Overview The key objectives of Primary Health Networks (PHN) are:

Increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and

Improving coordination of care to ensure patients receive the right care in the right place at the right time.

Each PHN must make informed choices about how best to use its resources to achieve these objectives.

This Activity Work Plan covers the period from 1 July 2018 to 30 June 2019.

Aim of Activity To improve the health outcomes of people with chronic conditions in rural areas by providing access to integrated care.

Description of Activity

This activity maintains the delivery of currently contracted models by Western Victoria PHN, that respond to local chronic condition management needs within the context of Wagner’s Model of Chronic Care. The prevalence of chronic conditions and the associated costs upon the health care system necessitates the optimisation of health resources, particularly human resources. Each of the contracted providers identified the most appropriate model of care for their local area. The models are based on scope of practice and capability to ensure cost effective and efficient service delivery for improved health outcomes. This activity responds to the chronic conditions priority identified in the Western Victoria PHN Needs Assessment providing services in rural areas to increase access and reduce the impact of chronic disease. This activity involves the delivery of services within a system that improves the health and consequently health outcomes for people experiencing chronic illness. Each of the models must also focus on delivering optimal service delivery by developing and enhancing:

practitioners’ knowledge base and quality of care, while educating patients about their condition.

appropriate clinical governance to provide best care.

clinical information systems to provide optimal service delivery that supports patients to achieve their own health goals, and assists practitioners to manage patient’s care more effectively.

Target population cohort People with identified chronic conditions in rural areas of Western Victoria PHN receiving funding under the Chronic Conditions Model of Care Programme.

Consultation - HSI Component

Information from consultations by Western Victoria PHN staff members feeds into the ongoing development and enhancement of this funded programme and occurs through community consultations, surveys, phone consultations, face-to-face interviews, and forums bringing all providers together.

Health consumers accessing services through the Chronic Conditions Models of Care programme are provided with the opportunity to provide input to Western Victoria PHN through the completion the following surveys:

Patient Assessment of Chronic Illness Care Survey (PACIC-S). The PACIC-S measures health consumers’ experiences related to the Wagner Model of Chronic Care.

Patient-Reported Outcomes Measurement Information System (PROMIS) survey. The PROMIS survey is a set of person-centred measures that evaluates and monitors physical, mental and social health.

Assessment of Chronic Illness Care (ACIC) survey completed by providers.

Collaboration - HSI Component

Services provided under the Chronic Conditions Models of Care programme were developed in 2016-17 and implemented in 2017-2018.

Support contracted organisations in the development and maintenance of services and promote the program through journal articles and conference presentations.

Identifying emerging issues and working with contracted organisation to identify rectification strategies.

Provide quarterly reports to contracted providers with aggregated and individual organisation performance data. Including service delivery, survey and systems data to drive quality improvement and identify emerging issues.

Performance monitoring

Continuous review of Minimum Data Set, quarterly review of qualitative data and survey responses.

Performance reporting Provide performance reports to management and Board. Monitor contracted providers quarterly to ensure the appropriate delivery of each model.

Quality Improvement Undertake quality improvement activities to enable benchmarking with other PHN’s to determine how activities and performance measures against likeminded activities in other PHN’s. Work with local and interstate PHN’s to devise a benchmarking program that enables information, capability and capacity building to improve outcomes.

Evaluation

Completion of the ACIC survey which is a validated survey designed to evaluate the level and nature of improvements implemented in the delivery of Chronic Conditions Model of Care. Provide a report back to contracted organisations on aggregated results and the individual organisations results.

Documenting changes within the models in keeping with an action learning model to inform the evaluation of the overall activity and the individual models that have been contracted.

Contract administration Development of comprehensive schedules for each of the contracted organisations and all reports have been completed in accordance with requirements prior to generation of payment. Identification and implementation of any variations to contract.

Needs Assessment Priority Area (eg. 1, 2, 3) This activity is related to the ‘Immunisation’ priority presented on page 73 of the Western Victoria PHN 2017-2018 Needs Assessment update.

Aim of Activity This activity aims to ensure 95 per cent of one, two, and five year old children in the Western Victoria PHN catchment on the Australian Immunisation Register are immunised, in keeping with the National Immunisation Program Schedule and immunisation coverage target.

