FORM Athletics is a tribute to hard working and dedicated athletes. Born through the cultural vision of two men, FORM Athletics launched January 2010 in California.

Urijah Faber, “The California Kid,” is one of the most successful featherweights and unfailingly popular athletes in the history of mixed martial arts. The fans’ love for Faber is evidenced by WEC record-breaking viewership, doubled attendance numbers and nearly three times the total gate when Faber is on the

fight card, as when Faber is not on the fight card.

Yet it is Faber’s mindset – as much as his world championship titles and beachy good looks – that draws in fans. Faber’s sincere dedication to advancing the sport is what led him to cofound FORM Athletics. “Urijah and I are both passionate about doing something meaningful,” says FORM founder Mark Miller. “We want to make people aware of the positive benefits of sports and a healthy lifestyle.”

True to his nickname, Faber is “all about being outside and enjoying Califor-nia.” Born in the California town of Isla Vista in 1979 and raised in a commu-nal environment in the northern part of the state, Faber learned healthy habits from an early age. He logged an outstanding high school wrestling career before becoming an all-time win leader, National medalist, and, eventually, coach at the University of California, Davis. The young contender launched his MMA career almost immediately upon graduation in 2003.

Known for his lightning-fast hands and incredible takedown skills, Faber captured a championship belt in only his third professional fight and has won nearly 90 percent of his pro contests overall. (At one point, the 5’6” athlete held three different MMA titles simultaneously.) In his very first World Extreme Cagefighting match, Faber won the Featherweight Championship; and he went on to successfully defend that title for more than two consecutive years. He currently contends for the WEC from a home base in Sacramento, CA.

Optimistic, clean cut, and lacking pretense, Faber credits his success to discipline, hard work, and a respect for the artistry and technical aspects of traditional martial arts – all combined with creativity that’s completely his own. Fans, who say that The California Kid brings a refreshing surf-and-skate style to the MMA ranks, are especially appreciative of the new techniques Faber contributes to the sport. The accessible Faber believes in a mind-body connec-tion and brings positivity to all that he does. “I’d like to be remembered as an innovator,” the athlete affirms, “both inside and outside the cage.”

For nearly three decades, FORM Athletics president, CEO, and founder Mark Miller has dedicated himself to excellence in sales, marketing, and

general management of consumer products companies that focus on action sports and youth culture. Miller was responsible for launching the insanely successful M3 snowboard line, and his work with DC Shoes made the trademark a household name. When Miller identified the underserved market and huge potential of Mixed Martial Arts and likeminded sports, he seized the opportunity to “build a business, lead a team, and do something meaningful.” So like the athletes Miller works with, he possesses discipline, finely honed expertise and bound-

less energy. With Miller’s single minded focus and vast experience, FORM Athletics went from concept to clothing line within mere months.

As vice president for Yoshida Recreation Products (1996–2001), he managed global marketing and sales, cultivated partnerships with global-leading brands and companies, spearheaded innovative business strategies, marketing, and sales plans, and made headlines (and major profits) by founding M3. Subsequently, as he rocketed to the post of senior vice president and general manager of the Americas at Quiksilver’s DC Shoes (2006–2009), Miller more than doubled regional revenues to well over $300 million in his first 2 years, despite a slumping economy. With additional experience at K2 Sports and his own Mark Miller & Associates consultancy, the forward-thinking executive has built a reputation for being dynamic aggressive, high powered, ultimately delivering outstanding results – the same qualities he recognizes in athletes and especially in Mixed Martial Arts.

At home in California, Miller is an MMA fan, he’s into snowboarding, guitar, video games, and, most of all, his family. Miller sees FORM as another way to make a positive impact, not only with outstanding products, but with corporate leadership. “I’m a big fan of leaders,” he comments, citing groundbreaking figures in music, sports, and the global scene. Miller goes on, “FORM is a trib-ute to our athletes’ hard work and dedication. I want to do something signifi-cant not only for myself, but for my family, the sport, and society.”
