Annual Enrollment 2017

Enrollment begins on Tuesday, November 1 and ends Tuesday, November 15, 2016at midnight Eastern Time.

Important decisions deserve a little homework – that includes making enrollment elections during Annual Enrollment. Your total rewards package from ADP includes benefits that are designed to help you be healthy and secure. For this year’s Annual Enrollment, make a commitment to take the time to understand all of your options and decide what’s best for you, your family, and your health. After all, what’s more important than your health?

The purpose of this interactive Enrollment Guide is to provide you with important information to help you make the right choices that fit the needs of you and your family. Additional enrollment details are available on the Associate Portal under myLife / My Benefits / 2017 Plan Information.

All Annual Enrollment elections become effective on January 1, 2017.

Covered dependent information needed...Under the Affordable Care Act (ACA), ADP is required to report to the IRS health care coverage offered to full-time associates and their dependents. In order to comply with this requirement, we must obtain the legal name and Social Security Numbers (SSNs) for all covered dependents. If you are covering dependents, regardless if you plan to change your benefit elections for 2017, be sure to provide their legal name and SSN during the Annual Enrollment period, if you have not already done so.

Annual Enrollment is your once-a-year opportunity to make changes to your benefits. Spend the time to learn about your benefit options so you can make the best decisions for you and your family based on your needs.

Enroll November 1-15 by accessing the ADP Benefits Enrollment System

The choices you make during Annual Enrollment will go into effect January 1, 2017 and remain until December 31, 2017, unless you have a qualified life event. Beginning in 2017, you’ll now have 60 calendar days to make benefit plan changes after the birth of a child or adoption. The election window will remain at 31 calendar days for all other qualified life events. To learn more visit the Associate Portal under myLife / My Benefits / 2017 Plan Information.

Eligibility

All regular full-time associates are eligible for Medical, Prescription Drug, Dental, Vision, Personal Accident, and Critical Illness Insurance coverage on their first day at work. Part-time associates working 20 standard hours or more per week are also eligible.*

Eligible DependentsIf you choose coverage for yourself, you may also cover your eligible dependents for Medical, Prescription Drug, Dental, Vision, Personal Accident, and Critical Illness Insurance. Dependents include your:

• Spouse, to whom you are legally married.

• Eligible domestic partners who reside in states which allow insured benefits (i.e., HMOs except for Triple-S, Dental DMO, Vision and Personal Accident Insurance) for domestic partners. Refer

to the “ADP Domestic Partner Policy” available on the Associate Portal for more details.

• Dependent children up to the end of the month they turn age 26.

• Dependent children age 26 or older and currently covered under the plan that are physically or mentally disabled and are incapable of earning their own living (Onset of disability must be prior to age 26 or while covered under the Plan.).

* Part-time associates under age 65 are eligible for CDHP with HSA or HMO Medical Plan options, but not Medical Plan Option 1. Associates age 65 or older are eligible for all medical plans.

Important... • If you do not actively enroll in a Medical Plan option during Annual Enrollment, you will remain in your current

plan at your current coverage level and tobacco surcharge election. Refer to the Tobacco Surcharge section for additional details.

• Associates who move from an HMO, Option 1 or No Coverage into a CDHP with HSA option for 2017 must review the HSA election page and choose an HSA contribution of zero or higher to receive ADP’s HSA employer contribution.

Affordable Care Act (ACA)• Under the ACA, everyone is required have health insurance, or pay a fine to the government. You will have the option of

obtaining coverage through ADP or obtaining coverage through government-run health care “Marketplaces” (also known as “Exchanges”). To learn more about “Marketplaces”, go to http://www.healthcare.gov. - ADP’s CDHP with HSA Medical Plan meets all of the law’s requirements for what the government calls affordable

coverage. As such, you will not qualify for a tax credit if you choose to purchase medical coverage through the health insurance marketplace.

- If you are currently enrolled in an ADP Medical Plan option and remain enrolled in 2017, you will meet the ACA individual mandate requirement.

• The government requires employers to provide employees with a certification of your medical coverage. This information will be sent along with your W-2 to you. ADP will also send the information to the IRS to show that ADP meets the minimum essential coverage requirements.

