SEPARATED PARTIES: We do not allow fees to be paid by two different parties when parents are divorced, or separated. The parent responsible for fees is the person who has legal custody, and brings the child to day care and pays the fees. Married Couples residing together are both responsible for payment. ITEM ONE: ENROLLMENT PROCEDURES, START OF CARE, & TERMINATION POLICIES

ENROLLMENT PROCEDURES: Prior to enrollment, Parents must read, agree to, and sign this contract. Please feel free to ask for clarification if you have any questions or concerns. There are also several licensing documents in your enrollment packet which are required by the state that must be completed prior to your child’s first day of attendance. You will receive a signed copy of this contract for your records, and the original will be kept in your child’s file. Please communicate with me any changes of address, phone numbers, place of employment, etc. as they are made.

There is a two week trial period for each new family in attendance. During this time care may be terminated immediately without notice by either party at anytime. Should the provider terminate care during this time, a refund for days not used, less a $50 application fee, will be issued immediately. Should the parent terminate care during this time, no refund will be issued. After the two week trial period expires, the contract payment obligation will resume as outlined in this contract.

Upon enrollment, please expect to pay your first two weeks tuition and an additional two weeks tuition to be applied as a deposit towards your last two weeks of care, plus any additional days you wish to add to your contract. Tina's House Family Daycare accepts Cash, Check, or Money Order.

START OF CARE: Please make sure that you have all the items necessary for your child’s enrollment PRIOR to your child’s first day. These items include, but are not limited to, immunization records, incidental medical services plan of operation, licensing documents, subsidy care paperwork, enrollment forms, payment, medications, change of clothing, diapers, wipes, rash cream, sunscreen, etc.

PARENT COMMUNICATION: Parent communication is important for the development of your child. Please keep me informed of any problems you may be having at home that could affect how you child acts or behaves at school. I will keep you informed by providing daily updates. I will send information through email, text, or Facebook posts periodically. Also, there will be notices posted to the bulletin board so please check it regularly. The line of communication is always open so please feel free to talk to me at any time.

BACK-UP PROVIDER: Parents are required to have a back-up provider in case of program closures for vacations, holidays, emergencies, child illness, or other circumstances in which back-up care may be necessary.

RESPONSIBLE ADULTS: It is your responsibility to have a list of responsible adults that are capable and ready to pick your child up in an emergency when you are not able to. You must call the school first to let us know that someone other than yourself is picking the child up even if that person is listed on your registration forms and a written permission slip must be provided. Parents are responsible to be sure the child is picked up on time. If you should be late then call one of your listed responsible adults to pick your child up. The school will call the names on your list should you be more than 30 minutes late It is imperative that someone be able to come get the child. NOTE: It is not acceptable at any time for any adult picking up your child to arrive at school under the influence of drugs or alcohol. They will not be allowed to take your child, and you and the authorities will be called immediately. Should this happen this person will no longer

be acceptable on your list of responsible adults and you will have to find someone else to list. Anyone picking up your child are required to have a car seat to use for your child.

DRESS CODE/CLOTHING: Children will arrive dressed, clean, and in a clean dry diaper or pull-up if potty training, and ready for the day, including wearing shoes and socks. If child arrives soiled or wet, parents will take time to change them before leaving.

Children should be dressed accordingly to the weather and should be sent in clothes that you do not care about and that the child is allowed to get dirty, stained, or otherwise ruined. Children need to keep at least one spare change of clothing in their cubby at all times. Children will need a warm jacket for winter time, and light weight jacket or sweater for Spring and Fall.

During the winter months please bring a pair of High top durable rain boots to leave at school and a pair of slippers for indoor play. The children do take walks in the rain and raincoats are highly recommended on rainy days. The slippers are nice for winter months to keep feet warm when children decide to remove their shoes.

Shoes with laces are not allowed. I try to foster independence and unless a child can tie their own shoes, it saves me a lot of struggle when we have a full house. All shoes must be safe and secure on the child’s foot. Slip-on Tennis shoes are preferred. Please try to avoid open toed sandals. Loose fitting shoes, or shoes that do not fit properly are not acceptable.

