Many changes can be approved at the college level.

• Change in list of courses on graduate certificates• Change in list of courses on concentration• Change in list of courses on electives • Change in list of courses on graduate options• Change in list of courses on major • Change in list of courses on minor• Change in list of courses on undergraduate credit-bearing certificates if total semester hour

requirements are eighteen (18) or fewer and the certificate is housed in a unit associated with an academic college

• Deletion of undergraduate credit-bearing certificate housed in a unit associated with an academic college

Other changes, such as a change in degree requirements, need SRC approval.

• Creation of a concentration in an existing major or minor• Change in degree requirements• Changes in number of credit hours on a graduate certificate, graduate option, major,

minor, undergraduate certificate over 18 hours, or an undergraduate certificate housed in a unit not associated with an academic college

• Change in titles of degree, major, minor, or graduate certificate/concentration/ option.• Change in list of courses on an undergraduate credit-bearing certificate proposed by an

entity not associated with an academic college• Deletion of an undergraduate credit-bearing certificate housed in a unit not associated with

an academic college

Approval levels are almost identical to the approval levels for the course-related proposals except for the

following items which need Academic Senate approval.

• Deletion of concentration, graduate certificate, graduate option, major, or minor • Deletion of a degree • Change in General Education Program: University Program or Competency requirements• Creation or deletion of a designator (same as course-related proposals)

Blue Form: New Program Request

• Revised Blue form was approved on May 2, 2012.

– Removes confusion of when to complete the APC, Blue Form, and additional information for the President’s Council.

– Condenses the APC, President’s Council, and Senate requirements into one form.

– Assessment plan MUST be approved my Assessment Council prior to submission to PECC, UCC, and Grad Committee. Not needed for APC.

– Information required to complete the form can be drawn from information sources such as Assessment, OFIS, and Institutional Research.

CAD Changes Fall 2012VII. Suggested Course Outline (CAD, page 13)

This section lists the topics to be covered, and the learning activities/assignments designed to achieve the stated objectives.

For the outline, indicate a sequence of topics that reflects a logical division of the course. The scope of topics must be aligned with the stated learning objectives for the course. The topics may be divided by percentage of the course time devoted to a topic, time in hours, or by weeks. A 3-credit course involves approximately 40 contact hours of instruction. Adaptations to this timeframe (courses in either compressed or extended timeframes or in hybrid or online formats) are expected to maintain the requisite number of contact hours.

Best Practices• Ensure the information on the Green Form reflects exactly what is on

the MCS. In addition, be sure the Green Form reflects the information stated in the minutes (hours, lab hours, etc).

• New Programs– When designing a new program make sure hours add up correctly.– Contact Laura for bulletin descriptions.– Contact Mike Carson if you have questions regarding Options,

Emphasis, Tracks, and Concentrations.• Be sure to notify stakeholders (students, advisors, faculty) when

designators are changed….good communication.• Use caution when planning to make changes to a new curriculum, since

these changes affect faculty, staff, and ultimately the students newly enrolled in the program.
