This module will discuss how to add and remove Responsible Party

information.

Responsible Party Information

• To capture responsible party information, the patient must be in a household.

Scenarios

Scenario #1

• Adding Responsible Party Info to Household

On the Household member page, click the ‘Edit’ button.

Under Responsible party, click the ‘Add’ button.

A line will open to enter the Responsible Party information. If more lines are needed, click the ‘Add’ button again.

Once the information is entered, click the ‘Save’ button.

You will be returned to the Member page.

Under Responsible Party, click the drop-down.

The drop-down will have those names that were added on the Household edit page under

Responsible Party.

Pick the name you want, then make sure to hit a ‘Save’ button. If you go straight to

Registration, this information will not be retained.

If the information is saved,

Responsible Party will now show up on the Registration label.

Scenario #2

• Delete Responsible Party info from a Household

You will need to start on the Member page.

Under the Household members section, click the drop-down for

Responsible Party.

On the drop-down, select the line that is blank. Note the blue line below.

This will eliminate the name from the member page. Make sure to hit a ‘Save’ button to save your

changes.

If the Responsible Party needs to be deleted, click the ‘Edit’ button on the member page.

On the household edit page, click the blue X next to the name to remove. Then click ‘Save’.

Important Information

• Make sure you remove the information on the Member page BEFORE you delete the Responsible Party on the Household edit page. If you delete on the household edit page first, the Responsible Party info will stay with the patient.
