1. Go to www.carelinknetwork.org

2. Click the link that states “Already a Provider? Access the Portal”

3. Enter the following:

• Providers are expected to submit certificates at the time of renewal– Accreditation (if applicable)– License (for Adult Foster Care

homes and Hospitals)– Insurance• CareLink and/or ConsumerLink, as well

as DWMHA should be listed as additional insured

• Fax DASH Logon requests to Provider Relations

• DASH is web-based and can be accessed from any computer

• DASH Logons are unique and passwords should not be shared– Every employee using DASH should have

their own Logon ID– There is no limit to the number of IDs

• If an employee leaves your organization, notify us to deactivate their access
