The Volunteer of Year will berecognized at the Utah HeadStart Association 2011 - 2012conference which will be heldNovember 2, 2012 in Sandy, UT.

Award Criteria

Note: Failure to meet any of thecriteria below will result inautomatic elimination.1. Applicant must have been avolunteer to the Head Start program in2011-2012. This can be a parentvolunteer or a volunteer from thecommunity.2. The volunteer can nominatethemselves or can be nominated bytheir local Head Start program.

Requirements

1. Include a one-page, double space,typed essay outlining why the nomineedeserves to be recognized as the UtahHead Start Association Volunteer of theYear.

Ranking

Applications will be ranked on thefollowing:

20 points- Total number of hours servedin Head Start from September 2011through May 2012.

80 points- Essay