• The ACA requires that eligible associates and other eligible individuals receive a summary of your medical coverage options, a Summary of Benefits and Coverage (SBC), for each available coverage option offered under the ADP Medical Plan. Login to the Associate Portal at https://www.myadp.adpcorp.com and then follow the links for myLife / My Benefits / 2017 Plan Information. Paper copies of the SBCs and glossary are available, at no cost to you, upon request by submitting a case to the HR & Payroll Resource Center or calling 1-877-237-4711. Each SBC is prepared using a required federally-designed template.

The legal plan documents for the ADP Medical Plan provide more complete, detailed coverage information for the medical plan options. If there is any conflict between the statements in the SBCs and the legal plan documents, the terms of the plan documents will govern all rights and obligations of plan participants, beneficiaries, and fiduciaries and of ADP.

For 2017, ADP will offer the Option 1 Plan and the Consumer Driven Health Plan (CDHP) with Health Savings Account (HSA) Medical Plan through Aetna and UnitedHealthcare. ADP will continue to cover most of the cost of your medical coverage. However, to keep up with the rise in overall health care costs, associate contributions will be increasing. Also, local HMOs will continue to be offered in some locations.

Tobacco Surcharge The tobacco surcharge on Medical Plan contributions for associates and covered spouses who certify as Tobacco Users is $50 per month.

IMPORTANT reminders about Tobacco Use Status:• If you or your spouse are newly enrolling in the ADP Medical

Plan, you must complete a Tobacco Use Certification during the 2017 Annual Enrollment period to avoid the surcharge.

• If you make no changes for 2017, the current Tobacco Use status for you and your covered spouse will continue for 2017.

Tobacco user definitions:

• Tobacco User: Individual who, in the last six months, has used tobacco products AND has not completed six coaching sessions through the Healthyroads Tobacco Cessation Coaching Program.

• Non-Tobacco User: Individual who, in the last six months, has not used tobacco products OR has completed six coaching sessions through the Healthyroads Tobacco Cessation Coaching Program.

Tobacco products include: Cigarettes, electronic cigarettes (e-cigarettes), pipes, cigars, chewing tobacco, smokeless tobacco or other tobacco products.

You and/or your spouse can avoid the surcharge by completing six coaching sessions within the previous six months through the Healthyroads Tobacco Cessation Coaching Program, which is available to you at no cost. To get started, visit the Healthyroads website, or call 1-877-330-2746.

Note: You and/or your spouse will have the opportunity to eliminate this surcharge by becoming a non-tobacco user at anytime during 2017 and updating the Tobacco Use Certification through the ADP Benefits Enrollment System. Once updated, the change will be effective the next available pay cycle. Retroactive elections cannot be honored.

Option 1The Option 1 Medical Plan is available through Aetna and UnitedHealthcare (UHC). Each time you or your covered dependent(s) need care, you can use a provider or hospital participating in the network you select in the enrollment process, either the Aetna Choice POS II or UnitedHealthcare Choice Plus networks (in-network), or out-of-network.

CDHP with HSAWhen you enroll in the CDHP, you may be eligible* to open a Health Savings Account (HSA). An HSA is a special account to help cover out-of-pocket health care expenses for you and eligible dependents. The graphic below shows how the CDHP and HSA work together.

Level 1: Preventive Care 100%• Annual physicals and screenings are provided at no cost when you use in-network providers• Includes well-child care• Deductible waived for certain preventive drugs

Level 4: Out-of-Pocket Maximum• Once you reach the out-of-pocket maximum, the health plan pays 100% of covered medical expenses

Consumer-Driven Health Planwith Health Savings Account (HSA)

Level 2: Annual Deductible• The amount you pay out-of-pocket, including prescription drugs, before the health plan pays for benefits

Level 3: Coinsurance• After the deductible is met, coinsurance begins• Coinsurance means you and the health plan share the cost of expenses• For example, the health plan pays 80% and you pay 20% for in-network providers

• Catch-up Contributions: If you are age 55 or older, or turn 55 in 2017, you can contribute an additional $1,000 to your HSA

• CDHP with HSA and Option 1 prescription drug costs apply to the out-of-pocket maximum under the medical plan.

