The war on Coronavirus, which Prime MinisterNarendra Modi had promised to win for us in 21days, gets more hopeless by the day with the dai-

ly increase in caseload crossing 90,000, a figure higherthan all other nations. India added the last million to thenumber of infected people in 13 days, a record of sortsin pandemic history the world over. With 41 lakh infec-tions, India is set to become the nation with the secondlargest number of Covid-19 patients, overtaking Brazilsoon. The nature of the pandemic has also been shifting. Those states which fared better in early stages are nowbecoming increasingly vulnerable. The southern statesof Telangana state, Andhra Pradesh, Tamil Nadu andKarnataka see no let-up in new cases; even Kerala hasstarted reporting cases upwards of 2,500 every day. Del-hi is reported to be witnessing a relapse with the num-bers increasing. Telangana state’s finance minister is thelatest high-profile person to contract the virus. The saving grace till now has been the case fatality ratewhich has been quite low but experts fear that the situa-tion could change as the pandemic moves towards thevillages from cities and towns where the health infra-structure is in better shape. It is in this context that thecriticism of Congress leader and former finance minis-ter P. Chidambaram must be looked at. He has pointedout that India is the only country that has not benefitedfrom the lockdown. When lockdown was declared, Indiahad less than 500 cases but the number rose to 5.6 lakhwhen it ended on June 30. The economic cost the nationis made to pay is going to be humongous if it cannot ar-rest the spread of the pandemic. The nation’s GDP ex-perienced a historic contraction by 23.9 per cent in thefirst quarter. It will be cold comfort to think that the pe-riod was spent mostly in lockdown. This quarter, whichstarted with half-a-million infections, will see the num-ber multiplied several times when it ends. It is time thegovernment explained to the people what its game planwas when it declared the lockdown and set a deadline towin the war, and how it all went awry. The lockdown hadnot only shut down the economy completely, it had alsoput people to untold miseries. Millions of guest workerswere made to trek their way back home in villages fromthe towns and cities and several of them fell and died bythe wayside. There was little help coming from the gov-ernment; even the courts refused to see the unfoldinghuman catastrophe in the earlier days. It will be suicidalfor the nation to return to lockdown; instead, it mustlaunch a massive awareness campaign on the pandemicand educate the people on the need of social distancing.

Literature & HealthCellular roadmaps project body'scoronavirus vulnerability: Study

Arecent research from Cornell Uni-versity developed potentialroadmaps for how the coronavirus

infects organs and identifies what molec-ular factors could help to facilitate or re-strict infection. "The data suggest thatCOVID-19 is not just a respiratory dis-ease," said lead author Cedric Feschotte,a molecular biology professor. "It's muchbroader than that and has the potential toaffect many other organs. Our analysessuggest that there is a widerange of cellular vulnerabili-ties."

The study maps the ex-pression of 28 human genesdubbed 'SCARFs' - SARS-Cov-2 and Coronavirus-Asso-ciated Receptors and Factors.By looking at the single-cellRNA expression of thesegenes, they can predict whichtissues and cell types are mostvulnerable to coronavirus in-fection - in both adults andembryos.

The team analysed the RNA expres-sion of healthy human tissues to developa comprehensive profile of the molecularfactors that both facilitate and restrictSARS-CoV2 infection.

Without the immune system's abilityto respond quickly, Feschotte said, natu-rally occurring restriction factors alreadypresent in the tissues represent the body'smain line of defense against SARS-CoV-2. Mapping the different entry points for

the virus also is essential for trying to pre-dict where the virus will go after it entersthe body. Moreover, by pinpointing themolecular routes of infection, other re-searchers can use those areas as targetsfor developing drugs to overcome the in-fection. The study indicates alternate en-try paths for how the virus could enter thelungs, central nervous system, and heart.Their research also supports emergingclinical data that shows SARS-CoV-2

also infects the intestines, kidney, andplacenta. They noted that specific groupsof cells within the prostate and testes arelikely to be permissive for SARS-CoV-2and may help explain male-specific vul-nerabilities. As part of this project, theteam also developed an open-access,user-friendly web interface where anyonecan look up the single-cell RNA expres-sions of SCARFs. This will facilitate easyaccess to data that will help scientistsaround the world.

COVID-19 patients cansuffer long-term lungand heart damage but,

for many, this tends to im-prove over time, according tothe first, prospective follow-up of patients infected withthe coronavirus, presented atthe European Respiratory So-ciety International Congress.

