Welcome!

This publication is reflective of Alzheimer’s Community Care’s commitment to providing care and service to our local communities during the past 20 years. The positive impact we have made on our families suffering with a neurocognitive disorder is singularly our greatest accomplishment.

Reports and surveys have documented these accomplishments throughout the years. The impact of our partnerships with law enforcement has made our families safer and their security has increased, as well. The Florida Silver Alert program is a testimony to that principle, which is that “we place a safety net around patients and caregivers every day.”

In October 1996, when Alzheimer’s Community Care received its 501(c)(3) Internal Revenue Service declaring it as a non-profit, our Founders envisioned our new organization centering its endeavors around the word “Community.” The early years were very challenging, and the Baxter Foundation’s visionary trustee, Don Baxter, along with our Founders, carried us during those early years and agreed upon the name of the organization.

Fast forward to 2017 – Alzheimer’s Community Care operates 11 Specialized Licensed Day Care and Service Centers, directs seven Family Nurse Consultants who work throughout Palm Beach, Martin and St. Lucie Counties, and has established seven core services within its continuous evolution of its mission, vision and values.

Dave Donten, Trustee of the Baxter Foundation and Gene Pranzo, Trustee of the Leo and Anne Albert Charitable Trust, have made and continue to make a great impact on the organization to advance our mission. The other dedicated individuals are our Board of Directors and staff. The leadership of our organization has made Alzheimer’s Community Care a special place within our society, as we move forward with the belief that a family-centered, comprehensive focus is the consistent strategy and is critical to its future mission and work for the next 10 years.

We need our supporters and deeply appreciate how they have helped to advance Alzheimer’s Community Care’s vocation well into the future. In return, we pledge ourselves to be change-agents so that human dignity becomes a fundamental aspect of all dementia-specific treatment. We believe we can continue to accomplish that promise through our relationships, collaborators, and partners, which is the secret to our sustainability, without a doubt.

HISTORY OF ALZHEIMER’S COMMUNITY CAREFour special founders - Gaston Jones, Frederick Kroll, James Gregory Fragakis and Mary M. Barnes - helped establish Alzheimer’s Community Care in October 1996. Each brought their own expertise and awareness to the non-profit organization because they were directly affected by Alzheimer’s disease and related disorders. More importantly, though, they wanted to influence change, so that this set of neurocognitive disorders would become known as a “family disease.”

They understood, believed, and advocated, that a dementia-specific, focused-care system embedded within local communities could solve problems that consistently put families in harm’s way. Such a system, they contended, could preserve the quality of life for both the patient and the caregiver throughout the duration of the disease.

This focus on “family attention,” for each patient and caregiver, has been a hallmark of Alzheimer’s Community Care’s model of care that continues to provide a one-of-a-kind experience for each family.

Today, Alzheimer’s Community Care is a unique health care provider and operator of the largest number of licensed Specialized Alzheimer’s Care and Service Centers throughout the country. Alzheimer’s Community Care’s staff members possess an unparalleled set of skills related to dementia-specific care. Those skills will continue to evolve as technology further drives the awareness of the disease and defines other medical practices for serving the ever-growing needs of our families and all of society.

Families and government entities continue to reach out to Alzheimer’s Community Care, seeking effective planning, care, and services for the challenging medical conditions brought on by Alzheimer’s disease and related disorders.

Why do they do so? What makes Alzheimer’s Community Care stand out? It’s our multidisciplinary team of specialists who work together to create a “safety net and infusion of resources inclusive of a care strategy” that takes into consideration the whole family’s needs. This relentless pursuit of an effective care plan on a progressive disease, which requires partnerships with nontraditional approaches, lends hope for every family that makes it to Alzheimer’s Community Care’s doors.

Education inspires both new health providers and solutions that will continually improve patient care and help to our families. Our Board of Directors and leadership team are committed to making new inroads with their collaborations and partnerships that will make our model of care accessible to every community that values their model of care for their local families.

Many important new care services have been created through the influence and expertise of the staff at Alzheimer’s Community Care. This influence is evident with the establishment of the first Alzheimer’s Medicaid Waiver (2005 to 2010), the Florida Silver Alert law that went into effect in 2011 and is saving many lives, incorporating those lost while driving, as well as lost on foot, and the Specialized Alzheimer’s Day Care Act that was signed into law in 2012.

As we celebrate two decades of service, we are proud to continue our legacy of teaching, practicing and advocating for the care and assistance that help thousands of families each month to learn that where there is help, there is hope.

