All-inclusive, comprehensive office visit copayment (where applicable) — Covers all diagnostic and treatment
services provided at the medical office of a participating provider. This includes preventive services, diagnostic
procedures, surgical procedures, medical supplies and more!
Doctor visits anytime, anywhere — Have access to a doctor by computer or smartphone with Blue CareOnDemandSM
for the same cost as a PCP visit for most plans. There is a separate charge for members with an HDHP.
Urgent care visits at Doctors Care for the same cost as primary care visits on most plans.
Adult routine vision coverage — FREE annual eye exam and $150 material allowance for glasses and contacts
that can be spent on frames, lenses and lens upgrades with no limits on frame or lens selection.
Pediatric vision coverage — Annual eye exam for $15 and $150 material allowance with a $25 copayment for glasses
and contacts that can be spent on frames, lenses and lens upgrades with no limits on frame or lens selection.
In-network providers for adult and pediatric coverage include Walmart Vision Center, Target Optical, LensCrafters and more.
Preventive dental that includes an allowance for exams and cleanings.
