• 24/7 access center: Same day service (“23 hour”)
• Assess for appropriate level of care
• Safely begin MAT induction process
• Seamless transition to next level of care
Access Point
• Currently in phase one
• Approval and medication to treat opiate use disorder only
• Phase two within two months
• Will serve full range of Substance Use Disorders
Patient criteria
• Individuals currently in withdrawal
• Individuals for whom withdrawal is imminent
• Individuals with medical or psychiatric issues have an increased risk during withdrawal and may require extended monitoring and medical support
Exclusionary Criteria
• Acute injury or medical problem requiring immediate medical care
• Unexplained or sudden alteration in mental status
• Acute suicidal or homicidal ideation
• Lack of physical functioning/mobility
Withdrawal management
• Safety of patient is paramount
• Goal is to safely suppress opioid withdraw with buprenorphine (Subutex)
• Recovery Support team (nurse, physician or NP, peer specialists, care coordinators) work together to monitor and assess need for next level of care
• Medical staff use continually assess withdraw symptoms and vital signs

• Provide continuous encouragement and support
• Available to facilitate warm hand off either upon entry or discharge to next level of care
Discharge options
• Outpatient withdraw management (10 to 28 days)
• Outpatient maintenance (7 days per week/ buprenorphine or methadone)
• Other options driven by medical/psychiatric or social needs
