Change & Progress in 2011

On behalf of the Board of Directors, our staff and the patients we serve, Access Carroll once again extends our great appreciation to every hand that contributed to the accomplishments of 2011. The generous support and commitment of volunteers, community partners, and financial donors enabled Access Carroll to continue to provide high quality and coordinated health care while making significant strides in the EVERY HAND COUNTS Capital Renovation Project.

After more than two years of hard work, mixed with delays and unexpected changes, we persevered and are pleased to announce that our renovation project is moving forward in 2012! At the new location on Distillery Drive in Westminster, we will have more than triple our current operational space for medical services which will also include a four-operatory dental suite for family dental services, the first of its kind in the county serving the at-risk population. Additionally, this location offers another available 5,000 square feet for future expansion and integration of care with our community partners who also provide health services to the financially disadvantaged in our community.

You may notice that Access Carroll has taken this exciting time of change and progress as an opportunity

to create a new identity. We believe this new look conveys the renewed energy and innovative approach of our organization as we move forward and embrace the change necessary to be on the cutting edge of patient-centered and integrated health care.

In the pages that follow, we are delighted to share with you the amazing and continuing story of Access

Carroll. Every piece of statistical information reflects a life impacted for better health, the service of volun-teers and partners, and a stronger community. Again, “thank you“ to everyone who has made this dream a continuing reality. We look forward to 2012 and hope you join us in our mission…where every hand counts!

Dreaming Big… it’s what we do at Access Carroll

We believe every individual should have access to health care that is coordinated, accountable, culturally sensitive, community based and of high quality. Our mission is to improve pathways to health care for uninsured, underinsured, and low-income residents of Carroll County. We want our neighbors—our patients—to be healthy from head to heart, from teeth to toes. We rely on private financial contributions, grants, in-kind services, and donated medical supplies to provide this comprehensive care, which includes primary care, laboratory and radiology testing, referrals to specialists, health education, case management, public assistance support, critical medication assistance and more… so much more.

We invite you to share our dream. Whether you help with a big and or a small hand...every hand counts!

Patient-Centered and Integrated Health Care

Patient-Centered Care puts patients at the heart and center of their own health care. With the support of a medical team, patients are able to manage acute and chronic illnesses, reduce health related complications, and lead healthier lives. Integrated Care takes fragmented pieces of health and social care services and ties them together in a seamless and organized system of delivery. Patients benefit from having coordinated care from a personalized, whole-person approach. Access Carroll has been practicing patient-centered care since 2007 and has developed a recognized model of care coordination. In 2009, Access Carroll partnered with the Carroll County Health Department Addictions Bureau to provide integrated medical and recovery support care for our community. With the completion of our “Every Hand Counts” capital renovation project, patients will benefit from having medical, dental, recovery support, and other coordinated health care services at one location.

Care Coordination & Medications

Since opening in 2005, Access Carroll has served a majority of chronically ill patients, many of whom need extensive care coordina-tion and long-term medication assistance. Access Carroll directly addresses these needs with two targeted team members, the Care Coordinator and Medication Manager. A patient-centered and integrated medical home is also critical to ongoing chronic disease management. The patient-centered team is compromised of an essential core staff that supports the services of 16 on-site and 57 community in-kind and volunteer providers. Access Carroll is appreciative of the continued commitment and support of strategic partners, volunteers, and donors who made possible chronic disease services in 2011.

The Care Coordinator functions as a specialized case manager for the most at-risk residents in our community, helping them achieve patient-centered and integrated care. Care Coordination at Access Carroll includes chronic disease management, specialty care, high-end diagnostic studies, SSI/SSDI applications, Public Assistance applications, and SOAR (preventing homelessness).

More than 75% of Access Carroll’s patients are diagnosed with a chronic disease and are prescribed an average of four medications. Unfortunately, uninsured and low-income patients generally cannot afford their medications and end up with critical health conditions that are difficult to manage. The Medication Manager, an essential member of the patient-centered team, assists patients, physicians, and other members of the healthcare team in determining the most efficient and affordable means to acquire acute and chronic medications, whether it be through reduced cost formularies, local pharmacy voucher programs, generic medications, brand name medications, or through Patient Assistance Programs (PAPs) directly from Pharmaceutical Companies. With the support of generous donors, the Medication Manager has been able to acquire $978,274 during 2011 of chronic disease medications through Pharmacy Assistance Programs at no charge to the patient., and maintain a limited formulary of on-site medications valued at $87,225.

SPECIAL EVENTS In 2011, Access Carroll raised funds and friends through four special community events: The Third Annual Bike Tour, Westminster Fallfest, The Seventh Annual Chili Cook Off, and the Big Band Merry Christmas Concert. The support of the Carroll County community is critical to the general operations of Access Carroll—literally keeping our doors open!

Access Carroll was the recipient of several community events, including the annual CROP Walk, Sykesville South Carroll Rotary Club BINGO, and first-ever Celtic Canter. Thank you to everyone who raised funds on behalf of access to health care for low-

I’ve been a volunteer at Access Carroll for a year and a half. The first person I met was Sandy Lynch; she gave me a tour and explained what Access Carroll was all about. I liked Sandy and what she said about the clinic. I thought I would like to volunteer there and be able to do some-thing for people who need help and give back to the community.
