Transient Elastography

• Ultrasound transducer probe• On same axis as narrow vibration source

• Pulse vibration creates elastic shear• Probe reads vibration return or echo

• Stiffness of tissue increases velocity of return.

When to use these tests

• When the underlying diagnosis is clear and cirrhosis, yes/no is all that is needed.

• Staging for hepatitis C therapy• Monitoring patients on methotrexate• Older or frail patient population• Step-wise approach with liver biopsy as a back-up• Deciding on whether cirrhosis care testing is needed.

• Decreased free water clearance• Hyponatremia

MELD-Na: the importance of hyponatremia

• Predictive of mortality beyond just MELD especially at MELD scores <20-22

• MELD-Na = MELD score + 1.59 (135 – serum Na).

Ascites Management

Ascites Management

• Dietary advice• Low Na diet: <2 g/d• Fluid restriction: 1.8-2.0 L/d free water

• How much fluid to withdraw a LVP: more art than science

• Spontaneous peritoneal peritonitis• Diagnostic tap recommend for any hospitalized cirrhotic with ascites• 1st choice: cefotaxime; 2nd : fluoroquinolone• Albumin must be given (1.5 g/kg day 1; 1.0 g/kg day 3)

• MELD >/= 10-12• Child-Turcotte-Pugh >/= 7

• Child’s B

• Index decompensatory event• Encephalopathy• Variceal bleed• Ascites

Exceptional cases: Referrals when MELD may be low

• Hepatocellular carcinoma• Two lung associated disorders

• Hepatopulmonary syndrome• Portopulmonary hypertension

Hepatocellular Carcinoma• Milan criteria or “5-3-3” rule or TMN </=T2

• One tumor < 5 cm or…• Up to 3 tumors, but…• All < 3 cm.

Qualification for MELD of 22:FDA approved therapy results in the following:

MPAP < 35 mm Hg and PVR < 400 dynes/sec/cm-5Satisfactory right ventricular function exists (center-specific testing).

Liver-Lung Disorders: Bottom line

• If cirrhotic patient complains of shortness of breath, consider…• Pulse oximetry• Echocardiogram with Doppler and bubble study.

A few last notes “from the trenches”…

• No NSAIDs in cirrhotics• How to give lactulose• We don’t biopsy for HCC lesions much anymore.
