1. From the Department of Labor website, click on the link in the yellow box for 1099G

information. The address is www.filectui.com

2. Click on blue link for access to 1099 G Tax Form System.

3. Enter USER ID and Password. This is the same sign-on when filing your weekly continued

claims. You must “Click” the box that you agree with the above warning before proceeding.

Note: If you haven’t used this account in over one year, you will need to create a new

account.

4. Select the year you are looking for. You may access the past 5 years’ 1099G forms.
