OFFICE MEMORANDUM Subject: Revision of Ceiling Rates for Coronary AngiopJasty and Coronary AngiopJasty with Balloon for CGHS beneficiaries. *"** With reference to the above mentioned subject the undersigned is directed to draw attention to the Office Memorandum No. 8.11011123/2009 IHospital Cell dated 17.08.2010 and other Office Memoranda issued subsequently whereby the CGHS package rates for Coronary Angioplasty were fixed by the Government for empanelled hospitals under CGHS in' Delhi and NCR and other CGHS cities and to state that in supersession of the earlier CGHS rates ceiling rates the following ceiling rates are approved for reimbursement to CGHS beneficiaries/CGH8 empanelied hospitals as per the details given below: Coronary Angioplasty Coronary Anglop\asty with Balloon : Rs.50, 000/- (for semi-private ward) : Rs.55, 000/- (for semi-private ward) Reimbursement to beneficiaries /ernpanelled hospitals shall be limited to ceiling rate or as per actuals, whichever is lower. The other terms and conditions as regards to CGHS package rates remain unchanged. 2. The revised rates shall come into force from the date of issue and shall be in force till further orders. 3. With reference to the above mentioned subject the undersigned is directed to drawattention to the Office Memorandum No. 8.11011123/2009 IHospital Cell dated 17.08.2010 andother Office Memoranda issued subsequently whereby the CGHS package rates for CoronaryAngioplasty were fixed by the Government for empanelled hospitals under CGHS in' Delhi andNCR and other CGHS cities and to state that in supersession of the earlier CGHS rates ceilingrates the following ceiling rates are approved for reimbursement to CGHS beneficiaries/CGH8empanelied hospitals as per the details given below:

With reference to the above mentioned subject the undersigned is directed to drawattention to the Office Memorandum of even No. dated 31.10.2011 and to state that the ceilingrates for reimbursement of Stainless steel bare metal coronary stents for CGHSI CS(MJI.)beneficiaries as mentioned at para (1) (2) (i) of the said order are hereby revised and the newceiling rates for reimbursement to CGHS/CS(MA) beneficiaries are fixed as under:

Reimbursement to beneficiaries /empanelled hospitals shall be allowed subject to theceiling rates or actuals, whichever are lower.

2. Coronary Stents shall be permitted on the advice of Govt. Specialist, of which not morethan two shall be of Drug Eluting Stents( in any o~ the coronary stents as per the decision oftreating specialist). Permission shall be granted as per the laid down procedure. If more thantwo drug eluting stents are implanted in an empanelled hospital and no written informedconsent was obtained from the beneficiary that he/she would bear the difference in costbetween the DES and Bare Metal Stent, and the hospital has charged this amount from thebeneficiary, the additional amount shall be paid to the beneficiary and shall be deducted fromthe pending bills of hospitals.

3. It is essential for the empanelled hospitals to quote the Batch number when a coronary/vascular stent of any type is implanted in the case of a CGHS/CS (MA) beneficiary and alsoenclose a copy of the relevant invoices pertaining to the procurement of the stents by thehospitals. In addition to this, the outer pouch of the Stent packet along with the sticker on it onwhich details of the stent are printed shall also be enclosed with the medical bill for claimingreimbursement from the Govt. In case of treatment from a private non-empanelled hospital,where the treatment was taken in an emergency, it is the responsibility of the beneficiary toobtain the batch number, invoice and outer pouches of the stent(s) before the submission ofthe medical claim to CGHS/ concerned department, as the case may be.

4. The empanelled hospital shall submit a self certified undertaking that the hospital hasnot charged the CGHS / CS (MA) beneficiary more than the rate at which the stent has beenprocured by the hospital and in case of any detection and establishment that the hospital hasovercharged, the hospital shall be removed from the list of hospitals empanelled under CGHSwithout any further notice. .

5. UTI-ITSL, while processing the hospital bills of coronary / vascular stents shall ensurethat the hospitals have enclosed copies of the relevant invoices pertaining to the procurementof the stents by the hospitals and the outer pouch of-the srent packet along with the sticker onit on which details of the stent are printed and that the 'prescribed rates and the guidelineshave. been followed, before making 'provisional payments to the hospitals.

6. The revised rates and guidelines shall come intoforce from the' date of issue and shallbe in force till they are revised. .

7. This issues with the concurrence of Integrated Finance Division vide Note dated20.12.2012 of AS&FA, Min. of H&FW.

OFFICE MEMORANDUM

Subject: Fresh empanelment of private hospitals and revision of packagerates applicable under CGHS, Delhi.

The undersigned is directed to state that CGHS had initiated action for freshempanelment of private hospitals under CGHS, Delhi (which covers areas in Delhi,Faridabad, Gurgaon, Ghaziabad and NaiDA) and also for the revision of packagerates (which were fixed in 2006), to be paid to hospitals, by floating tender for thesame. On the basis of the responses received rates for various procedures Jtreatments have been arrived at and have been uploaded in the website of CGHS:www.mohfw.nic.in\cghsnew\index.asp and can be downloaded.

