You need not satisfy another WaitingPeriod for a continuing illness or injury.For a “To Age 65” Benefit Period, in moststates up to 12 months are allowedbetween periods of Disability. Otherwise,up to six months are allowed.

If the Waiting Period has not been satisfied,periods of recurring Disability can beaccumulated to satisfy the Waiting Period.

� If you participate in a rehabilitationprogram while you are Disabled, we maypay the expenses of the program.

�After 90 days of Disability, we will waiveany premium that becomes due while youare Disabled.

�The Dividends provision has been deletedand replaced by the Endorsement on page45 of this booklet.

�The Conversion Privilege at ExpirationDate provision has been deleted andmodified by the Endorsement on page45 of this booklet.

�You should contact your MassMutualrepresentative or MassMutual’s HomeOffice within 20 days, or as soon asreasonably possible, whenever thepossibility of a claim exists.

�You must file proof of your Disability nolater than 90 days after the end of eachperiod for which benefits are payable.

We have the right to require proof of yourcontinuing Disability.

�For each Disability caused or contributedto by a Mental Disorder, you will receivebenefits for 24 months. However, if you arecontinuously confined in a Hospital fortreatment and you are under a Doctor’sCare, the 24 month limitation will not apply.

�Pre-Existing Conditions may be covered byyour policy if fully and accuratelydescribed in the application and notspecifically excluded by name ordescription.

�You may apply for additional benefits at any time while your policy is In Force.We will require a new application and current proof of insurability.

�You may receive benefits if you arePartially Disabled.

� If you are still under a Doctor’s Care, havereturned to your regular Occupation andyour current Disability continues to be afactor in producing at least a 20% Loss ofIncome, you may be considered PartiallyDisabled.

� If you have also purchased the OwnOccupation Rider, please note that the

definition under this section has beenmodified. Please review the OwnOccupation Rider language for details.

�During the first 12 months of Disability,your benefit will never be less than 50%of your Partial Disability Monthly Benefit.If your Loss of Income exceeds 75%, youwill receive the full Partial DisabilityMonthly Benefit.

� If you continue to have a Loss of Income ofat least 20% or more, after you are back towork in your Occupation, you will receive aRecovery Benefit for as long as you candemonstrate that the Loss of Income isrelated to your Disability. Recoverybenefits are payable to the MaximumBenefit Period for Partial Disability.

�An important feature of this rider is thebuilt-in adjustment to your Pre-disabilityIncome. After 12 months of PartialDisability, your Pre-disability Income willbe increased to reflect the increase in theConsumer Price Index (CPI). The increasewill never be less than 5%.

�The Own Occupation Rider provides extraprotection in the event you are unable,because of disability, to perform thesubstantial and material duties of youroccupation and you are working in a new occupation.

�This rider helps to offset the effects inflationwill have on your Disability benefits.

�Only Total Disability Benefits and benefitsunder the Extended Partial DisabilityBenefits Rider and the Social InsuranceRider, if In Force, are subject to an increase.

Increases begin after 12 months ofDisability or the full Waiting Period,whichever is greater.

�Benefits will continue to increase whileyou are Disabled up to the MaximumBenefit Period.

�Benefits increase at a rate of 3%compounded annually.

�Upon your return to active full-time work, you have the opportunity to purchase theincreased benefits provided by this rider without evidence of Insurability. Proof of youractive employment is required.

�This rider is designed to provide disabilitybenefits for a short period of time duringthe waiting period of employer-paid salarycontinuation plans or LTD plans with 180day or longer waiting periods.

�All benefits and definitions in the DisabilityBenefits section of your policy apply tothis rider.

�All benefits and definitions in the ExtendedPartial Disability Benefits Rider, if includedin your policy, apply to this rider.

�This rider provides Partial Disabilitybenefits and is designed to supplementthe benefits of your group insurance plan.

�You may be considered Partially Disabledunder Coverage A if you have returned tothe Occupation you had on the day youbecame Disabled and your current Disabilitycontinues to be a substantial factor inproducing at least a 20% Loss of Income.

During the Waiting Period, you must bePartially Disabled and have an IncomeLoss of at least 20% or you must be TotallyDisabled for at least 30 days.

�Under Coverage A, during the first 12months of Partial Disability, your benefitswill never be less than 50% of yourMonthly Benefit for this coverage.Thereafter, benefits are based on thepercentage of Income Loss.

�You may be considered Partially Disabledunder Coverage B, if you are working ata new Occupation for which you arereasonably suited, and your currentDisability causes at least a 20% Loss ofIncome, and you are under a Doctor’s Care.

