Allegheny County Wilbert C. Taylor, III, license no. BL053741, last known of Pittsburgh, Allegheny County, was suspended
pursuant to the Order of the Court of Common Pleas of Allegheny County dated December 11, 2017,
which the court issued under section 4355 of the Domestic Relations Code. The suspension is effective

PA Board of Cosmetology Philadelphia County Martha Pena t/d/b/a Martha’s Beauty Salon and Boutique Unisex, license no. CB120891, of Philadelphia,
Philadelphia County, was indefinitely suspended and was assessed a $2,250 civil penalty, based upon
employing unlicensed individuals, aiding and abetting in the unlicensed practice of the profession, and
failing to have an owner or designated person in charge of the salon during regular business hours.

PA Board of Massage Therapy
Berks County Melissa Jennifer Szymanski, license no. MSG003733, of Birdsboro, Berks County, was suspended for no
less than three years retroactive to July 15, 2016, based on findings she violated the terms of a previous

Cumberland County Liping Yan, a/k/a Li Ping Yan, L.M.T., d/b/a Li's Asian Spa, license no. MSG009316, of Carlisle,
Cumberland County, was order to pay a civil penalty of ten thousand dollars ($10,000), suspended for one
(1) year to be immediately stayed in favor of probation, and a public reprimand placed on her permanent
disciplinary record with the Board, because she utilized the services of the unlicensed Wang Yi Fan in
April of 2017 to provide massage therapy services for clients of Li's Asian Spa; aided and abetted Wang
Yi Fan in the unlicensed practice of massage therapy by allowing Wang Yi Fan to provide massage
therapy services; did not cooperate with the Board, the Department of State, or the Bureau of Enforcement
and Investigation in the investigation of a complaint; did not provide massage therapy records
immediately upon demand by the Board; did not maintain massage therapy records for at least 3 years
from the last date that services were provided to the client; did not conspicuously display the massage
therapist’s name and the title L.M.T. on an identification badge where massage therapy services were
being provided; and did not include the massage therapist’s license number in all advertisements

Delaware County William Eric Webb, license no. MSG004011, of Wayne, Delaware County, was revoked and assessed a

Lackawanna County Chris Clarke, of Scranton, Lackawanna County, was denied a massage therapist license based on his
felony conviction under the Drug Act, a crime of moral turpitude, and inability to practice with

Rehabilitative Disposition in the disposition of felony charges, in the courts of this Commonwealth, a
Federal court or a court of any other state, territory or country; was convicted of any felony; has been
convicted of any misdemeanor related to the practice of the profession and made a misleading, deceptive,
untrue or fraudulent representation in the practice of the profession or practicing fraud or deceit, either
alone or as a conspirator, in obtaining a license, certification or registration or in obtaining admission to a

John Francis Mitchell, license no. MD020824E, of Allentown, Lehigh County, was ordered to pay costs
of investigation of $3500 and successfully complete 30 hours of remedial education, because he failed to
comply with the Board's regulations when prescribing controlled substances to his patients and was guilty
of immoral and unprofessional conduct with respect to his prescribing of Vyvanse at dosages greater than
manufacturer's recommendations and prescribing Vyvanse together with simultaneous prescriptions for
other similar medications without adequate documentation in the medical record to justify the use of said

notarizations in chronological order; failed to be familiar with the duties and responsibilities of a notary
public, by failing to maintain custody and control of her notary seal, sharing a notary register with another
notary, and failing to record all notarization in chronological order; and failed to be of good character,
integrity and ability, by sharing a notary register with another notary and failing to record all notarizations

sanitary standards; and aided and abetted the unlicensed practice of pharmacy in violation of
