Single Use Trial Lenses The 'remote theoretical risk' of transmitting vCJD through contact lenses created something of a dilemma among practitioners used to relying on diagnostic lenses in order to assess best fit and prescription for a potential rigid lens patient. It also created a resistance to prescribe a product that could not first be tried when tolerance and suitability are so critical. The Cantor & Nissel initiative to supply single-use trial sets in PMMA to a practitioner's preferred design has ensured consistency - and even an upsurge - in the RGP market. Our free trial set policy means that the busy practitioner can continue to perform trials as before, but with single-use lenses. These can be promoted as a benefit - presented to the patient in sterile, sealed, condition and disposed of after use. Each set is accompanied by a supply of re-order forms for replacement of the single-use lenses as they are disposed of. Our RGP lenses all carry the right to up to two free exchanges if required and there is a 75% credit if the patient eventually discontinues. The single-use trial set does, however, largely remove the risk of eventual incompatibility. Single-use trial sets are supplied principally in our market-leading Oxyflex 4F and Metr02 designs ex-stock. They can also be produced for you in the traditional Percon design or in any design of your choice which we will then be pleased to reproduce for you for your Rx requirements.

New Bellina and New Fabula Cosmetic Lenses Cantor + Nissel have used their unrivalled expertise in hand painted and prosthetic contact lenses to develop two outstanding cosmetic lens designs available in plano and powered forms on a quarterly replacement basis. New Fabula lenses are a two colour composition based on the results of a specialist design programme to develop a ‘natural’ iris pattern. This iris pattern is available in six carefully selected colours. New Bellina lenses include an additional limbal ring to further improve the appearance of the lens. Availability and delivery Plano lenses are available from stock. Powered lenses are made to order which normally takes approximately 10 working days. ChromaGenTM Lens design Colour blindness is a condition affecting 0.5 per cent of women and eight per cent of men. The ChromaGenTM lens is tinted to allow the brain to perceive images differently, enabling the patient to discriminate between colours which they could not normally ‘see’. Hand Painted Lenses Fitting and parameters Any combination of parameters and powers can be manufactured, including Toric lenses. Lenses can be made up to 22.00mm diameter and can be stabilised if a non-radially symmetrical effect is required.
Colour details Photographs, iris buttons, prostheses or artwork can be copied.
Clear pupils Clear pupils are available in any diameter. Because the opaque is throughout the thickness of the lens, there may be some glare when very small pupils are requested (particularly with high plus prescriptions). This may be resolved by adding a black annulus around the pupil. This may also improve cosmesis.
Black pupils Occlusive black pupils are available in any diameter. Because the black pupil is added to the front of the lens, a larger than expected pupil may be required to get good occlusion. If this is not cosmetically acceptable, it is possible to paint an opaque ‘patch’ matching the iris around a smaller pupil to achieve occlusion. Opaquing The colours that are used are transparent and on most eyes require a white salt that is precipitated within the lens matrix to be effective. This is slightly translucent and the apparent colour of the lens will be affected by the colour of the eye; the same lens will appear darker on an Aniridic eye than when worn on a light, scarred cornea. Please indicate the condition of the eye on which the lens is to be worn. It is sometimes possible to use lenses with no opaque when fitting totally white corneas. Fading colours is not usually a problem with these lenses but there is a small percentage of patients whose tears may remove the opacity from the lens. If this happens, the lenses can be re-opaqued for a nominal charge but it is possible that the problem will recur. A thicker lens may reduce the loss of opaque material.