2. In order that CGHS beneficiaries get treatment from well maintained and runhospitals, it has been decided to have differential rates of reimbursements, as perdetails given in the enclosed rates list. The principle followed for the differentialpackage rates being:

(i) where L-1 rates were arrived at on the basis of rates quoted by non-NASH accredited hospitals / super-speciality hospitals, NASH accreditedhospitals will be entitled to reimbursement of certain percentage ofadditional amount over and above the L-1 rates;

(ii) where L- 1 rates were arrived at on the basis of rates quoted by NASHaccredited hospitals, then non-NASH accredited hospitals would beentitled to an amount lower by certain percentage than thereimbursement made to NASH accredited hospitals at L-1 rates; and

(ili) Rates for super-speciality hospitals have been identified separately andnotified in the rate list put on the website of the CGHS. Super-specialityhospitals covered under this category are:

(1) Cardiology & Cardio-thoracic surgery

(2) Joint replacement surgery made under orthopaedics

(3) Nephrology & Urology including renal transplantation;

(4) Endocrinology;

(5) Neurosurgery;

(6) Gastro-enterology & GI surgery; and

(7) Oncology

3.1 "Package Rate" shall mean and include lump sum cost of inpatient treatment Jday care J diagnostic procedure for which a CGHS beneficiary has been permitted bythe competent authority or for treatment under emergency from the time of admissionto the time of discharge including (but not limited to) - (i) Registration charges, (ii)Admission charges, (iii) Accommodation charges including patients diet, (IV)Operation charges, (v) Injection charges, (vi) Dressing charges, (vii) Doctor Iconsultant visit charges, (viii) ICU I ICCU charges, (ix) Monitoring charges, (x)Transfusion charges, (xi) Anesthesia charges, (xii) Operation theatre charges, (xiii)Procedural charges I surgeon's fee, (xiv) Cost of surgical disposables and all sundriesused during hospitalization, (xv) Cost of medicines, (xvi) Related routine and essentialinvestigations, (xvii) Physiotherapy charges etc. (xviii) Nursing care and charges for itsservices.

(b) Cost of Implants I stents I grafts is reimbursable in addition to package ratesas per CGHS ceiling rates for Implants / stents / grafts or as per actual, in case thereis no CGHS prescribed ceiling rates.

(c) Treatment charges for new born baby are separately reimbursable in additionto delivery chares for mother.

d) The hospitals empanelled under CGHS shall not charge more than thepackage rates I rates.

3.2 Package rates envisage upto a maximum duration of indoor treatment asfollows:

12 days for Specialised (Super Specialties) treatment;

7 days for other Major Surgeries;

3 days for Laparoscopic surgeries / normal deliveries; and

1 day for day care I Minor (OPO) surgeries.

3.3 However, if the beneficiary has to stay in the hospital for his / her recovery fora period more than the period covered in package rate, in exceptional cases,supported by relevant medical records and certified as such by hospital, theadditional reimbursement shall be limited to accommodation charges as perentitlement. investigations charges at approved rates, and doctors visit charges (notmore than 2 visits per day per visit by specialists I consultants) and cost of medicinesfor additional stay).

3.4 No additional charge on account of extended period of stay shall be allowed ifthat extension is due to infection on the consequences of surgical procedure or due toany improper procedure and is not justified.

4.1 CGHS beneficiaries are entitled to facilities of private, semi-private or generalward depending on their basic pay / pension. The entitlement is as follows:-

4.2 The package rates for semi-private ward.

4.3 It has now been decided that the CGHS beneficiaries taking treatment in theempanelled hospitals will be entitled for reimbursement /treatment on credit as per thepackage rates / rates. The package rates are for semi-private ward. If the beneficiaryis entitled for general ward there will be a decrease of 10% in the rates; for privateward entitlement there will be an increase of 15%. However, the rates shall be samefor investigation irrespective of entitlement, whether the patient is admitted or not andthe test per se does not require admission to hospital.

4,4 A hospital empanelled under CGHS, whose normal rates for treatmentprocedure I test are lower than the CGHS prescribed rates shall charge as per therates charged by them for that procedure / treatment from a non-CGHS beneficiaryand will furnish a certificate to the effect that the rates charged from CGHSbeneficiaries are not more than the rates charged by them from non-CG HSbeneficiaries.

5.1 Private ward is defined as a hospital room where single patient isaccommodated and which has an attached toilet (lavatory and bath). The room shouldhave furnishings like wardrobe, dressing table, bed-side table, sofa set, carpet, etc. aswell as a bed for attendant. The room has to be air-conditioned.

5.2 Semi Private ward is defined as a hospital room where two to three patients areaccommodated and which has attached toilet facilities and necessary furnishings.

5.3 General ward is defined as halls that accommodate four to ten patients.

5,4 Normally treatment in higher category of accommodation than the entitledcategory is not permissible. However, in case of an emergency when the entitledcategory accommodation is not available, admission in the immediate higher categorymay be allowed till the entitled category accommodation becomes available.However, if a particular hospital does not have the ward as per entitlement ofbeneficiary, then the hospital can only bill as per entitlement of the beneficiary eventhough the treatment was given in higher type of ward.

