• Clearly write your name and seven-digit ACCA Membership number.• Place a tick () in the box () of the candidates you are voting for.• The minimum number of votes is one (1) and maximum number is five (5).• This form must reach the ACCA Singapore office by post or by hand before 28 June 2019, 5.00pm.• Thereafter, ballot forms will be accepted on-site where the Annual General Meeting will be

conducted.

Please note that only ACCA Singapore members with registered Singapore address are eligible to vote. Non-members, affiliates and students are ineligible to vote.

Important Notes:• Ballot forms without the member’s name or membership number and signature shall not be included in the count.• Ballot papers with more than five (5) votes shall be declared void. The minimum number of votes is one (1).• All candidates shall be invited in writing to observe the count. Any candidate who is unable to attend the count

may delegate one representative provided that the Head of ACCA Singapore is notified in advance.• The declaration of the election by the Returning Officer is final (save for any manifest error).