Colour adjustments Although every attempt is made to be accurate, it is not always possible to exactly reproduce the required colour. If a lens is not acceptable, it may be returned and we will attempt to adjust the colour. ‘Offset’ lenses Strabismus can be masked by using a large stabilised lens with the iris painted eccentrically and the scleral portion opaqued, coloured and veined. (A separate information sheet is available on this.) Special effect lenses Lenses can be produced for special effects for photography, film and television. Reproducibility Each lens is individually produced and we cannot guarantee exact duplication. It is possible to create a photographic record of the lens and this can assist when reordering lenses; a digital image is retained in the studio and a print is despatched with the finished lens. By using the reference number included in the print when reordering we are able to access all the details of the lens. A charge is made for this service which also covers any subsequent photographs of lens adjustments. Delivery Lenses are normally despatched 10-21 days from receipt of order. Lenses that require special designs may take a little longer. It is sometimes possible to make lenses more quickly, however these ‘rush’ orders attract a surcharge of 30%. Hydrolens Hydrolens is manufactured to ensure a strong, clear lens which resists discolouration. Sometimes patients will need soft lenses which are outside the range of stock parameters; high powers, large or small diameters and steep or flat base curves. A graduated aspheric lenticular zone ensures minimum overall thickness in any power. Every prescription is optimised for handling characteristics and oxygen transmissibility using a computerised lens design facility. Nissel Keratoconus II soft The Nissel Keratoconus II Soft lens is designed to fit Keratoconus and irregular corneas. In most cases, it is expected that the prescription lens will
be a toric so the trial lenses are presented with toric stabilization. Manufactured from a 58% water content GMA material, the Nissel Keratoconus II Soft provides excellent oxygen permeability and exceptional wearing comfort for keratoconus and post-graft patients. The front surface is aspheric, calculated to provide the clearest possible vision for each patient. The fitting system has been designed so that the on eye characteristics of the trial lenses
match the prescription lenses as closely as possible. The Nissel Keratoconus II Soft trial lenses are designed with dynamic stabilization and have a range of powers to allow diagnostic evaluation with a power as close as possible to the final prescription. Orientation marks are at 12 and 6 o’clock. The location of the trial lens mark is important and must be noted when a toric is ordered, as it affects the axis direction of the final lens. All trial lenses are stabilised using the same design as the likely prescription lens. All trial lenses have orientation marks to enable the practitioner to evaluate the orientation of the lens when it has been stabilised on the eye.
Ideal fitting characteristics After the lens has been in the eye for a period of 5 to 10 minutes, the following should be observed in an ideally fitting lens:
If bubbles form under the lens on insertion, these should settle within 2 to 3 minutes
There should be vertical movement of approximately 1.00mm after a blink
The lens should have fast positive return to the central position after manual displacement

Pastel Tints Cosmetic enhancement or colour change Pastel tints will enhance the colour of light eyes but will have no effect on a dark coloured iris. They are particularly popular in countries where light coloured eyes can be made a stunning shade of blue, aquamarine, violet, green or brown. Pastel tints can be applied to both iris and pupil areas. It is recommended that the pupil area is tinted as well as the iris because on occasion a ghost image may be noticed by the patient when the clear central zone is smaller than the pupil size. Clear pupils can be supplied if required. Pastel tints can be combined with an opaque backing in 67% water content material only to provide cosmetic lenses which will change the colour of an eye. A black pupil is also available in 67 material.

Apex Lenses Design and material In cases where a patient may have a scarred or disfigured cornea. Either as the result of trauma or surgery, an apex lens may be used to bridge the scarring. An apex lens is a rigid lens made in a Bicurve design with a very large total diameter, typically 12.00mm or 12.50mm. Apex lenses will normally have a back optic zone diameter of 10.00mm. It has been traditional to produce lenses in clear material although nowadays many practitioners find that neutral handling tint is beneficial for the patient. The apex design can be produced in any material. The powers of these lenses is frequently high plus and it is therefore possible to choose any level of permeability with confidence. Bifocal RGP Lenses
Segmented bifocal In this design, a segment is cut using a high precision offset lathe. The result is a D-shaped segment of reading power which is orientated by means of a prism to sit inferiorly when worn. Solid segment bifocals are fitted similarly to a standard lens. A total diameter of 9.40mm is recommended as a first choice. Concentric bifocal Concentric bifocal can be manufactured in any RGP material. The central zone, typically 3.50mm in diameter, is powered for distance vision. This is surrounded by an annulus of reading power. The lens-cornea relationship must be designed to provide on-eye dynamics such that the lens will translate on downgaze. The lens must be well-centred on forward gaze to make sure that the patient is looking through the distance zone. Pupil diameters may be varied to suit individual patient needs.

