1. Header – repeats on top of every page

2. Top of Invoice – appears once at the top of the first invoice page

3. Invoice Body – main body of invoice that includes list of billing items

4. Bottom of Invoice – appears once at the bottom of the last invoice page

5. Footer – repeats on the bottom of every page

6. Appendix – appears at the end of invoice. Does not contain header or footer
