SECTION 1
Introduction
This report documents the field activities and associated results from the August 2002quarterly surface water and groundwater monitoring event. The quarterly monitoringprogram is a component of the U.S. Environmental Protection Agency's (EPA) ongoingRemedial Investigation/Feasibility Study (RI/FS) at the Lava Cap Mine in Nevada City,California.
The field activities performed were as specified in the Draft Addendum No. 3 to the FieldSampling Plan (FSP)for the Remedial Investigation/Feasibility Study, Lava Cap Mine SuperfundSite (EPA, 2001a). All sampling and analysis procedures followed the Project QualityAssurance Project Plan included in the Addendum No. 3 to the FSP (EPA, 2001a). Sampleswere collected in the following general areas: reference area, source area, mine area, anddowngradient of the mine, including areas between the mine and Lost Lake, the Lost Lakearea, and downgradient of Lost Lake. The sampling locations during the August 2002sampling event are shown on Figures 1 through 4.

Field Activities
2.1 Supplemental Data Gaps InvestigationAs a follow-up to the data gaps investigation conducted in 2001, three surface soil sampleswere collected in August 2002 from around the upper rental residence on the Lava CapMine property. Samples had not been collected in the immediate vicinity of this residenceduring the initial RI investigation. These surface soil samples were analyzed for metals.
2.2 Routine Groundwater and Surface Water MonitoringProgramAs outlined in the Draft Addendum No. 3 to the Field Sampling Plan for the RemedialInvestigation/Feasibility Study, Lava Cap Mine Superfund Site (EPA, 2001a), the routinegroundwater and surface water monitoring program included the following tasks:
Groundwater• Collected groundwater samples and measured water levels in 10 groundwater
monitoring wells installed during the RI (seven wells) and data gaps investigation (threewells). These wells are IB and 1R (upgradient reference area); 5A, 5D, 5E, 51 and 5J(waste rock/tailings pile); and 13Q, 13R, and 13S (Deposition Area).
• Sampled groundwater from 13 residential wells at the mine, along the Little ClipperCreek (LCC) drainage and around Lost Lake. These locations included three wells at themine (Wells 10G, 10H and 10J), four wells around Lost Lake (Wells 11AB, 11AE, HAGand HAI), four wells along the LCC drainage between the mine and Greenhorn Road(Wells HAL, HAS, 11AU, and 11AV), and two wells along the LCC drainage betweenGreenhorn Road and Lost Lake (Wells 11AF and 11AO).
• Collected split samples from five residential wells (Wells 11AB, 11AF, HAG, HAI, and11AO) to help assess the cause of increased arsenic concentrations in recent monitoringevents.
• Measured water levels in the three piezometers (5PZ1,5PZ2, and 5PZ3) that wereinstalled in the waste rock/tailing pile as part of the data gaps investigation.
• Measured field parameters, including oxidation-reduction potential (ORP) readings,from all wells during sampling using a closed cell flow-through instrument to providedata that are more reliable.
• Analyzed both filtered and unfiltered water samples for metals from all monitoring welllocations. All samples were analyzed for cations (calcium, magnesium, potassium, and

sodium), anions (chloride, sulfate, and phosphate), total dissolved solids (TDS), andalkalinity. Selected locations were also analyzed for sulfide and cyanide.
Surface Water• Sampled one reference surface water location along Clipper Creek (CC) above the
confluence with LCC (Location 2G).
• Sampled the source area surface water at the mine, including the adit discharge(Location 3A) and the base of the Log Dam (Location 4A).
• Sampled LCC between the mine and Deposition Area (Location 12J) just above theconfluence with CC.
• Sampled the northern (Location 16B) and southern (Location 16C) lobes of Lost Lake.
• Sampled CC at two locations below Lost Lake- Location 19B (the seep at the base of LostLake Dam) and Location 19A (just above the confluence with Little Greenhorn Creek[LGC]).
• Sampled one downgradient surface water location on LGC (Location 19M). The samplelocation is approximately 1.5 miles downstream from the confluence of CC and LGC.
• Measured field parameters, including ORP readings, from all locations during sampling.The field parameters were taken from probes in a closed cell container, except wherereadings are taken directly from a surface water body.
• Analyzed both filtered and unfiltered water samples for metals from all locations. Allsamples were also analyzed for cations (calcium, magnesium, potassium, and sodium),anions (chloride, sulfate, and phosphate), TDS, and alkalinity.

