Custom 360 for client
Given below is an outline of the design of the Questionnaire which has been proposed for use for the 360 feedback surveys for the Participants followed by the detailed Questionnaire. There will be two subjective questions where the respondents will be able to write subjective comments in response to these questions and the responses will be presented in the 360 report anonymously
Competencies Covered
The proposed Questionnaire design will cover the following 6 Competencies. The Questionnaire will consist of 4 statements under each competency.

Scale
We propose to use the following five point scale:
Never – Seldom – Sometimes - Often – Always
An additional option of “Don’t know/ Not Applicable” will be included in addition to these five scale points.
Subjective Questions
There will be 2 subjective questions included at the end of the 360 feedback survey. Respondents will be able to write subjective comments in response to these questions and the responses will be presented in the 360 report anonymously.

Subjective Questions:
1. What would you like this person to start doing more going forward? For example, “you should communicate changes to plans earlier” or “please provide more regular and honest feedback to your team members”. Your comments will be presented anonymously.
2. What would you like this person to do less or stop doing going forward? For example, “please stop coming late for meetings” or “Don’t make fun of other colleagues in team meetings”. Your comments will be presented anonymously.