DIAPERS/TOILETING/POTTY TRAINING: Parents are required to provide and keep a supply of wipes, rash cream or ointment, and Diapers or pull ups at day care at all times, and replenish such supplies as necessary.

The provider will help assist you in potty training when the child is ready to be trained. During the training process the child will wear pull ups while at day care. NOT CLOTH panties. When the child stops having accidents at day care, and the provider feels the child is ready then we will go to the next step and use cloth (thick) training pants with a Plastic pair of pants to wear over them to save furniture and carpeting from wetness. We will not start potty training any child that is under age 2 years. We will only assist in this process on any child that is able to meet the provider’s requirements. We also reserve the right to stop the potty training process if it traumatizes the child or the parents fail to follow through at home.

Potty-training is considered complete when the child is self-sufficient in using the restroom facilities without the aid of an adult.

PERSONAL ITEMS FROM HOME: We allow your child to bring items from home. Your child is not required to share their personal items. Our staff is not responsible for lost or broken items. Please do not bring personal items from home and take them from your child when they arrive at school.

BEHAVIOR PROBLEMS: Parents will be told of any behavior problems regarding their child. Extreme behavior problems can result in a conference with parents, or a call to the parents to come and pick the child up early. If extreme behavior problems continue and the behavior is a threat to the welfare of the other children the child may be terminated from care.

It is unacceptable for any child to hit, kick, bite, cuss, spit, pull hair, wrestle, shove, throw toys or objects, deliberately tear up or break toys or items, or intentionally hurt another person. If these

actions are violent or for the intent to hurt anyone or oneself, or if the child is uncontrollable in any action including tantrums, the child will be sent home without notice.

TERMINATION NOTICE: It is required that you give a TWO FULL WEEK PAID written/dated notice to the Provider before terminating child care services. Termination notices will not be accepted any time during which the facility is closed including weekends, holidays, vacation, or sick days. Any unused days or credits on your account will be forfeit upon notice. Any final tuition amount not covered by upon tendering your written termination notice and any notice given or withdrawal of care without payment will be sent to collections for any amount owing plus any and all applicable late fees. See late fees.

PROVIDER TERMINATING SERVICES: Provider has the right to terminate child care services without notice for any reason; however any of the following reasons will be cause for immediate termination and will be effective immediately upon receiving a written notice. Parent is responsible for any unpaid fees that have occurred up-to the termination date including late payment and bounced check fees.

● Child's behavior is uncontrollable and is a risk to the other children.● Child is not adjusting to the environment and it is to traumatizing to attend.● Fees were not paid and are overdue or there is a breach of contract.● Parent is a risk to the provider or other children in care● Parent arrives at daycare drunk, or under the influence of drugs.● Provider makes a judgment call and feels immediate termination is needed.● Parents breach any parts of the Contract agreement. ● Failure of parent to drop-off and pick-up child at scheduled times despite warning● Failure to replace/repair damaged properties per this agreement● The provider is incapable of providing services for a child with special needs identified

after the start of care.

Provider is within all legal rights to terminate services at any time for any reason. Whenever possible under other circumstances provider will give a one week notice should child care services need to be terminated.

ITEM TWO: HOURS OF OPERATION, RATES/PAYMENTS/DISCOUNTS, & PROGRAM CLOSURES

HOURS OF OPERATION: Normal contracted hours are for Monday through Friday. Any variation to contracted hours are at the discretion of the Director and may result in an increase in tuition. Any extended hours will be described on an individual written agreement apart from this contract.

FEES: A non-refundable placeholder fee will be charged for parents who wish to hold their child’s place for an extended period of time. The rate charged is ½ of the projected weekly tuition paid every week until the child starts care.

A Supplies Fee will be charged annually to help cover the cost of art and printing supplies and will be due and payable upon the first invoice in August.