Keratoconus RGP Lenses Keratoconus is a corneal ectasia that results in irregular astigmatism. Although keratometry can provide useful fitting information in the early to moderate stages, corneal mapping devices are better for interpreting the corneal topography, especially in the intermediate to later stages of this disease. Contact lens fitting in cases of keratoconus can test the skills of the practitioner. Diagnostic lens evaluation is mandatory. Currently, the most widely accepted rigid corneal lens fitting philosophy is one in which the intention is to distribute the weight of the lens between the cone and the more normal peripheral cornea. In cases where the cone is well established, this results in a lens showing an apical contact area of 2-3 mm, an intermediate clearance zone and a mid-peripheral contact annulus with standard edge clearance at the periphery. Stock artificial eyes These can be supplied singly from the size chart in any colour from the range of 72. They are also available in sets of 24, comprising 12 right and 12 left, presented in a carrying case. We usually supply a range of colours to customers’ indigenous requirements although any colours are available from the 72-sample range together with a variety of appropriate scleral shades and veining structures. These sets have the advantage of dual usage: they serve as a very good fitting set and also an excellent colour-matching set. Stock conformers We supply these in the same way as the stock artificial eyes but in blank form: usually white with the same sizing and set format. Loan sets We operate a loan system of fitting sets and colour matching buttons and impressions. Goods are charged at full price and then credited less a handling fee on their return.

Standard iris buttons We produce a set of 72 colour matching buttons which range in colour from palest blue through to darkest brown. They are all numbered on the back and presented in a carrying case. The dimensions are 11.00mm diameter iris with 3.50mm diameter black pupil. A smaller set of 18 buttons is available which can be any combination from the set of 72. The buttons are also available singly. Scleral lenses The unique benefits that scleral lenses offer ensure that they maintain a specialised role in most contact lens practices. Many practitioners opt for scleral lenses because of their therapeutic and protective benefits. Indications for scleral lenses include patients who have undergone corneal surgery or transplants, ocular surface disorders, abnormal corneal topography, tear retention in severe dry eye as well as protection from abnormal lids and lashes. Scleral lenses can also be used to manage ptosis by use of ptosis props. Haptic cosmetic lenses - shells These are shells: lenses which cover the whole of the front of the eye and sit just inside the lids. They can be made in any size, either taken from an impression or from a fitting shell. Any colouring, diameters and prescriptions are available with either a clear or a black pupil. The scleral portion can also be painted or left clear. Any thickness is available from 1.00mm to 3.50mm (an artificial eye would be made with greater overall substance). Hand-painted contact lenses The highly skilled team of artists assembled at Cantor & Nissel Limited produce a large range of hand painted scleral and micro-corneal lenses. Increasingly Cantor & Nissel Limited work from photographs and digital images. Digital images can be e-mailed directly to the company at [email protected]. These images are then printed out on a high-quality colour balanced printer for the artists to work from. We do recommend that the JPG format is used whenever possible as this offers the best colour depth and resolution. Parameters should include base curve, iris diameter, pupil diameter, whether the pupil should be black or clear, and the power (if required) in dioptres. Alternatively, iris matching sets and scleral matching sets can also be used for colour copying. Colour matching We offer an exclusive one-to-one colour matching service with our artist. If a patient wants a perfect colour match we can hand-paint a match of the ‘good’ eye during a visit to our studio at Hemel Hempstead. This takes about two hours. Once this is completed there is then a permanent record should the patient ever need a replacement.