Description of Activity

Activity one This activity will align with the eight strategic priority areas of the National Immunisation Strategy for Australia 2013-2018. The key focus will be on childhood immunisation rates in Western Victoria PHN i.e. 95% of registered children are immunised as per the National Immunisation Program Schedule. The activities to be delivered include:

Maintain immunisation data for all childhood immunisation providers (general practices, local government authorities, Aboriginal Community Controlled Organisations [ACCOs] within the Western Victoria PHN catchment

Create overdue childhood immunisation reports from the Australian Immunisation Register to inform and increase the timeliness of immunisation services

Procure biannual Immunisation forums Procure and conduct quarterly immunisation provider network meetings in collaboration with

the State Government of Victoria Department of Health and Human Services Activity two This activity will procure an evaluation of the current PHN childhood immunisation model. Based on the evaluation the PHN will also scope and assess the feasibility of an outcomes based model to improve childhood immunisation rates, to be commissioned from July 2019

Target population cohort Children aged five years and younger in the Western Victoria PHN catchment on the Australian Immunisation Register.

This is a modified activity based on activity OP4, ‘Building capacity in aged care’, included in the Western Victoria PHN 2016-18 Activity Work Plan, and activity F3, ‘Building Capacity in Aged Care’ included in the Western Victoria PHN Updated Activity Work Plan 2016-2018. Since the Western Victoria PHN 2016-18 Activity Work Plan was developed, more and more instructive data relevant to understanding ageing and aged care issues has become available. Furthermore, recent Australian Government budget announcements relating to aged care and changes to the PHN programme need to be reviewed and considered. As such, Western Victoria PHN believes a more in-depth investigation into the aged care system, the ageing population, and how the latter might shape population health outcomes and health service utilisation in communities across the Western Victoria PHN catchment is warranted. Other aged care activities discussed in prior Activity Work Plan submissions will not proceed until Western Victoria PHN is assured these activities represent the most productive use of resources in the aged care space.

The forerunner to this activity was discussed on pages 20 and 21 of Section 4 of the Western Victoria PHN Baseline Needs Assessment, under the heading ‘Aged Care’.

Aim of Activity

Provide a short description (no more than 300 words).

The aim of this activity is to engage a consultant with expertise in the aged care sector and current reform to work with the Western Victorian PHN and key stakeholders to develop deeper insights in to the impact of the ageing population on population health and health services within the Western Victoria PHN catchment. This will culminate in the development of a Regional Primary Health Aged Care strategy to guide Western Victoria PHN’s future activities in the aged care space.

Description of Activity

Provide a short description of the activity, including how the activity will address the Needs Assessment Priority and what results the activity is expected to achieve within the planning period (no more than 300 words).

This activity will involve the engagement of an expert consultant to development of a Regional Primary Health Aged Care strategy incorporating relevant population health and aged care data, service mapping and consideration to the system impacts relating to the current aged care reform. The activity will engage relevant community stakeholders, primary care providers and consumers to develop an integrated roadmap for future work. In doing so, the activity is in keeping with

most recent Western Victoria PHN 2017-18 Needs Assessment update, which noted that, compared to Victoria and Australia, a higher proportion of the population in the Western Victoria PHN catchment is aged 65 years and over (calculation based on ABS 2016 Census of Population and Housing. PHN-level data aggregated from Statistical Area Level 3 regions). Furthermore, the number of persons in this age group is projected to increase in most local government areas in the Western Victoria PHN catchment in the years ahead (see State of Victoria Department of Environment, Land, Water and Planning, 2016. Victoria in Future 2016. Population and household projections to 2051). The strategy will assist Western Victoria PHN to pinpoint and prioritise which issues relating to ageing and aged care to progress to the next stage in the commissioning cycle.

Target population cohort Describe the cohort that this activity will target. The target population cohort for this activity will be persons aged 65 years and over. However, a comprehensive assessment of health system challenges posed by the ageing population necessitates an additional focus on persons aged in their 40s, 50s, and early 60s.

Consultation - HSI Component

Provide details of community engagement and consultation activities – please outline whether the consultation has already occurred, or is planned for a future phase of the activity.

Community engagement will be undertaken with relevant stakeholders, including primary health providers and consumers, to ensure that the strategy identifies possible future actions/ outcomes. This will enable Western Victoria PHN to co-design new services/system initiatives moving forward in future activity work plans.

Collaboration - HSI Component

List stakeholders that will be involved in designing and/or implementing the activity, including Local Hospital Network or state/territory government. Describe the role of each party.

