3. Applic€nts shall be evaluated based on the following DepEd Orders:

4. The chair of the commitlee shall submrt both sott and hard copies oI the resulG ofthe validation and

evaluatisn of documents using the attached templale: Phase b9 rominded that the RQA for JHS

must b€ by leaming area and SHS by group or track offsrlng. Only tlro Dlstrlct Superylsor wlllsubmit and emall t re hard and soft copi€s of Di$tict ROA ior Elem to SHS.

5. Teacher-applicanis who have taken lhe EPT in 2016 or later, and underwent demo teaching and

inteMew may opt not to retake the lest and undergo demo leaching and interview butthey are advised

to updale thei. documents However, those who opt to undeGo lhe hiring process will be allowed bul

lhe new score will be considered flnalby lhe PSB. The distnd PSB shall be responsible lor the encoding

of scores for old and new applicanls.

6. Allteacher- applicants are required lo submit one (1) authenticated copy of their pertine.{ docum6nts

to th€ school and district level committee with writlen application letters addressed to the Schools

Division Superintendent, table of contenls supporled by thetollowing documents arranged accordingly:

Certjfied copy of PRC professional identificaiion card or a PRC certmcdion showing the

teache/s name, LET rating, and other information r€corded in the PRC offics;

Certified copy of ratings obtained in the Licensure Examination lor TeacheB (LET) /Professional Board Examination for Teachers (PBET);

Certified copy of Transcript of Records (Compulation of General Weighted avBrage (GWA)

should be included in the submission of documents):

Copies of SeNice Records, Pertormance Raling, and Sclool Clearance for lhos€ with

teaching experienca lf unavailable, ihe applicant musl submit a justifiGation lorunavailability.

Certificates of specialized training programs attended, if any;

Cefffied copy of the Vote/s lO and/or any proof of residency as deemed acceptaHe by the

School Screening Committee;

NBI Clearance: and

Omnibus cerlification of aulhenlidly and veracity of all documenls submitted, signed by the

toacher applicant.

7. All leacher- applicants assume full responsibility and accountability on the validity and authenticity

o{ the submitted pertin€nt documents as evidenced by the Omnibus Certification of authenlicity duly

signed by lhe applicantrs. Any violation will automalic€lly disqualify them from the selection process.

8. Schedule and Venue on the Admission of English Proficiency (EPT) will be announced on a sepaEte

Memorandum

9. The tinalization of the Regjstry of Quelilied Applicants (RQA) will be facilitated by lhe Division

Selection Committee Chairman, Members of the PSB; Educalbn Program Supervisors, Division

President of School Heads Associalion, Division PTA President and PPSTA represent live on March

20't9.

10. Teacher-applicants still working in the privale schools shall coordinate with the PSDS in their

respeclive distrid regarding their schedule for evalualion to ensure thal their cunent work place will

nol be aff€cled on the given schedule.

11. Travel and incidental expenses of all concemed during the selec-tion and ovaluaiion of qualilied

applicants shall be charged agsinst MOOS loc€l funds subjecl to usual accounting and auditing rules

and regulaiions.
