5 For different design options that may alter long-term energy consumption, have you performed and provided to Duke FMD a life cycle cost analysis?

6 Have you participated in a project Sustainability Charrette?

7 Have you reviewed the Energy Modeling Information Form with Duke FMD?

8 If applicable, have you provided input to the preliminary LEED scorecard for the project?

Construction Phase

9 At the 100% Construction Document (CD) phase, does the designed building comply with the current version of the North Carolina Energy Conservation Code?

10 At the 100% Construction Document (CD) phase, does the designed building meet the target energy & water efficiency improvements over the baseline building?

11 If submitting to USGBC, what is the expected level of LEED certification/

12 Has the final Energy Modeling Information Form been submitted to, discussed with, and approved by FMD?