In undertaking this activity, Western Victoria PHN will seek advice from the following:

Procuring the services of GPs to draw on their clinical knowledge to commission referral pages and health pathways clinical work stream activities. In addition to commissioning clinical knowledge, the health pathways pages are processed through a web portal managed by “Streamliners” to ensure consistency, and relevance to front line primary care providers.

Develop and procure place based innovation grants and practice based commissioned activities, built on PENCAT data collection, for rural providers to assist with development of High Performing Primary Care in rural locations.

This activity is designed to recognise that there are a number of place based community activities identified via the needs assessment. The aim of this activity is to establish a regional grants program to look for innovative activities that meet the objectives of the needs assessment and assist communities to test and demonstrate the merits of locally based innovative activities. It will also recognise that small communities and primary health service providers may have difficulty sourcing collaborative partners due to location.

Prevention, early detection and early intervention for bowel, breast and cervical cancer (National Screening Program).

Improving access for “under-screened” consumers, being our culturally and linguistically diverse, low socio-economic backgrounds, rural and remote and Aboriginal and Torres Strait Islander consumers by co-designing the service offer to these consumer groups.

Raising awareness and adherence to use of the Cancer HealthPathways among health professionals and the health services at system, organisation and individual level to promote quality cancer care and patient experiences.

Increasing the survivorship of patients in the region Supporting practices in recording and using patient data for quality improvement in key areas

such as smoking status, weight measurements.

Description of Activity

Screening rates for bowel, breast, and cervical cancer in Western Victoria PHN region as a whole exceeded national screening rates in 2013-14 and have continued to do so as captured in 2015-16 data (example 2015-16 Bowel Screening 45.4% vs 40.9% for Australia). However, there is significant variability across sub-regions with under-screened communities such as Corio-Norlane (Bowel 36.5%).

Informed by this national data and region specific insights, Western Victoria PHN will continue to focus on a number of initiatives to improve cancer-screening rates within General Practice, targeting under-screened, who are often vulnerable populations. Resources and services to support primary care practitioners are being further enhanced in 2018 to strengthen the cancer program and include:

Education for primary health care professionals – face to face, e-learning, smaller centres and extending to wider range of health professionals, use of tools such as i-PACED and building workforce capacity through nursing scholarships.

Development of e-patient information eg GoShare patient information for GPs to aid in patient health literacy.

Targeting vulnerable populations such as aboriginal communities and support for ACCOs. HealthPathways eg focus on psycho-social/supportive care referrals/support. Screening rates data - improve usefulness and accessibility of information for trending with

focus on under-screened populations. These initiatives reinforce our messages of prevention, early intervention and screening in the primary care setting. Western Victoria PHN continues work in collaboration with key partners such as Integrated Cancer Services (ICSs) and providers to support peak body programs across Western Victoria, such as Victorian Cancer Council Telehealth for Supportive Survivorship Cancer Project providing access to education, exercise and wellness programs for rural and regional cancer survivors and carers. Western Victoria PHN will continue to undertake targeted campaign on smoking cessation and other life-style induced illnesses using the QUIT and LiveLighter resources (Daylesford). As part of a State-wide PHN approach, Western Victoria PHN will work with two local Integrated Cancer services (ICS) to implement and embed the Optimal Care Pathways (OCPs) in 2018 Tranche 2 Oesophageal and Prostate Cancer and 2018-19 Tranche 3 Melanoma, Head and Neck and Pancreatic Cancers One new initiative 2018-19 is:

1. A community-led cancer screening program Department of Health and Human Services (DHHS) aims to increase the early detection of bowel, cervical and breast cancer by building local

3 HSI funding is to be used to deliver core functions within the PHN program such as population health planning, system integration and stakeholder engagement, as well as support to general practice which is not funded under the General Practice Support Funding Schedule. PHNs are able to use flexible funding to commission referral or health pathways activities (including non-staff costs such as ‘Streamliners’) but all associated PHN staff costs must be funded from HSI funding. HealthPathways activity to be undertaken by commissioned services should be separately identified as a Core Flexible Activity in 1. (b) Planned PHN activities – Core Flexible Funding Stream 2018-19.

PHNs cannot commission frontline services using HSI funding. PHNs may use HSI funding to subcontract specific activities under this stream, for example a health data analyst or consultant may be contracted to identify priorities for improved care coordination. Contracted or consultant arrangements are particularly appropriate for time-limited and specialist projects.

