As part of this effort, we have asked each student to identify nurses during their clerkships to assess them with the Professionalism Assessment Tool. use NM Interactive click the “more” tab at the bottom right on the next screen click “Education Portal”

use Web Links on Epic go to the Quick Reference on

the main page click “Education Portal”

go to this FSM web site to access the login page of the FSM Education Portal:

https://fsmweb02.northwestern.edu/portal/login.cfm

You will need to log in to the Education Portal using the following username and password:

Username: pat123Password: pat4565!

After logging in, click “Professionalism Assessment Tool”. Enter the student’s name and select it from the list. Then click the “Submit” button. Click the “New” link that corresponds with the date range displayed. (You may need to change the Academic Year to 2014-15 in some cases). Constructive comments are incredibly valuable to the students as they reflect on their professional development. The more specific to the students’ behaviors that you can be in providing comments, the better.
