INFERTILITY OVERVIEW

Infertility means difficulty in conceiving (becoming pregnant) despite having regular sex without using contraception. There is no definite cut off point to say when a couple is “infertile’ many couples take several months to conceive. About 17 in 20 couples conceive within a year of trying. About 19 in 20 couples conceive within two years.

Looking at this another way, about 1 in 7 couples do not conceive within a year of trying. However, more than half of these couples will conceive over the next year without any treatment.

IUI (INTRA UTERINE INSEMINATION)

• An intrauterine insemination (IUI) is a procedure in which the sperm is • “washed” and prepared then deposited into the upper portion of the uterus to

help achieve a pregnancy. It is often in excellent option for couples prior to considering more complicated and very expensive procedures, such as in vitro fertilization(IVF).

• The wash and IUI can be very helpful to couples seeking to have a child when the man has low sperm counts, low motility, hyper viscosity (very thick semen), motile sperm that are clumped together (antibodies), sperm that “ backs up” into the bladder (retrograde ejaculation) or other abnormal sperm test such a poor sperm shape (morphology). Many couples use IUI to accelerate the chances of pregnancy, such as after vasectomy reversal or if the woman is older.

• It is also useful when the woman’s mucus is hostile or compatible with man’s sperm, or if she has problems with her cervical positioning or opening, as well as unexplained causes of infertility.

Methods of Sperm Preparation.1. The fundamental objective of the various preparation procedure employed is

to separate the spermatozoa from the seminal plasma as thoroughly as possible with minimum mechanical force. These are adequate for normal semen sample. However, as the quality of reducing the level of physical damage inflicted upon the spermatozoa and at the separating the contaminating material. The modifications fall into the following basic categories.

The basic aim of semen washing / processing is as follows:1. Removal of seminal plasma.2. Removal of non motile sperms, morphologically defective and immature

sperms.3. Removal of debris and pus cells.4. Concentration of motile sperms.5. Capacitating of motile sperms.

3. SPERM COUNTING CHAMBER The Makler Counting Chamber is a simple to use device for Sperm Count and motility evaluation from undiluted semen. Depth of only 10 microns. Constructed with two pieces of optically flat glass, cover glass upper, 2nd layer with a 1sq.mm finde grid in the centre subdivided into 100 squares of 0.1x0.1 mm each. Percentage and quality of motility and sperm counts are easily obtained.

IVF (IN VITRO FERTILISATION)

In vitro fertilisation (IVF) is a process by which an egg is fertilised by sperm outside the body: in vitro. IVF is a major treatment for infertility when other methods of assisted reproductive technology have failed. The process involves monitoring a woman's ovulatory process, removing ovum or ova (egg or eggs) from the woman's ovaries and letting sperm fertilise them in a fluid medium in a laboratory. When a woman's natural cycle is monitored to collect a naturally selected ovum (egg) for fertilisation, it is known as natural cycle IVF. The fertilised egg (zygote) is then transferred to the patient's uterus with the intention of establishing a successful pregnancy. The first successful birth of a "test tube baby", Louise Brown, occurred in 1978. Louise Brown was born as a result of natural cycle IVF. Robert G. Edwards, the physiologist who developed the treatment, was awarded the Nobel Prize in Physiology or Medicine in 2010.

ICSI (INTRA CYTOPLASMIC SPERM INJECTION)

• ICSI is generally performed in addition an in vitro fertilization procedure to extract often several oocytes from a woman.

• The procedure is done under a microscope using multiple micromanipulation devices (micromanipulator, microinjectors and micropipettes). A holding pipette stabilizes the mature oocyte with gentle suction applied by a microinjector. From the opposite side a thin, hollow glass micropipette is used to collect a single sperm, having immobilised it by cutting its tail with the point of the micropipette. The oocyte is pierced through the oolemma and into the inner part of the oocyte (cytoplasm). The sperm is then released into the oocyte.

CRYO PRESERVATION UNIT Embryo cryopreservation is useful for leftover embryos after a cycle of in vitro fertilisation, as patients who fail to conceive may become pregnant using such embryos without having to go through a full IVF cycle. Or, if pregnancy occurred, they could return later for another pregnancy. Spare oocytes or embryos resulting from fertility treatments may be used for oocyte donation or embryo donation to another woman or couple, and embryos may be created, frozen and stored specifically for transfer and donation by using donor eggs and sperm.