TUITION FEES: Tuition Fees are based on enrollment, not attendance. There are no deductions in Tuition Fees, regardless if child is absent for any reason. Tuition Fees are paid 52 weeks per year. Your first two weeks tuition are due upon start of care. Monthly Tuition Fees are paid based on the formula (Fee x 52 weeks / 12 months). Bi-weekly Fees are paid based on the formula (Tuition Fee x 2 weeks). Additional payment schedules are at the discretion of the provider.

SUBSIDIZED PAYMENTS: Families who are funded for childcare services through a subsidized payment program such as Beanstalk, Child Action, or the like, are required to pay any tuition amounts not covered by the subsidy for time that their child is in care. This includes unauthorized overtime, days not covered by the subsidy, holidays not covered by the subsidy in which the provider is open and provides services, or any other services provided to the family that are not covered by the subsidy.

Any child who is funded through a subsidized childcare program is not permitted to attend if the subsidy does not cover their attendance unless the parent agrees to pay the full daily rates for their child out of pocket, in advance, at the time of service. Childcare will not be provided without pre-payment of services for such days.

In some cases, where applicable, parents may be required to pay regular tuition rates out of pocket until which time the first payment is received from said Subsidy. If at anytime, your enrollment on such program becomes inactive, you are required to pay any fees or tuition amounts due for care provided and care may be suspended or terminated if payment is not made.

Any fees that are not permitted to be charged as a part of the provider/subsidy agreement will be waived, however this does not relieve the parent of any liability of other charges or fees due as a part of this agreement.

Families who are funded through a subsidized childcare program agree to bring their child, at a minimum, the maximum hours allocated per their subsidy contract. For example, if you are approved for 25 hours per week of subsidized childcare you are required to bring your child for all of those approved hours. If your child is sick or you chose to keep your child home as is in the best interest of the child, you are required sign your subsidy paperwork so the provider can get paid for your child’s absence. If at anytime your subsidy does not pay for time in which your child was in care, the provider retains the right to bill the family directly for services rendered and is entitled to any and all late fees as a result of the non-payment.

PAYMENT SCHEDULE: Payments are due at Drop-off. Payment schedules can be arranged for Monthly, or Bi-Weekly payments. Monthly payments are due in advance on the FIRST day of the month. Bi-weekly payments are due in advance every other FRIDAY. NO EXCEPTIONS.

RATE INCREASES: All parents are required to renew their Childcare Services Agreement annually. The provider retains the right to increase rates annually and parents will be notified no less than 30 DAYS in advance for which the rate will become effective. Any and all discounts awarded at the time of signing of this contract become null and void as of 12:59pm on Dec. 31st the year in which this contract is signed.

LATE FEE / BOUNCED CHECK CHARGES: There is a bounced check charge of $25 for bounced or returned check payments. If your payment is late for any reason, a late payment fee of $15/day for every day the payment is not made will be applied to your account. Late charges will be imposed retroactively for returned checks that are not paid in full within 24 hours from the notification of late payment. Your account must be brought current, meaning no fees owed, before your child can resume care. If your account remains outstanding for more than 30 days, your account will be assigned to collections.

OVERTIME FEE: Please pick up your children on-time as I do not offer short-notice overtime. A fee of $5 for every additional 15 minutes or portion thereof will be charged for unarranged late pick-ups. Payment will be due when the child is picked up that day or when the child is next dropped off for care, at the latest. Repeated failure to comply with the hours of operation or contracted hour schedules without prior arrangement, is cause for termination.

HOLIDAYS: Holidays and scheduled program closures are included in your weekly fee. There are no deductions in fees for Holiday closings, EXCEPT for Winter Break. A 10% discount on tuition will be given for the Winter Break closure ONLY.

As you can imagine, having a business in your home takes away from one’s ability to spend one-on-one time with your own family. I schedule program closures in conjunction with the days in which my children’s school is out so that we can have time as a family without children in care, aside from busy weekends full of teenage extracurricular activities.