Review Your Beneficiaries! Annual Enrollment is a great time to review and update your beneficiaries. TIP:

Long Term Disability (LTD) All full-time associates working 32 or more hours per week are automatically enrolled in the LTD Plan effective from their date of hire. LTD replaces 60% of your monthly income (up to a maximum monthly benefit of $15,000) to normal retirement age for most conditions (or later, depending on when the disability begins) if you are unable to work after an initial disability of 180 continuous days. You may opt out of coverage at any time, however, if you later want to elect LTD coverage, you will have to provide proof of good health to Prudential and approval by the insurance company is not guaranteed. The cost for LTD is deducted from your paycheck on an after-tax basis so your LTD benefits are not subject to Federal taxes and most state taxes. The monthly cost for LTD is $.27 per $100 of covered monthly salary. For example, if you earned a salary of $75,000, you would only have to pay $203 annually for this benefit.

New! Group Critical Illness InsuranceAll full-time and part-time associates working 20 or more hours per week can apply for Critical Illness insurance through Aflac. The Aflac Group Critical Illness insurance plan can help protect against the financial liability associated with specific covered medical conditions such as cancer, heart attack, renal failure or a stroke. More importantly, the plan helps you focus on recuperation instead of the distraction and stress over the costs of medical and personal bills.

The plan offers two maximum coverage levels for associates: a $40,000 benefit and a $20,000 benefit. Dependent children are included in all coverage levels at no extra cost. Additional coverage can be purchased for your spouse. The benefit amount for covered dependents and spouses is 50 percent of the associate benefit amount.

You can enroll in Critical Illness coverage through the ADP Benefits Enrollment system during Annual Enrollment for coverage effective January 1, 2017. Note that you will not be able to enroll or make changes during the year unless you have a qualified life event.

Group Universal Life (GUL)GUL is term life insurance with an age-rated premium paid by the associate and is available to full-time associates working 32 or more hours per week. The coverage amounts are from 1.5 to 8 times your base pay ($3 million maximum). You may enroll in coverage any time during the year.

New associates have a special GUL enrollment opportunity during their first 60 days of employment. During this period, you can obtain 1.5 times your base pay or $500,000 (whichever is less) in guaranteed coverage. That means a medical exam is not required to receive this life protection as long as you meet eligibility requirements. Amounts greater than this will be subject to Evidence of Insurability. If you are a current GUL participant, be sure to review and update your beneficiaries.

Nationwide Pet Insurance is comprehensive insurance coverage you can use with any veterinarian. When you enroll you’ll receive a five percent premium discount. If you enroll multiple pets you can save even more.

Pet Assure is a veterinary discount plan that offers a 25 percent discount on all in-house medical services – including wellness visits – at network veterinarians. All pets are included regardless of type, breed, age, or health condition. One plan includes all of your pets for only $7.00 a month.

Retirement and Savings Plan – 401(k) PlanADP sponsors the 401(k) Plan to help you achieve your long-term financial goals. All regular full-time and part-time associates can enroll in the 401(k) Plan at any time during the year. Payroll deductions begin within two pay periods after you make your enrollment election.

The 401(k) Plan allows for before-tax, after-tax, and Roth contributions. ADP matches your contributions after 6 months of service.

More information about ADP’s 401(k) Plan is available on the Associate Portal. To access your personal account, visit the ADP 401(k) website. If you are a new associate you will receive a Welcome Letter with your login information in the mail shortly after you receive your first paycheck from ADP. Any questions regarding your 401(k) account can be directed to the 401(k) Service Center at 1-877-401-5237.

Pension PlanAll regular full-time and part-time associates hired on or before December 31, 2014 are also earning a benefit under the Pension Plan. The amount ADP contributes to your Pension account each year is based on your length of service and your pay. Annual accruals range from 2.1% to 10% of your base pay. The account earns interest each year based on the 10-year treasury rate for the month of September of the preceding year. For 2017, interest on account balances will be credited at 3.25%. You are vested in your Pension Plan benefit after you complete three years of service. When you retire, you can choose how you want to receive the money from your account.
