Since 2016, Coordinated Entry has

been developing a systematic

infrastructure to provide

connections for persons

experiencing homelessness to

housing. With the creation of

Coordinated Entry, individuals can

access appropriate housing

services more efficiently, with

fewer phone calls and less

screenings.

Through one entry point, we

assess individuals who are

experiencing homelessness or at

risk of homelessness, and work to

determine the best resources and

services for their needs

Supportive Housing Programs

The Coordinated Entry system for North

Central Florida works with multiple

supportive housing programs including

permanent supportive housing and

rapid re-housing.

Once Coordinated Entry is contacted,

we will work to help determine the type

of housing programs for which you may

be eligible. Housing placements are

based on assessed need. As housing

becomes available, we will work to

move the person with the highest need

into that housing.

Please note that housing is never

guaranteed and persons seeking

housing are encouraged to continue

seeking options on their own.

Emergency Shelter

When someone contacts Coordinated

Entry for a shelter bed, we will work to

explore other options, complete an

initial assessment for housing

placement, and offer referrals to other

services to address immediate needs.

Coordinated Entry does not fill these

beds but we can let you know which

shelters have openings and their

requirements for admission.

There are a limited number of

emergency shelter beds in North

Central Florida and they are filled on a

first come, first served basis.

The Process of Triage

and Assessment

When someone contacts Coordinated

Entry, we first see if there is a way to

keep them in housing. If that is not

possible, a VI-SPDAT, which is a triage

tool, is conducted. It highlights areas

of need, helping to inform the type of

support and housing intervention that

may be most beneficial to improve

long term housing intervention and

outcomes. The VI-SPDAT does not

make decisions, but helps to

determine the order in which people

should served.

Services to eligible persons is

provided without regard to race,

color, national origin, religion, sex,

age, familial status or handicap.