Researchers in theCOVID-19 'hot spot' in theTyrolean region of Austria re-cruited consecutive coron-avirus patients to their study, who were hospitalised at theUniversity Clinic of InternalMedicine in Innsbruck, the StVinzenz Hospital in Zams orthe cardio-pulmonary reha-bilitation centre in Munster,Austria. In their presentationto the virtual congress, theyreported on the first 86 pa-tients enrolled between 29April and 9 June, althoughnow they have over 150 pa-tients participating.

The patients were sched-uled to return for evaluationsix, 12 and 24 weeks aftertheir discharge from hospital.During these visits, clinicalexaminations, laboratorytests, analysis of the amountsof oxygen and carbon dioxidein arterial blood, lung func-tion tests, computed tomogra-phy (CT) scans and echocar-diograms were carried out.

At the time of their firstvisit, more than half of the pa-tients had at least one persist-ent symptom, predominantlybreathlessness and coughing,

and CT scans still showedlung damage in 88% of pa-tients. However, by the timeof their next visit 12 weeksafter discharge, the symptomshad improved and lung dam-age was reduced to 56%. Atthis stage, it is too early tohave resulted from the evalu-ations at 24 weeks.

"The bad news is thatpeople show lung impairmentfrom COVID-19 weeks afterdischarge; the good news isthat the impairment tends toameliorate over time, whichsuggests the lungs have amechanism for repairingthemselves," said Dr SabinaSahanic, who is a clinicalPhD student at the UniversityClinic in Innsbruck and part

of the team that carried outthe study, which includes As-sociate Professor IvanTancevski, Professor JudithLoffler-Ragg and Dr ThomasSonnweber in Innsbruck.

The average age of the 86patients included in this pres-entation was 61 and 65% ofthem were male. Nearly halfof them were current or for-mer smokers and 65% of hos-pitalised COVID-19 patientswere overweight or obese.Eighteen (21%) had been inan intensive care unit (ICU),16 (19%) had had invasivemechanical ventilation, andthe average length of stay inhospital was 13 days.

A total of 56 patients(65%) showed persistent

symptoms at the time of theirsix-week visit; breathlessness(dyspnoea) was the mostcommon symptom (40 pa-tients, 47%), followed bycoughing (13 patients, 15%).By the 12-week visit, breath-lessness had improved andwas present in 31 patients(39%); however, 13 patients(15%) were still coughing

Tests of lung function in-cluded FEV1 (the amount ofair that can be expelledforcibly in one second), FVC(the total volume of air ex-pelled forcibly), and DLCO(a test to measure how welloxygen passes from the lungsinto the blood). These meas-urements also improved be-tween the visits at six and 12

weeks. At six weeks, 20 pa-tients (23%) showed FEV1 asless than 80% of normal, im-proving to 18 patients (21%)at 12 weeks, 24 patients(28%) showed FVC as lessthan 80% of normal, improv-ing to 16 patients (19%) at 12weeks, and 28 patients (33%)showed DLCO as less than80% of normal, improving to19 patients (22%) at 12weeks.

The CT scans showedthat the score that defines theseverity of overall lung dam-age decreased from eightpoints at six weeks to fourpoints at twelve weeks. Dam-age from inflammation andfluid in the lungs caused bythe coronavirus, which showsup on CT scans as whitepatches known as 'ground-glass', also improved; it waspresent in 74 patients (88%)at six weeks and 48 patients(56%) at 12 weeks.

At the six-week visit, theechocardiograms showed that48 patients (58.5%) had dys-function of the left ventricleof the heart at the point whenit is relaxing and dilating (di-astole). Biological indicatorsof heart damage, blood clotsand inflammation were allsignificantly elevated.

Dr Sahanic said: "We donot believe left ventricular di-astolic dysfunction is specificto COVID-19, but more asign of the severity of the dis-ease in general. Fortunately,in the Innsbruck cohort, we

did not observe any severecoronavirus-associated heartdysfunction in the post-acutephase. The diastolic dysfunc-tion that we observed alsotended to improve with time."

She concluded: "Thefindings from this study showthe importance of implement-ing structured follow-up carefor patients with severeCOVID-19 infection. Impor-tantly, CT unveiled lung dam-age in this patient group thatwas not identified by lungfunction tests. Knowing howpatients have been affectedlong-term by the coronavirusmight enable symptoms andlung damage to be treatedmuch earlier and might havea significant impact on fur-ther medical recommenda-tions and advice."

In a second poster presen-tation to the Congress, MsYara Al Chikhanie, a PhDstudent at the Dieulefit Santeclinic for pulmonary rehabili-tation and the Hp2 Lab at theGrenoble Alps University,France, said that the soonerCOVID-19 patients started apulmonary rehabilitation pro-gramme after coming off ven-tilators, the better and fastertheir recovery.

