why you wish to inspect When you wish to inspect How you will go about it i.e. How to rate/rank hazards, what checklist to use Allocate enough time so you can be effective Ensure enough resources are provided so the corrective actions can occur in a

timely manner Assemble the checklist, pen and paper, materials, and the team and begin

Page 34: Safety training by Mohsin Raza Khan

Mohsin Raza

Conducting The Inspection Wear the necessary PPE: set a good example Do not operate equipment you are not competent to do so Inform the supervisor of the area if this is a regular inspection, and

be sure you are aware of the health and safety hazards in the area Get off the beaten path, look on, under, up and around, check for

hazards and prioritize them either high, medium, low…”A, B, C or 1-3

High hazards (A, 3) dealt with immediately, medium (B, 2) hazards 24 hours, (C, 1) low hazards 48 to 72 hours

Page 35: Safety training by Mohsin Raza Khan

Mohsin Raza

Writing The Inspection Report Use KISS principle Rank the worst hazards first Create an action plan Assign people responsible for corrective actions Record the implementation date of the corrective actions Assign a follow up person to check that these have been done Do not refer to people by name if they had an unsafe act Preserve the privacy of individuals this is the law Complete within 72 hours after the inspection (s) Explain why some corrective actions have not been done Post the inspection report

Issues May Contribute Towards A Major Accident Or Hazard:

Failing of the site safety management system Failure to recognize a hazard before and during

maintenance Failure to fulfill with the work permit system in hazardous

environments Communication failure during the use of a work permit

system.

Page 45: Safety training by Mohsin Raza Khan

Contributory Factors For An Assessor Whether staff have been sufficiently informed, instructed, trained and

supervised to minimize a potential human failing during operation of the work permit system

Whether the work permit system includes sufficient safety information, maintenance instructions, correct PPE and equipment for use

Whether the work permit contains sufficient information about the type of work required

Whether the employees responsible for control of the maintenance work are identified within the work permit system and that the work is properly authorized by a responsible person

Page 46: Safety training by Mohsin Raza Khan

Mohsin Raza

Whether the work permit system is managed, regularly inspected Whether all work permits are kept on file Whether sufficient precautions are taken prior to initiating a work

permit Whether staff are aware of the type of environment they are working

in during the operation Whether the work permit system involves a formal procedure

whereby the maintained plant or equipment is handed back to operation.

Page 47: Safety training by Mohsin Raza Khan

Mohsin Raza

Major Hazards Could Arise Wrong type of work permit used Wrong information about work on the work permit; Failure to recognize the hazards where work is carried out Terms of work permit not observed to Failure to hand-over plant in safe condition on completion of

work/cancelling of work permit Unauthorized staff performing work permit functions; Poor management of the work permit system Insufficient monitoring of the work permit system.