Practice support is to be provided through HSI funding and must be primarily delivered through PHN employees. Practice support cannot be commissioned out to a third party. Practice Support includes general practice support not funded under the General Practice Support Funding Schedule and support provided by your PHN to other practices, eg. allied health practices.

Alignment of clinical leadership activities to the Ten Building Blocks of High Performing Primary Care (Bodenheimer et. al, 2012). Procuring provision of Clinical Leadership coaching, and facilitate contracting services of GPs for advisor roles in key priority areas.

Aim of Activity This activity will provide funding for the procurement of co-designed models of after hours care, to improve availability and access.

Description of Activity

Western Victoria PHN will utilise the 2018-2019 period to consolidate learnings from the evaluation of a range of previously funded after hours activities and the After Hours Mapping and Stakeholder Engagement Project to guide commissioning of after hours funding going forward. Through this project, Western Victoria PHN will collaborate with key after hours system stakeholders to identify barriers and enablers to after hours service delivery and co-design innovative solutions that build the capacity and integration of after hours service provision in the region in a sustainable way.

2018/19 after hours funding will then be procured for the Access to After Hours Care Project to support

innovative and sustainable ways to address systemic after hours issues and improve integration of after hours services into the future. After hours service models supported through this funding are likely to be required to address some of the following; workforce issues including the recruitment and retention of GPs to participate in after hours work and the demand this places on a part time GP workforce, support for after hours services during period of peak demand, accessibility of after hours services in locations where there is limited access, significant geographical isolation and distance to travel to access services, equity of access to after hours services for vulnerable populations, working collaboratively with other local providers to share the after hours workload, changing work life balance needs of GPs less willing to work in areas where there is significant after hours commitments and the sustainability of after hours service provision.

Victoria PHN Regional Integrated Councils (comprised of clinical and community representatives) in November 2018 for input and feedback.

Procurement of co designed after hour models and system improvements in the Access to After hours Care Project will commence in November 2018. A minimum of fourteen weeks must be allocated to complete the procurement process, therefore it is planned for services to be contracted by mid to late February. This will allow sufficient time for the design of the activities to be procured under the Access to After hours Care Project and a longer funding period to allow for evaluation of the funded activities.

Continuation of Critical Area Support Grants until 30 June 2019 will ensure continuity of existing after hours services for the community until the new after hours funding arrangements commence. This will also allow a four month transition period for Critical Area Support Grant recipients to either be recommissioned under the new after hours funding arrangements or be decommissioned with adequate time to implement any required changes in after hours service delivery. All current providers will be provided the transition timelines to enable then to plan for their service needs.

Decommissioning

Critical Area Support Grants will be decommissioned from 30 June 2019 with opportunities for after hours funding available through the Access to After Hours Care Project.

Aim of Activity To facilitate patient access to the most appropriate and accessible urgent care in the after hours period following professional triage.

Description of Activity

A key focus of Western Victoria PHN is to improve the effectiveness and efficiency of after hours primary health care coverage in the Western Victoria PHN catchment. Western Victoria PHN provides coordination of urgent care in the after hours period through a streaming and information service and access to taxi transport assistance to and from urgent after hours care. Safety Link is the contracted provider of this service for Western Victoria PHN.

The streaming service is a fully integrated after hours service working collaboratively with the GP After Hours Helpline (Healthdirect Australia), Nurse on Call, Ambulance Victoria, local hospitals and general practitioners providing after hours services. The aim is to provide local care coordination after professional triage to identify the most appropriate response for patients with after hours urgent care needs.

In areas of limited after hours workforce and geographic distance from health services, taxi transport assistance provides access to transport to facilitate patient care at the most appropriate urgent after hours care provider. Established partnerships are in place with primary care providers, Safety Link, Ambulance Victoria and local health services to facilitate referral of patients to this scheme. Funded general practices and health providers will be required to ensure use of the National Health Services Directory so that patients are directed to the most appropriate point of care in the after hours period.

Aim of Activity To support the development of the current after hours workforce in rural areas.

Description of Activity

Based on the outcomes and recommendations of the evaluation of the Western Victoria PHN Rural and Isolated Practice Endorsed Registered Nurses (RIPERN) scholarships, Western Victoria PHN will continue to support the development of the after hours workforce in rural areas. This may include a revised scholarship model or more broadly support activities that optimise the utilisation of the current workforce within the scope of practice or allow the existing workforce to be upskilled and competent to work to the top of their professional scope, reducing unnecessary demands on general practitioners in the after hours period.
