PARTICIPANT INFORMATIONWHAT YOU NEED TO DO:
1. Register to participate with your local Eastern Harley-Davidson Dealers Association Dealer, HOG club or motorcycle club. ALL MOTORCYCLE BRANDS ARE WELCOME.
2. Start collecting donations from co-workers, business contacts, friends, family and neighbors. (If your place of employment has a matching gift program, ask to have the dollars that you raise matched to increase your overall donation.)
ADULT PARTICIPANTS • Collect a minimum of $75 for Saturday only participation • Collect a minimum of $30 for Sunday only participation • Or, to attend BOTH Saturday and Sunday, collect a minimum of $75
CHILDREN AGES 3-12 • Collect a minimum of $40 for Saturday only participation • Collect a minimum of $20 for Sunday only participation • Or, to attend BOTH Saturday and Sunday, collect a minimum of $40

3. By April 28, 2012, bring your packet including the completed envelope information and signed waiver, collection roster sheets and contributions to your EHDDA dealer or club coordinator.
• Place all donations in the participant envelope and make sure that all donations are converted to a check made payable to MDA.
• Pick up your official participant materials at the time of your packet turn-in. This will include a complete listing of event activities for May 5 and/or May 6, 2012, wristband for admission, food tickets, and incentive tickets for redemption at the event. Information about the Ride from your dealership to the event grounds will also be included.
• If you are not able to attend the event, return your completed packet to the EHDDA dealership or have a friend return your packet at the event to claim your incentives.

An MDA-funded research team identifies the gene for Duchenne and Becker muscular dystrophy.
For the last 24 years, both Ride for Life and the Muscular Dystrophy Association have dedicated themselves to conquering neuromuscular diseases. Here is a quick glimpse of some of their biggest milestones.
MDA’s first Ride for Life included high flying Malcom Forbes and his close friend Elizabeth Taylor.
MDA-supported researchers awarded the 1992 Nobel Prize in Medicine for their contributions to the discovery of a key regulatory process functioning of all cells.
Ride for Life 5 pushes the five-year total to over $1 million - $1,053,651!

The value of the prize is taxable income to the prize winner, and the prize winner is responsible for any and all federal, state, and local taxes associated with the prize. The fair market value of the prize (less the cost of the raffle ticket) exceeds $5,000, therefore, the winner must pay federal income tax withholding equal to 25% of the fair market value of the prize (and, if required, state income tax withholding) before receiving the prize.

ABBEY UMALI MDA’S NATIONAL GOODWILL AMBASSADORAbbey has been MDA’s National Goodwill Ambassador since 2008. She is 13 years old and has a form of Charcot-Marie-Tooth disease (CMT) that causes lack of coordination and balance along with muscle weakness. Abbey walks with the help of leg braces. She and her family travel throughout the United States to spread the word about MDA and its mission to find treatments and cures for muscular dystrophy and related diseases. Abbey is extremely outgoing and active; she loves to sing, dance, swim, ride her bike and read. She is very excited to participate this year and can’t wait to see everyone again for the 25th Anniversary of Ride For Life!