6.1 In case of non-emergencies, the beneficiary shall have the option ofavailing specific treatment I investigation from any of the empanelled hospitals of his /her choice (provided the hospital is empanelled for that treatment procedure I test),

after the same has been advised by CGHS lather Government Specialist I CMOin-charge and permission is obtained from the competent authority.

6.2 CGHS beneficiaries have, so far, been given the option to get themselvestreated in any hospital of their choice. However, in view of the increased outgo ongetting treatment in super-speciality hospitals, it has now been decided that CGHSbeneficiaries desirous of getting treated in super-speciality hospitals, in non-emergency conditions, prior approval of the concerned Additional Director, CGHSwould have to be obtained.

6.3 Permission for treatment, other than those covered by para 6.2 above, isgranted by CMO in-charge / Additional Director / Joint Director, CGHS in case ofpensioners, former Governors, former Vice-Presidents, ex-MPs, Freedom Fighters,etc., and by Rajya Sabha / Lok Sabha Secretariat as the case may be in case ofsitting Members of Parliament, concerned Ministry / Department / Organisation in caseof serving Government employees, serving employees and pensioners of autonomousbodies covered under CGHS.

7. The empanelled hospitals shall honour permission letter issued by competentauthority and provide treatment / investigation facilities as specified in the permissionletter.

8.1 Hospitals shall provide credit facility to the following categories of CGHSbeneficiaries (including dependant family members, whose names are entered onCGHS Card) on production of valid permission letter:• Members of Parliament;• Pensioners of Central Government drawing pension from central estimates;• former Vice-presidents, Former Governors and former Prime Ministers;• Ex-Members of Parliament;• Freedom Fighters;• serving CGHS employees;• serving employees of Ministry of Health & Family Welfare (including attached /

subordinate offices under the Ministry of Health & Family Welfare); and• such other categories of CGHS cardholders as notified by the Government.

8.2 Bills should be submitted to the Office of the Rajya Sabha Secretariat / LokSabha Secretariat in case of sitting Members of Parliament and to Additional Director,CGHS (Hqrs), Delhi, in case of other beneficiaries enlisted above, once in a month.

8.3 The hospitals shall extend credit facility to the above categories of CGHSbeneficiaries under emergency / with prior permission irrespective of the CGHS citywhere they are registered and send the bill to Additional Director, CGHS (Hqrs), Delhi.

8.4 Bills of serving employees of the Ministry of Health & Family Welfare and theemployees if attached / subordinate offices under the Ministry of Health & FamilyWelfare will be sent directly to the office / officer which / who referred the patients tothe hospitals.

8.5 In case of emergencies empanelled hospitals shall not refuse admission nordemand payment from CGHS beneficiaries on production of valid CGHS card, issuedby competent authority of any CGHS City.

8.6 Reimbursement in case of pensioners, former Governors, former Vice-Presidents, ex-MPs , Freedom Fighters, etc., is made by CGHS and by Rajya SabhaSecretariat I Lok Sabha Secretariat in case of sitting Members of Parliament and byconcerned Ministry I Departmentl Organisation in case of serving Govt. employees,serving employees and pensioners of autonomous bodies covered under CGHS.

8.7 Serving Central Government employees and their dependent family members notcovered by the CGHS will not be denied treatment at package rates if they approachthe hospitals with a proper referral letter from the Ministry / Department in which theyare working, after verifying the genuineness of the employee by production of his / hervalid identify card issued by the appropriate authority.

8.8 Serving Central Government employees and their dependent family members notcovered by the CGHS will not be denied treatment at package rates if they approachthe hospitals in emergency condition to the patient. The treatment will given afterverifying the genuineness of the employee by production of his / her valid identify cardissued by the appropriate authority.

9 During In-patient treatment of the CGHS beneficiary, the hospital will not ask thebeneficiary or his f her attendant to purchase separately the medicines / sundries Jequipment or accessories from outside and will provide the treatment within thepackage rate, fixed by the CGHS which includes the cost of all the items.

10 In case of treatment taken in emergency in any non-em panelled privatehospitals, reimbursement shall be considered by competent authority at CGHSprescribed packages I rates only.

11 If one or more minor procedures form part of a major treatment procedure, thenpackage charges would be permissible for major procedure and only at 50% ofcharges for minor procedure.

12 Any legal liability arisinq out of such services, responsibility solely rests on thehospital and shall be dealt with by the concerned empanelled hospital themselves.

13 This Office Memorandum supercedes all earlier instructions relating toempanelment of hospitals for specialised and general purpose treatment andinvestigations for Delhi, Faridabad, Ghaziabad, Gurgaon and NOIDA areas.

14 This issues with the concurrence of Internal Finance Division in the Ministry ofHealth & Family Welfare, vide Dy. No: AS & FA 12457/2010 dated the 16th August,2010.
