Inspection Notes
1. The BIN plae is located on the begin abutment stem under girder G6. It is in OK condition.
2. There are no changes to the Access, Debris or Electrical forms. The debris form is attached.
3. All stone veneer was inspected and no loose stones were noted.
4. All bays, except Bay #6 are covered with stay-in-place (SIP) forms. No loose concrete was noted on the underside of theexposed deck and overhangs.
5. Vertical clearances were verified.

6. Non-structural condition observations have been issued during this inspection.
7. There is a Joint Maintenance Report attached to report.
8. Steel and Collision Vulnerability Assess Reviews are requested in this inspection.
