הצגת מקרה ). מעקב IVF בהריונה הראשון (30תאומים, בן ובת, נולדו לאם בת
עקב יתר לחץ-דם. מספר שעות 32הריון תקין. אושפזה בשבוע טרם הלידה היתה ירידת מים. ילדה בניתוח קיסרי.
.10 ו-9 גרם, אפגר 1600הבן, מצג ראש, משקל , נזקקה 9 ו-8 גרם, אפגר 1825הבת, מצג ראש, משקל לידה
, אינטובציה כשעה לאחר הלידה, RDSלמעט חמצן.סבלה מ-קיבלה סורפקטנט, הונשמה למשך יממה.
הבן החל לקבל כלכלה (חלב אם) ביום השני לחיים. ביממה הרביעית היו ערכי גלוקוז גבוהים בסטיק. בדיקה גופנית – תקינה,
בדיקות דם – תקינות, תרביות דם עקרות. עקב היפרגליקמיה עקשנית הוחל במתן אינסולין, למרות זאת ערכי גלוקוז המשיכו להיות גבוהים. ביממה הששית התינוק נראה חולה מאוד והחל
לפרכס.
מג%, 12 גלוקוזCSF -ב נעשה ברור זיהומי מלא – שלילי. מג%.39 מג%, גלוקוז במעבדה 234בסטיק בדם –
הבת החלה לקבל חלב אם ביום השלישי לחיים. ביממה החמישית הופיעו ערכי 13%סוכר גבוהים בסטיק. בהדרגה נראתה פחות טוב: חוורת, אפטית, ירדה
ממשקל לידה. קיבלה עירוי של גלוקוז בריכוז נמוך עם אינסולין. עקב החמרה כללית במצב ואפנאות הונשמה. נעשה ברור זיהומי – שלילי.
מג14 מג%, גלוקוז במעבדה – 196 מג%, גלוקוז בדם בסטיק 1סוכר CSFב-.%
פער בין בדיקת הסוכר במעבדה ובסטיק:-Galactose 1הסטיק בודק חומרים מחזרים ולא גלוקוז בלבד. נשלח דם ל-
Phosphate Uridyltransferase .ונמצאה פעילות נמוכה מאוד של האנזים Classical Galactosemia אבחנה:
הוחל במתן גלוקוז בריכוזים עולים, הופסק אינסולין, בהמשך קבלו כלכלה נטולת galactose.

Maternal-fetal hormone relationship
• Glucose concentration gradient – facilitated diffusion
• Umbilical venous plasma glucose concentration is ~60%-80% of mother’s
Fetal uptake of glucose is regulated by maternal glucose concentration
Intrauterine glucose metabolism
1st trimester – organogenesis2nd trimester – exponential growth3rd trimester – growth +storage
Insulin: stimulates glucose incorporation into liver glycogen, acts as growth hormone (anabolic)
• During the first 2 hours of life, glucose levels fall and followed by rise to a steady state level by 2-3 hours of life
• This rise is associated with increase of counter-regulatory hormones, hepatic release of glucose (glycogenolysis) at 4-6mg /kg/min
• Fat becomes significant contributor (~50%) to energy metabolism (ketogenesis)
• By 4-6 hours of life, gluconeogenesis begins, however, enzyme activity reach adult levels at ~1-2 weeks of age
• Breast milk provides lactose but not more than 50% of glucose utilization rate
Discontinuation of maternal substrates and nutrient supply :Need to mobilize glucose or other substrates to meetenergy requirements
Therefore, neonates are markedly dependent on active glucose metabolic cycles to maintain normal glucose levels
Extrauterine adaptation
Glucose kinetics in the newborn
Rates of glucose production and utilization in the newborn are between 4-6 mg/kg/min
Rates are higher in the newborn compared with adults due to:* Higher metabolic rate* Higher brain-to-body weight ratioof the newborn brain
After 2 days, with the introduction of feeding, the normal infant maintains plasma glucose at 45-70 mg/dl(2.5-6 mmol/L)
Definition/Diagnosis – Controversial!!
No specific value is quoted for potential clinical signs or brain injury. Most investigators would consider plasma glucose concentration < 2.6 mmol/l
or 47 mg/dl to be low in the first 72 hours of life .
The definition is the same for term and preterm infantsATTENTION TO LATE PRETERM INFANTS (34-37 wks)
Hypoglycemia= glucose delivery inadequate to meet glucose demand in a specific neonate(consider age and overall infant status)
Glucose concentration is only an indicator of glucose insufficiency

Management of Neonatal Hypoglycemia

Mechanism of injury:* Neuronal membrane breakdown and increase in glutamate* Aspartate and glutamate are excitatory amino acids* Increase in glutamate-receptor activity (N-Methyl-D-Aspartate, NMDA-receptor)* Increase in mitochondrial free radicals and initiation of apoptosis * Binding of glutamate results in Ca++ influx

Consequences of Neonatal Hypoglycemia: Clinical Outcome

• Increased cerebral blood flow• Follow-up studies have methodological flaws,

Neonatal Hypoglycemia: Conclusions
Maintenance of normoglycemia is a complicated adaptive process in the newborn
Recognition of newborns at risk and high indexof suspicion are necessary for early diagnosis/intervention
If persistently hypoglycemia is found, move to a second line agent (with glucose infusion)
Severe and profound hypoglycemia can cause devastating neurological sequelae

• IMPORTANCE OF EARLY BREASTFEEDING IN GESTATIONAL/TYPE I DIABETES
• The exposure of infants to second-generation sulfonylureas (eg, glipizide, glyburide) through breast milk is expected to be minimal, based on the limited data available.
• Women with type 2 diabetes treated with sulfonylureas should not be discouraged from breastfeeding.
• The benefits of breastfeeding greatly outweigh the risks of these medications, if any. The baby should, however, be monitored for signs of hypoglycemia.