A UNIQUE PERSPECTIVEBy G. MARK SHALLOWAY, Esq., Board Member since 1996

For the past two decades, I have been consistently inspired, amazed and gratified to have been a part of Alzheimer’s Community Care (ACC). Just as Alzheimer’s disease and related memory disorders carry a unique attack and impact on the patient and caregiver, ACC, since its inception, has developed counter-attacks with precocious programs and services born and nurtured with critical core values. These core values are the bedrock of its success. For me, the most significant values include its commitment to measurable, accountable dementia–specific standards for compassionate community-based care for the patient and caregiver; authentically innovative community-based services organically cultivated from the needs discovered from our diverse population; active collaboration with governments, professional disciplines and other service providers and vendors; and, continuing education and advocacy at every level.

How do I measure success?

ACC has a lifelong history of gathering empirical data used to guide and develop warranted programs and services, with keen attention paid to feedback from patients, families and other community stake-holders. ACC is accredited by The Joint Commission, a nationally–recognized accrediting authority.

Community-Based Care for Patient and Caregiver

Our commitment to the patient extends a safety net to our caregivers. Studies show that long-term caregiving creates a catastrophic risk

of a caregiving spouse’s death before the patient and catastrophic nursing home cost. ACC has created and defined the Family Nurse Consultant (FNC) program, a dementia-specifically trained nursing model to provide caregivers with the support, tools and resources needed to continue providing home-based care for their loved ones with better health and lower costs by delaying or avoiding a nursing home stay.

Robust Collaboration with the Community

ACC works with various sectors of the community it serves to leverage its impact. Here are some of my favorite examples to showcase: ACC trains and coordinates with first responders to deal with patients with dementia and caregivers in crises; ACC provides standards and support for dementia-specific special needs shelters; ACC revitalizes churches with space-sharing partnerships for dementia-specific adult day care; ACC engages in joint ventures with assistive technology vendors and government to help locate lost patients with the Silver Alert, ID Bracelet and Lost on Foot programs.

Continuing Education for Families and Professionals

One of our crown jewels is ACC’s Annual Alzheimer’s Educational conference at the Palm Beach County Convention Center. With its dual track approach for families and professionals dealing with dementia, ACC draws national speakers on research, interdisciplinary carepractices and an “on-site” day care forpatients so caregivers can attend.

Advocacy

With the compelling need for financial support and sound public policy, ACC works at all levels of government and private sector advocacy and technical support to promote awareness and information critical to thwart the personal and economic devastation of the disease. ACC works year-round with the Florida legislature and governor’s office crowned with an annual bus pilgrimage to visit and educate legislators and staff.

In short, the values of the ACC board and staff as well as our community act as a North Star in our journey to a cure with care, compassion, personal dignity and financial security along the way.

FOUNDERS’ AWARDThe Founders’ Award is the most prestigious award given by Alzheimer’s Community Care. The Founders’ Award is presented to an individual who has provided extraordinary support to Alzheimer’s Community Care and who embodies the founding spirit of this organization. This award represents the highest level of significant dedication

to the founding vision of Alzheimer’s Community Care. Our newest recipients, Dave Donten and Gene Pranzo, demonstrate the true virtues of generosity, willingness and wisdom.

As the Trustee of the C. Kenneth and Laura Baxter Foundation, Dave Donten has continued to ensure the originality and integrity of a remarkable gift to Alzheimer’s Community Care, one that the organization has honored and cherished over virtually the entirety of its first 20 years of existence. Since July 1997, nine months after the organization was founded, the headquarters for Alzheimer’s Community Care has been at the beautiful Baxter Building, located at 800 Northpoint Parkway in West Palm Beach, Florida. In addition, a Specialized Alzheimer’s Day Care Center has been located in the same building over that same time.

Mr. Donten is one of the founding shareholders of Caler, Donten, Levine, Cohen, Porter & Veil, P.A., and he has more than 30 years of public accounting experience in the area of taxation. He serves clients in the areas of corporate, individual, fiduciary, real estate, and construction taxation. In addition, he has spent extensive time in the area of litigation support.

Mr. Donten’s involvement with and commitment to Alzheimer’s Community Care can be traced to the early months of 1996, when a gentleman named Don Baxter became concerned with the increasing numbers of patients diagnosed with Alzheimer’s disease and related disorders in Palm Beach and Martin Counties. Mr. Baxter researched the availability of services and resources for patients and caregivers and came to the conclusion that an organization comprehensive in purpose and mission for caring for patients and caregivers did not exist. He envisioned the establishment of such an organization.