The benefit under Coverage B is based ona percentage of the Income Loss.

�Coverage C is no longer available underthis Rider.

� If you are not Disabled, you can increasecoverage under this Rider regardless ofyour health or Occupation.

You may apply to increase coverage underthe Option feature once a year during the90 day period prior to, and including theOption Date.

�Coverage under this rider will increase by5% while you are Disabled. Increases willbegin after the later of 12 months ofDisability and the full Waiting Period. Theincreases are on a compound basis.

�This rider allows you to purchaseadditional Disability benefits regardless ofyour health or nature of your Occupationas long as you are not Disabled.

�You may apply for additional Disabilitybenefits under this rider once each yearup to age 60. You must apply during the90 day period prior to and including yourOption Date.

�Unless you request us not to, coverageunder certain riders will be increased.

�Prior to your 51st birthday, you may applyfor an increase up to the full amount of theinitial Total Disability benefit under yourpolicy or up to the total additional benefitsunder this rider, whichever is less.After your 51st birthday, you may apply for anincrease up to the lesser of one-half theinitial Total Disability benefit and the totaladditional benefits under this rider. Increasesare based on your financial qualifications.

�Premiums for additional benefits are basedon your age and the table of premiumrates in use at the time of the option.

�Up to age 55, you may increase the TotalAvailable Benefits under this rider. Proofof Insurability will be required.

�This rider provides a Disability benefit thatis reduced by benefits provided by OtherBenefit Programs.

�Other Benefit Programs include Worker’sCompensation, government sponsoreddisability programs, and benefits fromgovernment retirement systems.

� If no benefits are payable from Other BenefitPrograms, you will receive the MonthlyBenefit for this rider. If benefits are payablefrom one or more of the Other BenefitPrograms, the benefit under this rider willbe reduced by the total amount payable.

�No benefits will be paid under this rider ifyou are receiving Social Securityretirement benefits.

�The monthly benefit under this rider will beused to determine the benefit under thePartial Disability Benefits Rider or theExtended Partial Disability Benefits Rider ifIn Force on your policy.

�We will require proof of benefits payableunder Other Benefit Programs and proof asto whether you are receiving those benefits.

We also have the right to pursue anyappeals process for benefits for which youare entitled under Other Benefit Programs.We will pay for any appeal we may pursue.

�This rider allows you to annually increaseyour benefits automatically without Proofof Good Health, other than proof that youare not Disabled.

�Unless you request us not to, coverageunder certain riders will be automaticallyincreased as well.

�Benefits will automatically increase oneach Policy Anniversary provided you arenot Disabled.

�You may refuse an increase. If you refusetwo consecutive increases, we will cancelthis rider.

�After five consecutive increases, you mayapply to renew this rider provided four ofthe five previous increases are In Force.

�The rider provides benefits to an investmentfund that will be available at retirement tosupplement retirement income.

�These additional definitions apply tothis rider.

�You will be sent the necessary paperworkto establish a Trust Account with TheMassMutual Trust Company, FSB to receivebenefits under this rider.

�Benefits are payable to the Trust andaccumulate to the maximum benefit periodof this rider.

�Contributions to the Trust Accountwill automatically increase by 3% after12 months of benefits if the policy includesthe optional Cost of Living Adjustment(COLA) Benefit. This amount will increaseevery year, compounded for as long asyou are disabled.

�After a period of disability when monthly benefit increases were paid under the COLA rider,you can buy additional coverage without medical evidence.

If you are not disabled, you can periodically increase coverage under this rider.

�CATASTROPHIC DISABILITY OR CATASTROPHICALLY DISABLED – The Insured may sufferfrom a Catastrophic Disability due to a Presumptive Disability or a Total Disability whichresult in either the inability to perform Activities of Daily Living (ADLs) or a Severe CognitiveImpairment.

�WHEN WE PAY: The Waiting Period and Maximum Benefit Period are specific to this Rider.No benefits accrue during the Waiting Period, unless the disability benefit qualifies based ona complete and irrecoverable Presumptive Disability.

�The option of accumulating dividends at aminimum of 5% interest, as well as payingpremiums with accumulated dividends, isno longer available. Dividends are notguaranteed.

�The “Convertible at Age 65” option hasbeen replaced with a “conditionallyrenewable” option at age 65.

�Based on premiums paid, a credit willaccumulate that may be applied to a LongTerm Care (LTC) policy purchase.

�The credit may be applied to a Long TermCare Policy (LTC) for the Insured, or amember of the Insured’s immediate family.

This endorsement completely replaces the Presumptive Total Disability wording in both theDefinitions section and the Disability Benefits section of your policy.