As a result, Tina’s House Family Day Care will be closed for the following Holidays and substitute care will not be provided. In the event that the holiday falls on a weekend, the holiday will be observed on the day in which it is observed by the School District. Tina’s House Family Daycare will be CLOSED:

● President’s Week as observed by the Roseville Joint School District (February)● Spring Break as observed by the Roseville Joint Union School District (April)● Memorial Day (May)● Labor Day (September)● Thanksgiving Break (Wednesday – Friday: November)

● Winter Break as observed by the Roseville Joint Union School District* (December- January) including, but not limited to, Christmas Eve, Christmas Day, New Year’s Eve and New Year’s Day. *This is a FULL TWO WEEK closure. The provider may chose to close the week prior to Christmas in lieu of the week after the New Year and may choose to extend or modify this closure at her discretion.

Children enrolled through the Beanstalk Subsidized Child Care Program will not be permitted to attend on days that the Beanstalk Program is CLOSED and fees are not paid through their subsidized payment schedule UNLESS a parent agrees to pay any tuition or fees associated with the day of care out of their own pocket, in advance, at the regular drop-in rate, at the time of service.

Beanstalk has identified the following days as CLOSED: ● Independence Day● Labor Day● Thanksgiving Day● Christmas Day● New Year’s Day● MLK Jr. Day● President’s Day● Memorial Day

VACATIONS & BEREAVEMENT: The provider is entitled to a ONE WEEK (FIVE DAYS) vacation per year and may be taken in addition to closures for Holidays. Parents will be notified no less than 30 DAYS in advance of a provider vacation. Tuition fees, for the days the program is closed due to vacation and alternate care is not provided, will be waived for the duration of the vacation and reflected on the invoice for which the vacation is taken.

Parents are entitled to ONE WEEK (FIVE DAYS) of vacation, tuition free, per year, providing at least 30 DAYS notice is given. Your tuition free vacation week must be approved by the provider, in advance. Vacations cannot coincide with program closures, provider vacation, or another families already scheduled vacation. A discount for tuition fees will be reflected on the month’s invoice for which the vacation is taken. Parents MUST notify Provider 30 DAYS in advance if planning a vacation for which the child will be absent from care if the parent wishes to receive the discount for the vacation. All fees for regular tuition, outside of the vacation, are due on time, regardless of the duration for which child will be absent before you leave on vacation. Failure to pay tuition fees by the time they are due and payable could result in services being suspended and/or forfeited.

Both Provider and Parent are entitled up to THREE days of paid bereavement leave. Days for which your child is not in care under the Parental Bereavement Leave will be applied as full day’s credit toward the next invoice. There is no change in tuition dues for Provider Bereavement Leave and credits will not be applied for program closure due to Provider Bereavement Leave.

EARLY CLOSURES: Provider retains the right to close early on days necessary to provide transportation or care to provider’s own children. This includes illness, extracurricular or school activities or commitments, medical appointments, or other imperative commitments. Provider

will give at least two weeks’ notice (or as much time as possible) for days in which the program will be closed early. However, should the provider’s own children be sent home from school with a communicable or highly contagious illness and alternate care cannot be found for the provider’s own children, parents will be required to pick up their children immediately. No credit or refund will be given for such instances.

INSERVICE DAYS: Occasionally, the provider may be absent to attend professional development activities. These are typically days outside of regular program closures so that the provider may attend workshops, conferences, or other trainings for professional growth. Substitute care is generally provided during these times, however if substitute care is unavailable, the provider retains the right to close and the parent will be notified with no less than two weeks notice. There will be no change in dues for such closures.

PROVIDER ILLNESS/EMERGENCY CLOSURES: In the event that I am too ill to care for children, my children’s illness is one in which they cannot simply be separated from the other children in care and/or requires medical attention or a doctor’s visit, or in the case of an emergency, I will notify parents as soon as possible via group text. Should I become unable to provide service for more than 2 consecutive days and substitute care is not provided, meaning that the facility is closed, a daily pro-rated amount will be credited towards the parents next month’s tuition for each day after the second consecutive day of closure, not exceed 5 days worth of credit.

In the event that there is a major illness or emergency causing a closure for more than two consecutive weeks, parents will be notified as soon as possible via email and given the option to receive a discount on their future tuition, not to exceed one full week’s worth of tuition, or terminate services without penalty. No refunds will be given for tuition amounts already paid, including any deposit paid towards your child’s final two weeks of care.