The monitoring results discussed below focus on arsenic - the primary contaminant ofconcern at the Lava Cap Mine site. Table 3-1 presents a summary of environmental samplescollected during the August 2002 quarterly monitoring program. Table 3-2 presents waterlevels measured in the monitoring wells and piezometers. Tables 3-3 through 3-5 presentarsenic results for monitoring wells, residential wells, and surface water samples,respectively. Previous monitoring results, where available, are also shown on the tablessuch that trends over time can be reviewed.
The complete analytical results from the August 2002 quarterly monitoring are included inAppendix Al. Listings of detected concentrations, by constituent, are included in AppendixA2. Field parameters are included in Appendix B.
3.1 Supplemental Data Gaps SamplingThe surface soil samples collected around the residence at the mine contained arsenic atconcentrations ranging from 100 to 300 milligrams per kilogram (mg/kg). Theseconcentrations are well above reference area concentrations, which are generally less than 30mg/kg, indicating the presence of tailings-impacted soils around the residence. Theconcentrations are considerably lower than the surface soil concentrations around the tworental residences located closer to the waste rock/tailings pile area (EPA, 2001b).
3.2 Routine Groundwater and Surface Water MonitoringProgramGroundwater Monitoring WellsAll 10 monitoring wells installed during the RI and data gaps investigation were sampledduring the August 2002 quarterly event. The arsenic concentrations detected in monitoringwells from the RI to present are shown in Table 3-3. In August 2002, arsenic concentrationsexceeded the drinking water maximum contaminant level (MCL) of 10 micrograms per Liter(ug/L) in all wells, except the bedrock monitoring well (Well 13S) installed in theDeposition Area (see Table 3-3). The concentrations were generally consistent with theresults from the RI monitoring.
The arsenic concentration in the upgradient reference well IB was lower than in previoussampling events, with a result of 14 ug/L in both the filtered and unfiltered samples. Incontrast, the arsenic results from the upgradient reference well 1R were higher than inprevious sampling events. In August 2002, the arsenic concentrations in well 1R were 24ug/L (filtered sample) and 25 Ug/L (unfiltered sample).

Arsenic concentrations throughout the waste rock/tailings area continue to be elevated withthe results ranging from 25 ug/L (5D and 51 filtered samples) to 470 ug/L (5E unfilteredsample). The highest arsenic concentrations detected in groundwater continue to be frommonitoring well 13R in the Deposition Area. This shallow well, which is screened entirelywithin the tailings, had concentrations of 1,700 ug/L (unfiltered) and 1,500 Mg/L (filtered).The Deposition Area monitoring well (13S) completed in the bedrock that underlies thetailings had a concentration of 2 ug/L in both filtered and unfiltered samples.
For several of the monitoring wells the variation reported between filtered and unfilteredsamples is relatively small (IB, 1R, 5A, 5J, 13R, and 13S). Two monitoring wells, 5D and 51,exhibited substantial differences between the filtered and unfiltered samples. In these wells,the unfiltered arsenic concentrations were approximately 4 and 5 times higher than thefiltered results (Table 3-3). The data presented in Table 3-3 indicates that large differencesare common in well 51, but not in 5D. The field parameter data presented in Appendix Bindicate that well 51 has the highest turbidity of any monitoring well, which likelycontributes to some of the observed differences between filtered and unfiltered results.
Residential WellsThirteen residential wells were sampled in the August 2002 quarterly event. The resultsfrom these wells over time are shown in Table 3-4. Because there were a number ofapparently anomalous detections of arsenic in residential wells during the previous 2002monitoring events, split samples were collected from five locations and sent to a separatelaboratory during the August 2002 quarterly event.
The arsenic concentrations in the three residential wells sampled at the mine ranged from 20ug/L to 49 Hg/L. These concentrations all exceed the drinking water MCL of 10 ug/L, butthey remain below the concentrations detected in monitoring wells completed beneath thewaste rock/tailings area at the mine. The concentrations in all three wells are consistentwith previous quarterly events (Table 3-4).
Three of the four residential wells located just downgradient of the mine property continuedto exhibit elevated arsenic concentrations ranging from 31 ug/L (HAL) to 140 ug/L (HAS).Concentrations have increased slightly in each of the last two quarters in well 11AV anddecreased slightly in HAL. However, concentrations in both wells remain within the rangeof historical detections (Table 3-4).
Arsenic concentrations in the four wells sampled around Lost Lake and the two wellssampled along the lower reaches of the LCC drainage were all very low, non-detect at 0.5ug/L in five wells and detected at 1.5 ug/L in one well. The split sample results, whichwere all collected from this group of wells, were consistent with the results from the EPARegion 9 Laboratory (see Table 3-4), once the Region 9 lab re-analyzed the samples toaddress matrix interference problems. Three of the wells that had exhibited the anomalousarsenic detections earlier in 2002 were non-detect in both the EPA samples and the splitsamples (Table 3-4).
The discovery of interference problems in the process being used at the Region 9 laboratoryprovides an explanation to the increased arsenic concentrations reported in severalresidential wells earlier in 2002. To provide further confirrnation, another round of split