Ultimately, after Mr. Baxter contacted Mary Barnes, for her assistance in reaching out to individuals within the community to form a group to aid in the establishment of such an entity, he allocated funds to establish the foundation of this unique organization. With the enthusiasm and inspiration of such founders as James Gregory Fragakis, Gaston C. Jones and Fred Kroll, and other caregivers from the community, the dream of establishing a dementia-specific health services provider became reality in October 1996, with the formation of Alzheimer’s Community Care Association of Palm Beach and Martin Counties (eventually renamed Alzheimer’s Community Care).

Through the C. Kenneth and Laura Baxter Foundation, the construction of the building to house Alzheimer’s Community Care’s headquarters and specialized Alzheimer’s Day Care Center began in September 1996. Sadly, Mr. Baxter passed away in November 1996, never realizing his dream. The building project was completed in July 1997 in memory of C. Kenneth Baxter. “Mr. Baxter fully supported the mission of the organization, and we are pleased that the Baxter Foundation has been able to continue his legacy,” says Mr. Donten.

Mary M. Barnes, President & CEO of Alzheimer’s Community Care, adds: “The C. Kenneth and Laura Baxter Foundation’s generosity has ensured a dignified and respectful environment for the professional, effective and efficient care provided by Alzheimer’s Community Care to all caregivers and families who reach out for assistance. In a major way, the Baxter Foundation, under Dave’s leadership, has helped make Alzheimer’s Community Care a leader in the care and well-being of those families affected by Alzheimer’s disease and related disorders.”

FOUNDERS’ AWARDA quick scan of our 11 Alzheimer’s Specialized Adult Care and Service Centers reveals a recurring name – those in Pahokee, Port St. Lucie and Wellington all are entitled the Leo and Anne Albert Alzheimer’s Care and Service Center.

The late Mr. Albert was the chairman of the board of Prentice-Hall InternationalPublishers and widely recognized for his philanthropic efforts. His late wife, Anne, suffered from Alzheimer’s disease and when he learned of the mission of Alzheimer’s Community Care, Mr. Albert immediately valued the work of our organization. He attended many of our organization’s events, which was deeply appreciated by all associated with Alzheimer’s Community Care. In Gene Pranzo, the Trustee of the Leo & Anne Albert Charitable Trust and a recipient of our Founders’ Award, Alzheimer’s Community Care has a committed benefactor who has passionately supported our mission.

Also, Mr. Pranzo, the messenger on behalf of Leo & Anne Albert, has attended the openings of each of the three centers bearing the names of the husband and wife he recalls with great fondness. A quiet, dignified man, Mr. Pranzo delights when he sees firsthand how grants from the Trust have led to the beautiful refurbishments of existing buildings that are now filled with patients who are singing and engaged in a variety of activities.

The quality of life for our patients in the Specialized Alzheimer’s Care & Service Centers has been directly affected because of the grants provided by Mr. Pranzo.

“Leo and Anne would be so pleased with the many ways that Alzheimer’s Community Care has advanced its mission and is continuing to meet the needs of the families it serves,” says Mr. Pranzo. “Alzheimer’s Community Care is a very special organization because it touches lives at a grassroots level, which is so meaningful. I am humbled to be recognized with the Founders’ Award and am honored to be able to carry on the names of Leo and Anne Albert.”

The late James Gregory Fragakis, one of the founders of Alzheimer’s Community Care, had developed a strong personal bond with Mr. Pranzo, because of their childhood memories having both grown up in Brooklyn, N.Y.

“Words alone cannot express our deep gratitude to Gene and the Leo and Anne Albert Charitable Trust,” says Mary M. Barnes, President and CEO of Alzheimer’s Community Care. “Gene is a special man who values the extraordinaryneeds and challenges of our families. I once told him that Leo and Anne are sprouting more feathers on their humanitarian wings of goodness as they see the completion of another project. Gene cares deeply about what we do and the lives we touch. We are incredibly blessed to have him as part of our family.” At the formal dedication of the Alzheimer’s Specialized Adult Care and Service Center in Pahokee, in October 2013, a prayer was offered. It included these words: “Yes, the heart ... How can it love so much! Perhaps that question can be partially answered today as we ponder the great, heartfelt generosity of Leo and Anne Albert as expressed through the actions of their representative, Mr. Gene Pranzo. For today we see the results of what the human heart can desire and complete. For through Mr. Gene Pranzo the hearts of Leo and Anne Albert continue to beat. The hearts of Leo and Anne Albert continue to fulfill their dreams.”