Patients with severeCOVID-19 can spend weeksin intensive care on ventila-tors. The lack of physicalmovement, on top of the se-vere infection and inflamma-tion, leads to severe muscleloss.

While a potential linkbetween blood pres-sure drugs and can-

cer has been debated for morethan 40 years, the findings ofa breakthrough study re-vealed that there is no evi-dence that blood pressure-lowering drugs increase therisk of cancer.

The late-breaking re-search is presented at ESCCongress 2020. "Our resultsshould reassure the publicabout the safety of antihyper-tensive drugs with respect tocancer, which is of para-mount importance given theirproven benefit for protectingagainst heart attacks andstrokes," said study author

Emma Copland, an epidemi-ologist at the University ofOxford, UK.

The evidence for an in-creased or decreased risk ofcancer with the use of antihy-pertensive medication hasbeen inconsistent and con-

flicting. This was the largeststudy on cancer outcomes inparticipants of randomisedtrials investigating antihyper-tensive medication - around260,000 people in 31 trials.Investigators of all trials wereasked for information on

which participants developedcancer. Much of this informa-tion has not been publishedbefore, making the currentanalysis the most detailedyet. Five antihypertensivedrug classes were investigat-ed separately: angiotensin-converting enzyme (ACE) in-hibitors, angiotensin II recep-tor blockers (ARBs), beta-blockers, calcium channelblockers (CCBs), and diuret-ics. The investigators esti-mated the effect of each drugclass on the risk of develop-ing any type of cancer, of dy-ing from cancer, and of de-veloping breast, colorectal, lung, prostate and skin cancers.

Alot has been deliberatedon the transformativepotential of the recent-

ly issued National EducationPolicy (NEP). The policy thathas stood out for introducingsweeping changes in schooland higher education is a boonfor Telangana where it willimprove learning outcomes, especially for socially andeconomically vulnerable stu-dents. It is important to under-stand the performance ofTelangana in education. Theperformance grading indexused by the ministry of educa-tion ranks Telangana at 17 outof the 37 states and Union ter-ritories in 2018-19. The broad-based metrics included learn-

ing outcomes and quality, ac-cess, equity, infrastructure andfacilities, and governanceprocesses. Telangana wasranked 30th in infrastructureand facilities and 20th in ac-cess. It also performed below-par in two other parameters —learning outcomes and quality, and access to education. The

National AchievementSurvey, conducted byNCERT, too, revealedthat the performance ofTelangana students fromclasses five to eight aspoorer in comparison tostudents from othersouthern states. Even theComptroller and AuditorGeneral in its audit re-

port in 2018 pulled up Telan-gana, where 14-26 per cent ofprimary school students and21-47 per cent of upper pri-mary school students scoredless than 40 per cent between2014 and 2017. It is thus im-perative for Telangana, whichis financially well endowed, toimplement the NEP. The NEP

understands the critical impor-tance of good quality child-hood care and education(ECCE).

Experts believe that over 85per cent of a child’s cumula-tive brain development occursprior to the age of six.Bybringing ECCE to the centreof education with an explicitfocus on building foundation-al and numerical skills forevery child by end of Grade 3,a quantum improvement inlearning outcomes is expected. The NEP also aims for a holis-tic, comprehensive educationas it breaks down prevailinginsularity by introducing the5+3+4+4 system of school ed-ucation.

Let Me Not ForgetIf it is not my portion to meetthee in this life

then let me ever feel that I havemissed thy sight

---let me not forget for amoment,

let me carry the pangs of thissorrow in my dreams

and in my wakeful hours.

As my days pass in the crowded market of this world

and my hands grow full with the daily profits,

let me ever feel that I have gained nothing

---let me not forget for a moment,

let me carry the pangs of this sorrow in my dreams

and in my wakeful hours.

When I sit by the roadside, tired and panting,

when I spread my bed low in the dust,

let me ever feel that the long journey is still beforeme

---let me not forget a moment,

let me carry the pangs of this sorrow in my dreams

and in my wakeful hours.

When my rooms have been decked out and theflutes sound

and the laughter there is loud,

let me ever feel that I have not invited thee to myhouse

---let me not forget for a moment,

let me carry the pangs of this sorrow in my dreams

Researchers have foundthat Keto diets and in-termittent fasting may

affect cardiovascular health. But Mediterranean diets andwhole food plant-based dietswhich include fruits, vegeta-bles, legumes, nuts, and wholegrains may prevent cardiovas-cular problems, they suggest-ed. "With diets like keto andintermittent fasting, social andpopular media has been flood-ed with claims, promises andwarnings that are at best un-verified and at worst harmfulto your health," said.

