Community Connections Grant Program

• Grants of up to $5,000 or $7,500• Regular grants of $5,000• Challenge grants• A $5,000 grant + a challenge grant of up to $2,500

with a1:1 match

• Best and promising practices• Increase access to care

Challenge Grants

• One $5,000 payment• One payment of up to $2,500• Matches identical amount raised between July

2013 and October 2014

• Sources of the match can include• Grants from other foundations• Private donations• Fundraising events• Finance Fund loan

Who Should Apply

• Safety net healthcare providers• Primary care, dental, behavioral, etc.

• Social service organizations• Schools• For projects that help children access services

• Government or quasi-government agencies• Single agency or collaboration• Serve at least 1 of our 36 counties
