We are a community service organization incorporated in 2012; we received our 501(c) (3) designation in 2014. Our mission is to empower individuals and families in SW Utah and SE Nevada with resource connections, educational outreach and support to help and strengthen all affected by dementia. More than 1,200 individuals have participated in our classes, seminars and personal consultations. We have a contact list of 750 people who have expressed support and interest in our services and a Facebook following of over 600. We request your consideration of a grant of $6,000.00 to help us supply caregiver education and cognitive training for women with memory loss the women who account for 2/3 of all dementia caregivers. This funding would also provide wellness calls to check on women who live alone with dementia and women who give care and are isolated. We are requesting funding for three projects that heavily impact women in our community of 19% seniors: 1. $2600.00 for 26 Financial Need Scholarships to the Early Stage Memory Loss classes, a 10-week series held twice annually which serves on average 110 women who give care, 70 women who are victims to Alzheimer’s and related dementia and 15 women who live alone with dementia. 2. $2400.00 to fund a Wellness Calling System for one year to make daily calls to approximately 30 women who live alone with dementia or who care for a demented partner with no family locally to check on them. 3. $1000.00 to fund 10 rural outreach trips providing education and support group consultations to underserved areas in Southern Utah and SE Nevada helping female caregivers and women with dementia. The outcomes we hope to see from instituting these programs are:

The outcomes we hope to see from instituting these programs are: 1. An increased number of women made vulnerable because of dementia or caregiving, would participate

in education and support classes and prepare for the future with legal, financial and community resources.

2. Women with dementia living alone would feel protected and safer knowing they were being contacted daily by phone and helped if they are not able to answer.

3. Isolated female caregivers would feel protected and safer knowing that someone would contact them daily to check on their wellbeing and offer consultation, socialization and help reducing their vulnerability and isolation.

4. Women in rural areas facing a dementia caregiving journey or the future of an illness of dementia will have access to support, education, consultation and referrals.