A range of ¾ spheres are produced normally for use in puppets and models which are manufactured from a specially formulated resin. Spheres vary in both size and price, up to a maximum size of 152mm. The Early Presbyope: EV Lenses The plus effect inherent in the aspheric optics of the EV38 product will enable most marginal presbyopes to read at normal reading distances. Simply fitting the patient for distance with either the EV38 lenses in both eyes will usually provide the desired amount of addition for near vision and delay the requirement for a spectacle over-correction. If there is still a slight deficiency in near vision, the addition of +0.25/+0.50D to the lens in the non-dominant eye will usually correct the situation without detracting from binocular distance vision or upsetting distance stereopsis. Step 1 Use spectacle Rx with usual allowance for BVD where applicable.
Step 2 Transpose cylinder to minus form by adding 25% of the cylinder to base sphere only to cylinders in excess of - 0.75D.
Step 3 Allow lenses to settle for about 10 minutes.
Step 4 Check binocular acuity for near without occluding either eye. If satisfactory, proceed to check distance and correct, if necessary.
Near additions of up to + I .50D may be corrected by adding +0.50D to +0.75D to the non- dominant eye without affecting distance stereopsis.
The addition of +0.25D to the dominant eye will not affect the distance stereopsis but will provide satisfactory binocular near vision.

The Advanced Presbyope Presbyopic Correction Requiring +1 .25D to +2.75D Addition The PV Series progressive multifocal contact lenses are ideal for today’s demanding patients. Step 1 During the refraction, identify the dominant eye.
Step 2 Select the appropriate lens for the dominant (distance) eye and the correct lens for the non-dominant (near) eye from the chart, and select the appropriate lens base curve.
Step 3 After insertion, allow the lenses to 'settle' for 10 minutes.
Step 4 Check binocular acuity for near first, then intermediate and distance in normal light without occluding either eye.
Step 5 If distance and near binocular acuities are within a line or two of patient's potential, leave the lenses in for another 45 to 50 minutes to equilibrate the eyes.
Step 6 If correction is needed, binocularly over-refract the reading (near) eye first, then the distance eye, without occluding either eye or using a phoropter. Change lenses if necessary.
Use spherical equivalent correction, i.e. transpose cylinders into minus form then add half of the cylinder to the spherical distance correction before checking the chart.
Because of the 2.OOD power spread, 0.25D can have unusually positive effects on acuity.
When it is difficult to reach a satisfactory balance between distance and near, try a flatter lens.
The Emmetropic Patient. If no distance correction is required, all that may be necessary is to fit the patient with one Cantor + Nissel presbyopic lens in the non-dominant eye as in Step 2.

Optimal Modified Monovision Front Surface Aberration Controlled Lenses Standard spherical monovision rarely gives acceptable distance stereopsis and the need to suppress the non-dominant eye in distance vision is frequently unacceptable to the patient.
Cantor + Nissel lenses revolutionise the approach to monovision fitting and optimise the potential for success by exploiting the special characteristics of aspheric lenses. Simply fit a Cantor + Nissel multifocal lens in the non-dominant (near) eye and a single vision aberration controlled lens in the dominant (distance) eye. The result provides positive stereopsis for both distance and intermediate vision, leaving the patient monocular only at near, which is well-accepted by most patients.
Step 1 During refraction, identify the dominant eye.
Step 2 Select appropriate EV38 lens, using spherical equivalent Rx, then select appropriate lens base curve.
Step 3 From the non-dominant near chart, select a PV Series lens having the appropriate base curve.
Step 4 Allow lenses to settle for about 10 minutes. Check binocular acuity for near first, then intermediate and distance in normal room light without occluding either eye.
Step 5 If correction is required, over-refract, and change lenses if necessary.
Use spherical equivalent correction by transposing cylinders to minus form. Add half of the cylinder for the PV lenses. For the EV38, ignore cylinders up to and including 0.75D, and add 25% to the spherical distance correction before checking the charts.
As little as 0.25D can have unusually positive effects on acuity. Use flatter lenses if VIDs are large.
