Integrated care: a revolution in UK health and care practice?

Supporting people tolive the lives they want

The team understands what’s important to me.

I get information I need, at the right time, to make the choices that are right for me.

I feel in control.

Those around me, and looking after me, get help too.

I only have to tell my story once.

I can live the life I want.

Living Well - supporting people to live the lives they want • Championing a new way of working that enables everyone to live

the lives they want to the best of their abilities

• The focus will be on people, their stories, their needs and their aspirations

• We recognise that everyone is different so our response will vary from person to person

Living Well – supporting people to live the lives they want

• Championing a new way of working through a joined up approach with health, social care, the voluntary sector, police and the clergy

• People told us to focus on their stories, needs and aspirations

• Everyone is different – we will agree goals to help them lead the lives they want with services helping them to do this
