Complete one form for CogAt Grade 2 or Grade 3:This form will be emailed to you from the Student Assessment department Indicate the grade level, school number, school name and number of TO BE SCORED documents- Fax a copy to Student Assessment at 754-321-4291 on the last day of the assessment window (3/11 or 5/6)Do not use the Building Identification Sheet or Grade/Class identification sheet provided with your materials