DROP OFF/ARRIVAL: Drop off times are established upon enrollment. If you have not arrived or made contact within one hour of your scheduled drop off time, I will assume your child will be absent and continue on with daily activities as planned. Please contact me if you are running behind schedule so that I can make accommodations (saving a meal, postponing walks to the park, etc.). If you do not make contact, you may not drop off your child after an hour past your scheduled drop-off time.

Upon arrival, please discuss any circumstances that may be pertinent such as a lousy night’s sleep, fussiness during morning routine, or anything that is different from your child’s normal daily routine. Place your child’s personal items in their assigned cubby and say goodbye before departing. Sneaking out on your child is NEVER allowed. Relationships are built on trust and this simple task is very important for your child.

PICK-UP/DEPARTURE: Parents are required to pick up their child on-time, as outlined in this agreement. Any variance to the pick-up time contracted and outlined above is subject to a late pick-up fee. I ask that parents please be respectful of my hours of operation as they are essential to both the operation of this childcare facility and my family.

● If you are going to run late and are unable to pick up your child at your contracted time, you are required make contact as soon as you become aware of the possibility of a delay.

● If you are behind schedule, but able to pick-up within the hours of operation, you are still required to notify the provider.

● If, at closing time (5pm), your child has not been picked up and the provider has not been notified, Provider or a representative will contact each emergency contact on file until they have reached someone who is available to pick up. In the event that the provider has arranged for someone else to pick up, and you arrive, it is your responsibility to notify that person that retrieval of your child is no longer necessary.

● If a child is still here after closing, and contact has not been made within a reasonable amount of time, a call will be made to the authorities as required by law. Late pickup fees apply and are described within the rates section of this agreement. As a result, care will be terminated immediately and any and applicable fees will be due at pick-up or sent to collections within 24 hours.

If someone I have not yet met is scheduled to pick up your child, they must be on the emergency contact list, or you must notify me in advance. Additionally, they must be prepared with a current and valid ID.

Please check your child’s cubby for art, dirty clothing, etc. and make note of what needs to be brought for their next day in attendance.

NAP TIME: It is requested that parents try to avoid dropping-in during nap time hours if possible. You always have the right to come during open hours, but we ask that you take into consideration these times. Any parent who drops in during nap time and their child awakens, the parent will be required to take the child with them upon departure.

EARLY PICK-UP: If you plan to pick up your child early, please let the provider know. We will have your child cleaned up and ready to go.

APPOINTMENTS: If at all possible, please avoid appointments that would require you to drop your child off at day care, come back and pick up from day care for the appointment, and then

bring the child back to day care. This confuses the child and can cause a lot of stress and anxiety. Children are use to one routine: drop off and pick up.

PARKING: Please be courteous when parking in front of the house and leave room enough for other parents to park. Please park on the street next to the curb. DO NOT BLOCK DRIVEWAYS AT ANYTIME. Do not leave your car motor running during pick-up or drop off. This is a safety and liability issue. Do not leave any child unattended in your vehicle at any time. These things are violation of city codes, state laws, and ordinances and provider is not liable for any damages, citations, or other violations received.

CAR SEATS: Children under age 8 and less than 80 Ibs must still use a booster or car seat. Adults picking up child must have a car seat when they are picked up. This includes either parent picking up the child or responsible adults picking up the child. The car seats must be in good working order and secured properly in the rear seat of the vehicle. NOT THE FRONT SEAT. The school does not have car seats to loan out. If the child does not have a car seat someone else will have to come and get the child that does have a car seat. Parents will be responsible for late fee payments should this happen. This is a state and national law for the safety of your child.

VISITORS: Only Parents are cleared to be here because of Licensing regulations therefore only parents are allowed to visit during the day. If you have a family member that wants to visit your child then please bring them with you at pick up or drop off. Otherwise include the family member on your release forms for alternate pick up and have the person pick up the child for the day so they can visit elsewhere outside of the facility.

