    1. RESERVATIONS: Once you have selected your camp date

    and location and have an estimated number o students who willattend, you may make reservations by calling the USA oce. Yourfnal numbers will be confrmed when you have submittedyour Enrollment Form and per person deposit.

    2. REGISTRATION: In order to secure your reservation,please complete the attached Enrollment Form and mail it inwith your $50.00 per person deposit no later than 45 daysprior to the start o your camp. The orm may be duplicated oradditional names. Send one Enrollment Form per team/squad(include all participants and non-participants). A $50.00 perperson registration deposit must be sent with the Enrollment Form.The $50.00 non-reundable deposit secures your place at camp andis applied to the total camp ee. Full payment may be sent with theEnrollment Form i desired. (Please see item #3 or detailed inorma-tion regarding payments)

    The On Campus ee includes all housing, meals, and instruc-tion. The Commuter ee includes instruction only. (Meals may bepurchased or commuters on a per meal basis at most acilities

    contact the USA oce or more inormation). The Non-Participantee includes housing and meals. Due to contractua l obligations withhost acilities, camp ees will be charged to all attendees re-gardless o age or participation. Adults may also audit studentclasses and attend directors workshops. Participants must be atleast 10 years o age to attend a Junior High Camp and atleast 12 years o age to attend a High School Camp.

    Once the USA oce has received your Enrollment Form and perperson deposit, a camp packet will be mailed to the person des-ignated to receive correspondence. This packet includes medicalrelease and waiver orms (please do not ax or mail these orms tothe USA oce), and an invoice or each team/squad (e.g. Varsity, JV,or reshman) and program refecting the nal numbers submitted onthe Enrollment Form. Please check these invoices or accuracyand contact the USA ofce i they are incorrect. Also includedin the packet will be any pertinent camp inormation.

    IMPORTANT - Camps may, and oten will, close to additional

    participants even beore the deposit deadline. Please secure your

    camp space by getting your $50 per person deposit, completed En-rollment Form, and signed Camp Policy orm into the USA oce assoon as possible.A reservation does not guarantee a spot atour summer camp.

    3. PAYMENT INFO: Payment can be made by credit card,school or cashiers check, money order or school purchase order. Per-sonal or organizational checks are not accepted. Make all paymentspayable to USA Summer Camps. Please no cash, personal checks,or payments made by individual squad members.A $20.00 eewill be assessed or each returned check.

    I you are paying with a purchase order, it should be written orthe entire camp balance and submitted with a completed EnrollmentForm. Make sure the school name appears on the purchase order.Teams submitting purchase orders without a completed En-rollment Form will not be registered.

    As an added convenience, payment can be made with Mas-terCard, VISA, Discover, or American Express. For deposit paid bycredit card, use the space or credit card authorization on the

    4. CANCELLATION/ADD/CHANGE POLICY: Any changes/can-cellations ater your Enrollment Form has been processed will needto be submitted in writing to the USA oce. All payments, except the$50.00 per person deposit, are ully reundable i written notice ocancellation is received in the USA oce at least 14 days prior to yourcamp opening. I you do not receive an invoice conrmation refectingyour revised numbers within 48-hours o submission, please ollowup with the USA Registration Department. CAnCEllATionS ArE o-iCiAl only WHEn rECEivED in WriTing AnD ConirMED byTHE USA oiCE.USA will ONLY accept cancellations in writing.

    Because o acility deposits paid by USA, no-shows at camp or can

    cellations received less than 14 days prior to camp will not receive antype o reund AnD Will bE rESPonSiblE or THE Ull TUiTionPayments may be applied to substitutes; however, i there is no alternate person, the $50.00 deposit cannot be applied to the balance othose attending. Should a no-show individual at a camp havean outstanding balance (e.g. unpaid school purchase orderetc.), FULL TUITION WILL STILL BE DUE for the individual eventhough she/he did not attend camp.

    5. REFUND REQUESTS: A separate reund request or cancellations received in writing at least 14 days prior to the camopening must be submitted in writing to the USA oce by Oc tobe1, 2009. Again, the reund request is separate rom cancellationrequest. No reund will be issued beore the end o camp or ateDecember 31, 2009.

    6. ROOM ASSIGNMENTS: Room assignments will be madeby the team representative upon arrival at camp. To expedite thiprocess, please prepare your own rooming list (two to a room amost locations) and bring it to camp.

    7. FULL CAMPS: Many USA camps ll to capacity. I bed spaceis not available when you register, you will be notied immediatelregarding alternate camp dates.

    8. CAMP CHAPERONE: Each school is required to bring at lea sone adult chaperone that is over 21 years o age. The camp chaperone ee is $125.00 and is limited to one per school/organization.

    Chaperone responsibilities primarily involve assisting and supervising students at night. Chaperones are assigned to areas wittheir own students; however, they may be asked to oversee studentrom other teams.

    Any additional adults (directors, coaches, etc.) should registeon the Enrollment Form and pay the Non-Participant ee.

    9. COMMUTING COACHES/ADVISORS FEE: Commutinteams may register one (1) coach/advisor to attend camp at no costAny additional coaches/advisors/non-participants may attend at thrate o $25.00 per person. Please be sure to list all commuting adulton the Enrollment Form.
