Transmigration of mandibular canines across the midline is rare.The patient was asymptomatic.
We considered a canine to be transmigrant when one-half or more of its length was seen
to have crossed the mandibular midline. We based the diagnosis on the clinical findings, particularly on the panoramic radiographs.

The etiology of transmigration is unknown; however, abnormal displacement of
the tooth or deviation during development is the most commonly accepted
explanation.

The migrating canine showed no evidence of resorption or pericoronal
radiographic changes suggestive of cystic degeneration.
Treatment, in part, depends on the canine’s radiographic position and clinical
manifestations.

The combined surgical- orthodontic treatment of impacted canine has been divided into 3 step:
1) FIRST ORTHODONTIC STEP: preparation of anchorage;
2) SURGICAL STEP: a) localization;
b) surgical exposure;
c) surgical attachment of an orthodontic button or bracket.
3) SECOND OTHODONTIC STEP: a) application of direct traction;
b) orthodontic alignement.

Mupparapu proposed a classification of mandibular canine transmigration based on the migratory pattern
and position of the canines in the jaw:
- Type 1: a canine impacted mesio-angularly across the midline, labial or lingual
to the anterior teeth;
- Type 2 :a canine horizontally impacted near the lower border of the mandible
inferior to the apices of the incisors;
- Type 3: a canine erupting on the contralateral side;
- Type 4: a canine horizontally impacted near the inferior border of the opposite
side
- Type 5: a canine positioned vertically in the midline. with the long axis of the tooth crossing
the midline.

radiographs.

- The patient finished treatment with a normal and stable occlusion between the maxillary and
mandibular arches and an adeguate width of attached gingiva;
- All teeth were successfully erupted;
- Transmigration is a rare dental condition that can be treated successfully with a collaborative effort
from several dental disciplines.