PARENTS: We have an “Open Door” policy and that means you can come any time during the hours which your child is in care, for whatever reason. In fact, this is you're right. Please be advised that the front doors are locked during the day after the last drop off. If you happen to drop in during the day, we promise to answer the door as quickly as possible.

The provider reserves the right to dismiss any child for the rest of the day if a visit and eventual departure causes the child undue stress. If the child will not settle back down because you are leaving you may be required to take your child with you and will be discouraged to drop in again until your child can acclimate to your absence.

MEALS: Meals are provided for all children in care. Breastmilk and/or preferred formula other than Parent’s Choice (walmart brand) for infants must be provided by the parent.

I provide Breakfast, Morning Snack, Lunch, Afternoon Snack, and Evening Snack/ Dinner – depending on the time in which your child is in care. Meals are only served while child is in attendance. I will not provide meals to a child arriving later than the scheduled meal time or departing earlier than a scheduled meal time.

All meals are healthy and prepared using whole foods wherever possible (meats, fruit, veggies, whole grains) and minimally processed foods such as cheese, noodles, crackers, muffins, breads and yogurts. If your child has a specific dietary need, please disclose it upon enrollment so that we may work out a specific meal plan. Parents will be required to provide special foods, ie: vegan “meats”, lactose prefered dairies, soy milk, etc.

GUIDANCE AND DISCIPLINE: To assist children in developing self-control, self-confidence, sensitivity in their interactions with others, and to ensure a peaceful atmosphere and prevent injury, our approach to discipline is a literal “hands-off” approach. Children are encouraged to work out their differences with each other on their own and adults are only there to facilitate a peaceful resolution by validating the children’s feelings and boundaries. We try to let children work out their own differences as long as no one will get hurt. We model problem solving by using democratic group discussion for behavior disrupting the group. Children help decide how to solve problems confronting the group.

We use a mixture of lots of different styles depending on what the situation warrants. We use a positive approach offering choices, logical consequences, re-direction, and encouragement for a job well done. We teach gentleness by being gentle, and respect for others by being respectful. We listen at the child’s level when they want to talk, and help them find words to settle conflicts. By establishing eye contact and remaining near the child in situations where they may be losing self-control, we are able to model the behavior in which we desire the children to have.

“Time-outs” are used when the child needs to gain self-control and are never viewed as a punishment. Children are allowed as much time as they need in order to refocus and recenter themselves before returning to activities. This time is given to the child if they choose it, it is never demanded of the child however a child who is unable to exhibit self control may be required to refrain from group activities. Personal quiet time may involve reading in the quiet corner, drawing or writing, or other free play activities that require individual reflection time.

PHOTOGRAPHY: Photographs of the children, by the children, and for the children may be taken from time to time. Your permission for photographs of your child to be used without compensation is implied upon enrollment, unless otherwise noted. In addition, careful documentation of preschool activities and projects are kept on a regular basis, and photo’s are posted to the Tina’s House website or Facebook page. Permission for sharing your child’s photo without compensation is implied upon enrollment.

OUTSIDE PLAY: Outside play is a mixture of on-site and offsite play. We attempt explore outside environments as often as possible. Exploration may include activities, projects or free-play.

We offer all types of activities during outside play including painting, creative role play, and project based learning. The outdoors give children opportunities to learn about nature through bird-watching, collecting bugs, leaves, rocks, and sticks. All types of materials are available for children to play and explore with. Outside play activities may change depending on the season or weather.

FIELD TRIPS: Field trips are offered on a limited basis, usually to support or help facilitate a child’s interest. Plenty of notice will be given before any planned field trip and a permission slip will be posted to sign if you would like your child to attend. Children not attending field trips will need to stay home that day and reimbursements nor credits will not be given for the missed day.Occasionally, Tina’s House Daycare will meet with other local providers at their home, free community parks, or libraries. Parental permission is implied upon enrollment at Tina’s House Daycare.

Any such trips will require parents to provide a CA State legal Child Restraint Car Seat and or Booster Seat (age and weight limit appropriate).

