July 01, 2015 Mr./Ms.: Chairperson/CEO or Director or Partner(s), Chairperson of Nomination Committee I would like to be appointed as the Vice President of Operation, a Senior Executive position, of the company and my term shall start and terminate upon my resignation or at the prerogative or decision of the President or Board of Directors or Chief Executive Officer. I shall be paid of Salary amounting to 90,000 pesos monthly or 45,000 pesos every after the 15 th and 30 th day of every month (negotiable) subject to appropriate Withholding Tax and other mandatory withholdings applicable to Salaried Senior Executive. I shall be entitled, if applicable, to appropriate executive benefits and incentives such as company car driving, company house stay and/or company car with driver. I shall be entitled, if applicable, to appropriate annual bonus to be determined by executive committee. I shall not be covered of regular hours of a day’s work. I shall be expected to work at least 4 hours daily, Monday to Friday. Being the Vice President of Operation I have the following responsibilities or functions: 1. To oversee the operation of the company including the sales or revenue growth, expansion and to make recommendation to the CEO or President or other Senior Executive on improving the sales or revenue. 2. To approve contracts to be entered into and to make appropriate recommendation to the President or CEO or other Senior Executive. 3. To lead the executives (s) meeting and to preside the meeting if necessary. 4. To oversee the management of branches by other executives including advertisement initiatives. 5.
