Community hospitals
The community hospitals provide health services such as
rehabilitation, outpatients, out of hours services, first aid care
and end of life care for people living in Oxfordshire.
Where are my community hospitals?
Oxford Health NHS Foundation trust has services at the
following locations: Abingdon, Bicester, Didcot, Henley, Oxford
City, Wallingford, Wantage and Witney.
All of the hospitals provide a 24-hour service. Visiting times
differ at each of the hospitals so please contact the relevant
hospital reception for more information. All of the hospitals
provide inpatient care and some of the hospitals provide day
hospital and outpatient services.
What can I expect from my
community hospital?
People are admitted to a community hospital when they no
longer require the services of an acute hospital. Community
hospitals provide active rehabilitation and palliative care,
particularly for older people.

There are a range of health care professionals working within
the community hospitals. These include registered nurses,
healthcare assistants, speech and language therapists,
physiotherapists, occupational therapists, porters, domestic
and housekeeping staff together with administration and
clerical staff. Volunteers also provide support to the hospitals.
The community hospitals are supported by GPs and
gerontologists, who are doctors specialising in the care of
older people. They work in close liaison with other community
services such as community nurses, intermediate care,
community matrons, dieticians, and mental health services.
Our community hospital staff aim to work in partnership with
you when planning for the next stages of your care, whether
that is your discharge home or to another care setting.
Planning for your discharge will start on the first day of your
admission. Staff will agree with you a predicted date of
discharge which they will then work to. Plans for your
discharge will be agreed in partnership with you, your relatives
and carers, members of the multidisciplinary team who are
usually responsible for your care and if relevant, other
organisations such as social services.

How do I get referred to a community
hospital?
If you are living at home then you can be referred to a
community hospital by your GP. If you are already in another
NHS hospital, then you can be referred by that hospital via a
centralised team. You cannot refer yourself to a community
hospital. If you are feeling unwell you should contact your GP.

What to bring with you to hospital
If you are being admitted to hospital, you should bring the
following items in with you:
dentures/shaving
equipment
walking stick/frame
hearing aid
continence aids
special shoes
spectacles
any appointment cards
writing material, postage
stamps
books to read
anything special to your
needs
all medicines, tablets and creams that you are
using
day clothes and shoes, night clothes, slippers,
dressing gown and underwear (including tights
and stockings)
hairbrush, comb, shampoo, toothbrush,
toothpaste, denture products, soap, face flannel
and paper tissues.
If you use or need any of the following then you should also
bring these in with you:

Laundry
We are unable to provide a laundry service for patients at our
community hospitals. However, in exceptional circumstances
we may be able to help.
Money and valuables
We advise that you do not bring large sums of money or
valuables into the hospital. If you cannot avoid this your
admitting nurse can arrange for their safe keeping and will
give you a receipt.
We regret that community hospitals cannot accept any liability
for money, valuables or property that are not handed over for
safe-keeping.
Smoking
All community hospitals operate a no smoking policy. We
provide a smoke-free environment for your health, other
people’s health and the general safety of all. If you do smoke
and wish to take this opportunity to stop please ask your team
for further information.

Visiting
Each hospital has different visiting hours. Please ask a
member of staff for more information.
Car parking
There is limited car parking available on all sites and some
operate pay and display.