MANDATED REPORTING: The State of California requires that all members of child care institutions be on the lookout and report to the State, any and all cases of abuse or suspected abuse to a child. I am, therefore, obligated to report to the State any suspected cases of child abuse and/or neglect and I take this obligation very seriously.

FINANCIAL ASSISTANCE: Provider retains the right to provide financial assistance or discounts to families with extenuating circumstances. Any extenuating circumstance can be determined at the provider's discretion.

LIABILITY: Parent agrees to be responsible for any damages to the provider’s real personal property or to the property of another child at the facility caused by his/her child. Parent agrees to repair or replace said property promptly.

Parent agrees to indemnify and hold provider harmless for any and all liability for an injury to or death of any person, including provider and employees of provider, arising from any act or omission by the child of said parent in or about the facility or while under supervision / fare of provider.

ITEM FOUR: WELLNESS & MEDICAL POLICIES AND PROCEDURES

CHILD ILLNESS: I attempt to maintain an environment as free from illnesses as possible. Housekeeping and disinfecting procedures serve as key parts of this effort, however parents play a critical role in maintaining the health of the children in the program. A brief wellness check will be conducted each day of attendance upon arrival. I look for obvious signs of illness, and if a child is showing any signs of illness, the parent will be required to remove them from care immediately.

If your child falls ill during the day, he/she will be separated from the other children and you will be contacted to retrieve them immediately. If you are unable to retrieve your child in a timely manner, your back-up provider or emergency contact will be required to pick up your child.

After a suspected illness, your child must be symptom free for 24 hours without the use of medication, or with a doctor’s note stating they are not contagious, before returning to care. The provider has the right to refuse the doctor’s note if the provider feels that the child is still too sick to attend. There are no deduction in fees anytime your child is absent due to illness.

Children are encouraged to attend daily in order keep a routine, however for the protection of all children, your child should be kept home and will not be permitted onsite if the following symptoms are present:

● · A temperature of 100 degrees or higher ● · Diarrhea or vomiting● · Redness or discharge from the eyes● · Excessive or severe coughing that prevents the child from participating in routine

activities● · An excessively runny nose that requires constant attention● · Extreme unexplained fussiness or change in typical behavior that prevents the child

from participating in routine activities or makes it difficult for provider to properly care for other children.

● · ANY type of rash (all children presenting with a rash MUST be cleared by a doctor’s note)

UNACCEPTABLE Illnesses are: Measles, Chicken Pox, Hand Mouth and Foot Disease, Flu, Pink Eye, Conjunctivitis, Impetigo, Severe Colds, green or yellow mucus discharge from nose or eyes, Whooping Cough, Croup, Head Lice, Scabies, TB, Meningitis, Rubella, Rosella, Strep Throat, Scarlet Fever, Ring Worms, Pinworms, intestinal worms, or anything contagious and transmittable including any viral or bacterial infections. IF IT IS CONTAGIOUS THEN IT IS NOT ALLOWED AT SCHOOL. Any communicable diseases must be reported to the Health Department.

REPORTING ILLNESSES: Parents are obligated to report to provider any time the child is ill. The parent must also report to provider if the child has been exposed to another child with a contagious illness. Especially illnesses like Chickenpox, Measles, Hand Mouth and Foot Disease, etc. that have a 2 week to 3 week incubation period. At no time are parents allowed to mask illnesses by giving the child medications or fever reducers and then bring the child to school. Masking an illness and deliberately bringing a child to school ill can result in immediate termination. Always keep in mind - would you want someone to bring a sick child to day care and infecting your child?

IMMUNIZATIONS: All children are required by law to have certain immunizations and to have a record of those immunizations on file PRIOR to your child’s first day of care. All children must have their immunizations completely up to date before entering school. Parents are responsible for keeping these immunizations up to date at all times. Parents are required to bring at Xerox copy of the child's immunizations records and give to the provider upon enrollment. Parents are required to provide an up to date copy to the provider each time the child receives immunizations. This facility does not accept children who are not immunized. If your child gets an immunization, he/she may not return to child care that day. If you have any questions about which immunizations your child is required to have, please see the Parent’s Guide to Immunizations Required for Child Care published by the California Department of Public Health that was provided to you in your enrollment packet.

