Determination of Isotretinoin and Acitretin in Pregnancy

Acitretin (trade names Soriatane and Neotigason) is a second-generation retinoid. It is taken orally, and is typi-cally used for psoriasis.. Acitretin is an oral retinoid used in the treatment of severe resistant psoriasis. Because of the potential for problems and severe side effects it is generally used in only very severe cases of psoriasis that have been unresponsive to other treatments.

The goal of this study was to develop a method for the simultaneous measurement of biologically active endoge-nous retinoids in human serum. Due to the challenges in separating the isomers of RA as well as the metabolites, chromatographic separation was optimized for UPLC. The method was validated with spiked charcoal-treated blank human serum and used to determine retinoid con-centrations in serum from healthy men.

Epidemiology

Use most usually occurs in adolescents In 1988, to 60 000 female patients of childbearing age,

per year, in the USA were treated with isotretinoin Tertiary centre in Leeds demonstrated

: Leeds has an approximate population of 160 438 females of childbearing age (15–44 years). Therefore, in a year it would be expected that approximately 1 in every 500 of the child-bearing population will be exposed to isotretinoin.

In 2004, British Association of Dermatologists reported 16 pregnancies over a 6-month period

Canadian study of 8609 women treated with isotretinoin found that 90 became pregnant (1%)

BNo risk in other studies동물을 대상으로 한 연구에서 위험성이 없는 것으로 판단되나 인간을 대상으로 한 연구가 결여된 경우로 동물이나 인간에게 유의한 위험이 없는 약물들 , 예로 페니실린이나 인슐린 , 타이레놀 등이 여기에 해당된다 .

CRisk not ruled out동물이나 사람에게서 적절한 연구가 되어 있지 않거나 , 혹은 동물실험에서 태아에 해로운 영향을 다소 끼치기는 하지만 사람에게서 적절한 자료를 얻을 수 없는 약물들이 여기에 포함되며 대다수의 약물이 여기에 포함된다 .

DPositive evidence of risk태아에 대한 위험의 증거가 있으나 약물을 사용하여 얻을 수 있는 이득이 태아에 대한 위험보다 크다고 판단되는 약물들이다 . 항암제와 간질환자에게 사용하는 항경련제 등이 여기에 해당된다 .

Contraindicated in pregnancy명백한 기형의 위험성이 증명되었고 , 그 위험성이 이득보다 현저한 약물들로 임신 중에 절대 사용하지 말아야 하는 약물들이다 . 여드름치료제인 이소트레티노인 (isotretinoin)이라는 약물은 임신 중 중추신경계 , 안면 , 심혈관계의 기형을 만드는 대표적인 X 등급의 약물이다 .