Andrew Freeman, MD,director of cardiovascular pre-vention and wellness at Na-tional Jewish Health and co-author of the study said, "Di-ets recommended by healthexperts, such as plant-basedand Mediterranean diets, havebeen extensively studied forsafety and efficacy, anddemonstrated conclusively toimprove cardiovascularhealth." Keto is a very lowcarbohydrate dietary approachthat sends the body into keto-sis, a metabolic state in whichit has reduced access to glu-cose and is instead mostly fu-elled by fat. While the limitedstudy of the keto diet showsthose who follow it initiallylose weight, it tends not to be

sustainable according to 12-month data. It is also unclearwhether the weight loss iscaused by ketosis or simply bycalorie restriction. Re-searchers also have concernsabout the type and amount offat consumed by those follow-ing a keto diet. While existingstudies strictly controlled thetype of fat and foods partici-pants consumed, many whotry keto consume highamounts of unhealthy saturat-ed fat, which is associatedwith an increased risk of heartdisease and high lipid levels inthe blood. There is also evi-dence that eating a keto dietfor an extended period of time

may lead to stiffening of thearteries, and several studiesfound that those who eat aketo diet have a greater risk ofdeath. Keto does, however, show promise as a potentialtreatment for diabetes, withstudies showing improvedglucose levels, as well as low-er fasting glucose and insulinlevels in mice fed a keto diet.Further research is needed toconfirm these benefits and as-sess risk before keto is clini-cally recommended. Re-searchers are also optimisticabout the potential health ben-efits of intermittent fasting butare concerned about possiblepitfalls.

CHANDIGARH : The Pesticides Management Bill 2020 (PMB) was introduced in the Rajya Sabha on 23 March 2020 to replace The Insecticides Act 1968, which currently governs the registration, manufacturing, export, sale and use of pesticides in India. While PMB is noble in intent, its many lacunae could end up hurting the interests of farmers and Indian agriculture. In 2019-20, for the first time, Punjab was no longer the largest wheat contributor to the central pool as MP delivered the highest quantity for a single season by any state. The production of food grains have also decreased in the last year from 31.69 Million Tons in 2017-18 to 31.52 Million Tons in 2018-19 (Source: Directorate of Economics & Statistics, DAC&FW). The incidence of indebtedness of marginal agricultural farmers in the state stands at 46.2% as per the Agricultural Statistics 2019 released by Ministry of Agriculture & Farmers Welfare. As it is, depleting groundwater levels is a major concern for the state, certain provisions of the The Pesticides Management Bill 2020 (PMB) shall exert a far-reaching impact not just on Indian agriculture but on farmers' livelihoods.

HANDIGARH: With an aim to speed up product innovation, the largest private sector lender HDFC BANK Chas started creating dedicated teams by taking executives

from various business functions to execute different projects. There are nearly a dozen such 'agile pod teams' having up to 40 members each drawn from across the hierarchy and various relevant business functions or departments to drive a project, sources said. This approach has so far delivered three projects - instant account opening, biometric know your customer (KYC) and fixed deposit renewals over a call, adding over 10 other projects are already in the works. Earlier, innovation proposals used to move from one department to another which was a time-consuming process, they said, adding under the new approach adopted as part of the 'One Bank' vision, the teams sit together. Strategic areas where innovative interventions need to be adopted are already outlined in broader areas like digital banking, customer relationship management, instant loans etc, and these teams sit together to conceptualise and execute the specifics. Decision making becomes quicker because the team sits together for a project and the ongoing pandemic has necessitated a need to look at more solutions. "The pandemic and the lockdown has hastened the need to evaluate and execute with agility," said one of the sources, citing the experience with the insta-account opening feature, where it was able to add 3 lakh customers despite the lockdowns. The pod team consists of executives from branch banking, information technology and digital banking functions, and the bank was able to launch the feature within 12 days from concept to execution.

States rose 20 per cent to USD44.8 billion despite tariff hikesimposed by the Trumpadministration in a fight withBeijing over its technologyambitions and trade surplus.Imports of American goodsgained 2 per cent to USD 10.5billion. The changes were duemostly to lower prices andcomparison with last August'srelative weak exports, according to Julian Evans-Pritchard of Capital Economics.Evans-Pritchard estimated thevolume of goods exported rose9.7 per cent from a year earlier, while import volumes rose 9.5per cent.

Other exporters havebenefited from increasedChinese demand for their goodsafter growth in the world'ssecond-largest economyrebounded to 3.2 per cent over a
