Marshall, Michigan - On Sunday, July 25,
2010, a segment of a 30-inch-diameter
pipeline ruptured in a wetland. The
rupture occurred during the last stages
of a planned shutdown and was not
discovered or addressed for 17 hours.

San Bruno, California - On September 9,
2010, a 30-inch-diameter segment of an
intrastate natural gas transmission
ruptured in a residential area. The rupture
released natural gas and ignited; resulting
in a fire that destroyed 38 homes and
damaged 70.
Eight people perished, many were injured,
and many more were evacuated from the
area.

2018 – Merrimack Valley, MA distribution pipeline
incident. Resulted in 1 death and several
hospitalized.
A series of explosions and fires occurred after HP
natural gas was released into a low-pressure gas
distribution system. The system overpressure
damaged 131 structures; most of the damage was a
result of structure fires ignited by gas-fueled

• These pipeline incidents and many more alike are contributed by, in part to disconnects
within an organization.
• Issues of Concern:
• Lack of awareness, training and accountability with key personnel
• Data management issues or disparate information systems makes it difficult to collect, use and
integrate data
• Lack of comprehensive procedures (WHO, WHAT, WHEN, WHERE, HOW)
• Documentation and record-keeping are critical parts of a pipeline safety management
system (PSMS) program.

• Safe and effective pipeline operation requires awareness and management of many linked
activities which can become complex processes that must be able to be performed
consistently and effectively
• Examples of such activities include design, construction, operations, maintenance and integrity
management
• Managing the safety of a process, no matter how complex, requires coordinated actions in
order to address multiple, dynamic activities and circumstances

Addressing Safety Related Concerns
• The NTSB accident investigation for Merrimack Valley focused on two key principles to
address safety for pipeline operation: Safety Management Systems and a Management of
Change (MOC) Process.
• Proposed legislation was introduced in the wake of Merrimack Valley directing PHMSA to
mandate all pipeline operators to implement a pipeline safety system management system
in accordance with API RP 1173 requirements.
• MOC procedures must identify changes to the pipeline system (before they occur) as well
as identify the potential risks and implications associated with the change.