samples was collected from all of the residential wells during the November 2002monitoring event. This data will be presented in the next quarterly report.
Surface WaterNine surface water/mine discharge sample locations were sampled as part of the August2002 quarterly sampling event (see Table 3-1). Overall, surface water concentrations werehigher than those detected in the previous two monitoring events conducted in 2002 (seeTable 3-5). As shown in Figure 5, the August 2002 arsenic concentration in the Mine Adit(Location 3A) discharge (700 Mg/L) is the highest concentration observed in the three yearsof monitoring. The concentration at the base of the log dam was also higher than in recentevents (Figure 5). However, all of the surface water results still fall in the same generalrange as those observed during the RI and subsequent quarterly monitoring (Table 3-5).
Figure 6 shows the trend of arsenic concentrations in surface water with distance for severalmonitoring events. The graph shows arsenic concentrations starting from LCC above themine and continuing through the downgradient LGC sample. Figure 6 shows that theconcentrations in the last two August monitoring events are generally higher than thosedetected during the RI monitoring. At the Lava Cap Mine site, arsenic concentrations insurface water are generally higher during the summer months because most of the water inthe LCC/CC drainage is from mine discharge as other surface water sources are dry. Thefollowing table lists the field-estimated surface water flow rates in cubic feet per second (cfs)at each of the sampling locations during the August 2002 sampling event.

As has been the case for the last several quarters, the unfiltered result from location 19B (atthe base of Lost Lake Dam) is much higher than the filtered result (Table 3-5). The highconcentration in the unfiltered sample is caused by the presence of extensive solids (ironbacterial growth) at this sample location.
Samples collected from the northern (16B) and southern (16C) lobes of Lost Lake containedarsenic at higher concentrations than most previous events, ranging from 32 ng/L (16Cfiltered) to 97 |ig/L (16B unfiltered). At location 19A, just above the confluence with LGC,

the unfiltered arsenic concentration was 25 |ig/L, which is consistent with most prior events(Table 3-5). The arsenic concentrations detected in the downstream LGC location 19M werelower than those detected in the May 2002 event (see Table 3-5) with an unfUtered arsenicconcentration of 3.1
The reference location sample collected in Clipper Creek (2G), above the confluence withLCC, had arsenic at 3.8 Hg/L in the unfiltered sample. This is higher than any of theprevious surface water reference location samples (see Location IDs starting with 1 and 2 onTable 3-5). It should be noted that the filtered sample was non-detect for arsenic at 0.5

Conclusions
With the exception of some minor deviations, all activities planned in the FSP, DraftAddendum No. 3, were performed during August 2002. A few of the sample locationsidentified in the Sampling Plan were again modified for the quarterly events in an attemptto collect samples from a broader range of residential wells.
Arsenic was detected in surface water, monitoring wells, and residential wells both at theLava Cap Mine and downgradient of the mine. The highest concentrations detected were inmonitoring wells completed within the tailings of the Deposition Area where concentrationsranged as high as 1,700 Mg/L. Arsenic concentrations in surface water were greatest at themine discharge adit (700 Hg/L). This is the highest concentration detected in adit samplescollected over the last three years (Figure 5). The arsenic concentration at the downstreamLGC sample location remained low (3.8 Hg/L) and was actually several parts per billionlower than recent quarterly events. The residential well with the highest reported arsenicdetection was again well HAS, located just downgradient from the mine property, wherethe concentration was 140 ptg/L.
The discovery of matrix interference problems affecting the arsenic analyses performed bythe EPA Region 9 laboratory provides an explanation for the unexpected detections ofarsenic in residential wells earlier in 2002. The arsenic concentrations originally reported bythe EPA laboratory for the August 2002 were again elevated and consistently higher thanthe split sample results from a CH2M HILL-subcontract laboratory. However, afterdiscovering the interference problem and identifying a solution, the EPA laboratory re-analyzed all of the lower concentration samples (those with arsenic reported below 30Hg/L). The arsenic data from the re-analysis are included on the tables (shown in bold onTables 3-3,3-4, and 3-5) and appendixes of this report. It should also be noted thatperformance evaluation (PE) samples were also sent "blind" to the EPA laboratory and thesubcontract laboratory and both were able to accurately produce the expected results.
To verify that the EPA Region 9 laboratory had corrected the interference problems, splitsamples were collected during the November 2002 quarterly monitoring event from allresidential wells included in the monitoring program.
Both filtered and unfiltered metals samples were collected from all surface water andmonitoring well sample locations. At two monitoring wells and several surface waterlocations, the arsenic results were considerably higher in the unfiltered samples. Thesedifferences are likely caused by elevated solids in some groundwater and surface water atthe site.
The arsenic concentrations detected in surface soil around the rental residence at the minelocated furthest from the waste rock/tailings pile source areas ranged from 100 to 300mg/kg. These concentrations are above those found in reference areas (which are generallyless than 30 mg/kg), but are considerably lower than the surface soil concentrations aroundthe two rental residences located closer to the waste rock/tailings pile area (EPA, 2001b).