INCIDENTAL MEDICAL SERVICES: An Incidental Medical Services Plan of Operation is required to be on file and is written to ensure safe and accurate administration of medication and incidental medical services to all children in care. Tina’s House Daycare may enroll children who require services for any of the following:

Tina’s House Daycare will maintain documentation of medication (LIC9221) and Incidental Medical Services on log after every medication or service is administered.

Proper safety precaution will be in place. Staff must wear gloves when dealing with blood or bodily fluids, properly wash their hands, and properly dispose of instruments in an approved manner.

Tina’s House Daycare will have designated trained staff that will be appointed by the child’s parent and properly trained on the various incidental medical services the facility provides.

Tina’s House Daycare will keep documentation of training in the child’s file. Director will provide training on Incidental Medical Services to all new staff upon hire and annually thereafter.

Children are not permitted to transport medications to and from the facility; this includes medication placed in a diaper bag or backpack.

Tina’s House staff may not deviate from the written authorization from the child’s physician.

Tina’s House Family Daycare will have a designated trained staff on the premises when the child is present.

Medications that have expired or are no longer being used at the facility, will be returned to the child’s authorized representative. If the medication has not been picked up within one week of the day of the request, then medication will be properly disposed of by trained staff.

MEDICATION ADMINISTRATION PROCEDURE: Incidental Medical Service supplies and medications will be stored in an area off-limits to children in care, ensuring that it is secure, inaccessible, clean and under conditions as directed by physician or pharmacist. Medications that require refrigeration will be stored in a designated area of the refrigerator, separated from food and will be inaccessible to children.

Once all requirements are met, the designated trained staff will administer medications or incidental medical services by utilizing the following requirements: 1. Right Child2. Right Medication3. Right Dose4. Right Time5. Right Route (mouth, inhaled, etc.)

DOCUMENTATION: Any medication or incidental medical services routinely administered will be documented on a log by staff member who administered (LIC9221). Authorized representative must be informed of each occurrence when their child received medication or when incidental medical services are performed, via the medication log. If a child is administered an Emergency Supplemental Therapy (i.e. EpiPen Jr.), the child’s authorized representative and 911 will be called immediately. Also, an Unusual Incident Report (LIC624-626 B) will be sent to the Department of Social Services, Community Care Licensing. Immunization and Incidental Medical Services Policies are written by the State of California, Department of Social Services, Community Care Licensing and are mandatory as part of enrollment in any child care. These policies are written word for word as provided to the Child Care Provider by the Department of Social Services and cannot be modified.

OVER THE COUNTER/NON PRESCRIPTION MEDICATIONS: Medications used for teething such as Tylenol and Motrin (or the like), numbing gels, topical ointments, lotions or other over the counter medications including sunscreen, will only be administered if absolutely necessary and will follow the same guidelines and administration procedures as outlined in the Incidental Medical Services section of this agreement. A separate form is required to be on file if you wish for your child to use such items. All items must be provided by the parent and are required to be unopened, in the original container, and clearly labeled with your child’s name.

ALLERGIES: If your child has any allergies (food, environmental, etc.), a completed Allergy Action Plan must be written and signed by your child’s physician. You are required to notify your provider if there are any changes that take place after your child’s enrollment and the Allergy Action Plan must be updated accordingly. It is at the discretion of Tina’s House Daycare as to whether or not to continue care for children with severe allergies that would otherwise impair the ability to give appropriate care to all children enrolled in the event of an emergency.

EMERGENCIES: In the event that emergency services need to be called in order to provide treatment, care or transportation for your child to an emergency facility, the parent is responsible for any and all bills occurring from the incident.

A Final Word - Tina’s House admits all children regardless of race, color, nationality or religious background and will facilitate understanding to all the children regarding any differences in these areas. I want to exchange thoughts and information on your child whenever necessary. I welcome parents to visit during hours of operation at anytime and are encouraged to call, text or email when needed.
