COVERAGE FOR SPOUSE AND DEPENDENT CHILDRENYou can provide medical, dental, vision and life insurance coverage for your spouse to whom you are legally married, as well as your dependent children or your spouse’s dependent children.

COVERAGE FOR ADULT DEPENDENT CHILDRENYou may cover your adult children up to age 26 on your medical insurance regardless of student, marital or employment status. For additional information, including details about dependent eligibility for dental, vision and life insurance coverage, log in to HR Self Service and select Eligibility on the Benefits Resources tab.

MEDICAL AND PRESCRIPTION DRUG PLANThe medical plan is administered by Wellmark Blue Cross Blue Shield and includes prescription drug coverage, which is administered by Express Scripts, Inc.

There are no waiting periods or pre-existing condition limitations for medical coverage. Medical plan options include:• HDHP With HSA – High-Deductible Health Plan With a Health Savings Account.• HDHP W Out HSA – High-Deductible Health Plan Without a Health Savings Account, if you are not eligible to open and contribute

to an HSA or if you want lower premiums and no company HSA contribution.

HEALTH SAVINGS ACCOUNTThe HSA is administered by WageWorks, with banking through BNY Mellon. If you enroll in the high-deductible health plan with an HSA, the company will deposit a fixed contribution to your HSA in January based on your 2018 medical plan coverage level:

DOCTOR ON DEMAND VIRTUAL VISITSEmployees and family members enrolled in the company medical plan can use Doctor on Demand to visit a board-certified physician via video on a smartphone, tablet or computer. Virtual visits may cost less than an in-person visit with your doctor and you can be seen within 3-5 minutes. Physicians are available 24 hours a day to treat minor, non-emergency health issues. To get started, have your Wellmark member ID card ready and download the Doctor on Demand app or visit www.DoctorOnDemand.com.

HSA CONTRIBUTION LIMITSYou may make pretax HSA contributions through payroll deduction. Each year, the IRS sets limits for maximum HSA contributions. During enrollment, if you elect a health plan with an HSA, the benefits enrollment screen will indicate the maximum contribution limit for this year based on your coverage level and your age.

The annual HSA contribution limits include your contribution, the company contribution and any wellness dollars you receive as contributions to your HSA during the year.

HEALTH CARE FLEXIBLE SPENDING ACCOUNTIf you enroll in a health plan without an HSA, you may contribute to a Health Care FSA, a pretax benefit account used to pay for eligible medical, dental and vision expenses not reimbursed by insurance. The benefits enrollment screen in HR Self Service will display the maximum contribution allowed by the IRS. Your total annual election amount will be available at the beginning of your plan year.

LIMITED PURPOSE FSAIf you enroll in the high-deductible health plan with an HSA, you may contribute to a limited purpose FSA, a pretax benefit account used to pay for eligible dental and vision expenses not reimbursed by insurance. The benefits enrollment screen in HR Self Service will display the maximum contribution allowed by the IRS. Your total annual election amount will be available at the beginning of your plan year.

If you reach your medical plan deductible in 2018, your Limited Purpose FSA can become a Health Care FSA. That means you’ll be able to use your FSA funds for eligible medical and prescription drug expenses in addition to dental and vision expenses. When you reach your deductible in 2018, call the customer service number on the back of your Healthcare Card. Ask the WageWorks representative for details about converting your Limited Purpose FSA to a Health Care FSA.

DEPENDENT CARE FSAYou have the option to contribute to a dependent care FSA, a pretax benefit account used to pay for day care expenses. The benefits enrollment screen in HR Self Service will display the maximum contribution allowed by the IRS.

CAREFULLY ESTIMATE YOUR ELIGIBLE ANNUAL EXPENSES BEFORE DETERMINING YOUR FSA CONTRIBUTION. ANY UNSPENT FSA FUNDS WILL BE FORFEITED AT THE END OF THE PLAN YEAR.

WELLNESS PROGRAMWe want you to be as healthy as possible – and we are committed to helping you get there. That’s why we partnered with WebMD Health Services to offer our wellness program, Healthy U, and reward your healthy behaviors. Employees and spouses enrolled in medical coverage can earn up to $600 each in wellness dollars, which are awarded monthly, as you complete wellness activities. Wellness dollars are distributed as contributions to your HSA or as taxable income for employees without an HSA or who have exceeded the maximum annual HSA contribution limit.

DENTAL The two dental plans offer national service providers and cover preventive, restorative and prosthetic services. One plan covers orthodontia for dependent children up to age 19.

VISIONThe vision plan offers a choice of national service providers or out-of-network providers. Each has a schedule of benefits for eye exams, eyeglasses or contact lenses, and eyeglass frames.

401(K) PLANThe company matches 65% of the first 6% you contribute to the 401(k) plan pretax or for Roth.

EMPLOYEE ASSISTANCE PROGRAMThe Employee Assistance Program, administered by Health Advocate, is confidential and free for you and your family, regardless of whether you are enrolled in company benefits. Health Advocate offers expert guidance, counseling, referrals and resources – including articles, webinars and online seminars – for a wide range of issues including grief, eldercare, financial planning, legal concerns and more.

LIFE INSURANCE AND PENSION(IF APPLICABLE)To designate or update your life insurance or pension beneficiary, access the appropriate designation form in HR Self Service on the Benefits Resources tab. Complete the form and return it to Benefits, DMU1, or to your human resources representative. Change your beneficiary designation at any time by submitting a new form.

BENEFICIARY DESIGNATIONS

LIFE EVENTSBecause you pay health care premiums on a pretax basis, the Internal Revenue Service limits your ability to make changes to your benefits during the year. Outside of the annual open enrollment period, you can change your benefits if you have a qualifying life event, such as a marriage, birth or adoption. Life events must be submitted within 30 days, or within 60 days for a birth, adoption, placement for adoption, or changes to Medicaid or SCHIP eligibility. For additional details, review Life Events Overview, which can be found in HR Self Service on the Benefits Resources tab.
