• Providers are expected to submit certificates at the time of renewal– Accreditation (if applicable)– License (for Adult Foster Care

as DWMHA should be listed as additional insured

• Additional funds (from DWMHA) to increase the wages of non-professional direct care staff– Funding covers $1.00 per hour
increase• Providers should submit
completed Attestations monthly– Providers will receive an email, with
an attestation template, if they are required to submit the attestation

• For Residential Providers• Vacancies are to be reported in
DASH weekly– Ensure we have the most up to date
information when assisting consumers with housing

• Fax DASH Logon requests to Provider Relations
• DASH is web-based and can be accessed from any computer
• DASH Logons are unique and passwords should not be shared– Every employee using DASH should have
their own Logon ID– There is no limit to the number of IDs
• If an employee leaves your organization, notify us to deactivate their access
