A Questionnaire is an instrument for data collection.

The questionnaire was invented by Sir Francis Galton.

It consists of a series of questions.

The purpose of questionnaire is to gather information

from respondents.

1. Deciding which questionnaire to use.

2. Question wording & structure.

3. Sequencing and layout decisions.

4. Piloting the questionnaire.

5. Obtaining a high response.

6. Questionnaire design checklist.

One need to decide if he/she intend to construct a :

Closed-ended questionnaire.

Open-ended questionnaire.

Combination of both.

Use simple question.

Be as specific as possible, avoid being vague.

Avoid leading and loaded questions.

Avoid double-barreled questions.

Avoid questions that are too difficult to answer.

Always begin with simple, non-offensive

questions.

Pay attention to whether certain questions are

relevant for every respondent or only some

Give a very clear instructions for completing

questionnaire.

Use appropriate fonts and white space to avoid

crowding.

Pretest is done to ensure that the questionnaires meetthe researchers expectation in terms of informationthat will be obtained.

The objective of pretest is to identify and correctdeficiencies of the questi0nnaire.

It also reduces ambiguities.

Though it is a long and laborious process, it isincredibly important to construct an effectivequestionnaire.

To obtain a high response from the respondent we may

Translate the questionnaire into another language if

necessary.

We may assure the respondents that data will be

used for completing the survey only.

We may also inform the respondents who the

research is for & what will happen to the results.

The questionnaire should be:

1. As short as possible.

2. A mix of question formats.

3. Easy to answer questions should be followed by

comparatively complex questions.

4. Non offensive and shouldn’t cause frustration,

sadness or anger.

5. Questions should be simple with straightforward

directions.
