  • Accreditation Statements

    Nurses: The American Diabetes Association is accredited as a provider of continuing nursing education by the American Nurses Credentialing Center’s COA. California Board of Registered Nursing: The American Diabetes Association is also a provider approved by the California Board of Registered Nursing, Provider No. CEP-12196. This activity is approved for 9.25 contact hours.

    Dietitians: The American Diabetes Association is a Continuing Professional Education (CPE) Accredited Provider with the Commission on Dietetic Registration (CDR). Registered dietitians (RDs) and dietetic technicians, registered (DTRs) will receive 9.25 continuing professional education units (CPEUs) for completion of this program/material. Pharmacists: The American Diabetes Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. The American Diabetes Association designates these sessions as knowledge-based per ACPE guidelines. Certified Diabetes Educators: To satisfy the requirement for renewal of certification by continuing education for the National Certification Board for Diabetes Educators (NCBDE), continuing education activities must be diabetes related and approved by a provider on the NCBDE List of Approved Providers (www.ncbde.org). NCBDE does not approve continuing education. The American Diabetes Association is on the NCBDE List of Approved Providers. Successful Completion Statement Certificates of Completion/Attendance are provided to registered attendees base upon completion of the online evaluation that will be available at http://professional.diabetes.org/wisconsin on March 4, 2016 through March 25, 2016.     This seminar is designed for nurses, dietitians, pharmacists,

    certified diabetes educators, and other healthcare professionals

    with an interest in diabetes and promoting diabetes education.

    The 2016 activity will update participants on current diabetes

    management.

  • 2016 Registration Information

    Pre-registration is required for the 2016 American Diabetes Association - Wisconsin Area Conference being held on March 4 & 5. The cost of the conference is $175 for professionals and $100 for full-time students. Fee includes breakfast, luncheon, and beverages. Online registration is highly recommended at http://professional.diabetes.org/wisconsin. If you are unable to register online, please return this form with credit card information or your check made payable to American Diabetes Association no later than Monday, February 22, 2016. A late fee of $25 will apply if postmarked after February 22, 2016. Fee applies whether attending all or part of the conference. Room Reservations: When contacting Radisson Paper Valley Hotel, please mention that you are attending the American Diabetes Association Education Conference. A conference room rate is $118/night for single or double and room reservations must be made by Thursday, February 4, 2016. Contact the hotel for more information at 920-733-8000. Cancellation Policy: If notice of cancellation is received at the American Diabetes Association office by Monday, February 22, 2016 a full refund will be made.
