    Adrian Simone is a 19year old lance corpo-

    ral who joined theMarines after graduating fromMontville Township HighSchool last year. In mid-July, hisunit, 1st Battalion, 6th Marines,Bravo Company was deployedto Afghanistan. One month in,while on patrol, his unit walkedacross an IED (improvised ex-plosive device). Adrian tookthe brunt end of the explosionand both his legs were severely

    damaged. One of his squadmates violated protocol to stayin position and minimize addi-tional causalities and raced tohis side to stop the bleeding,saving his life. He was taken totwo hospitals in Afghanistanand a hospital in Germany,where the doctors tried to save

    one leg. The damage was too se-vere and both legs had to be am-

    putated at or above the knee.He is now recuperating at a hos-pital in Bethesda, Md. He suf-fered other injuries, but none assevere, and there is no evidenceof spinal cord injury.

    Friends who have visitedAdrian in Bethesda confirmthat his lighthearted personalityand upbeat view of the worldhave not changed.

    If you thought layawayplans had gone the way of

    eight-track players andfloppy disks, think again. Thedeferred payment or in-stallment plan, oncepopular with budget-conscious consumersespecially during theholiday season is mak-ing a big comeback.

    For people whoare unfamiliar with theconcept or were born

    after the 1980s (wheninstallment plans gaveway to credit card pur-chases and other types of fi-nancing), layaway is a simpleway to purchase merchandise,interest-free, over time. Con-sumers select the items theywant to buy from a retailer of-

    fering the plan, make a de-posit, which often includes a

    modest service charge, and payfor the merchandise over a

    specified period. Once theitem has been paid in full, theretailer releases the goods tothe consumer. The only poten-tial glitch is if the consumerfails to make payments in full

    or on time. In that event, theretailer has the option of re-

    turning the customers pay-ments (fewer service charges)and reselling the items.

    While theres nodoubt that todays shakyeconomy has contributedto the revival of layawayplans, economists andrepresentatives of severalof the nations leadingretailers and layawayproviders. Sears, K-Mart,

    Burlington Coat Factory,Marshalls, TJ Maxx, ToysR-Us, and Hallmark insist

    that the recession is only partof the story. For retailers, lay-away plans open the doors tomillions of new customers whowould not be able to make pur-

    R onnie is in kinder-garten. Ronnies mo-ther describes him as adifficult child. He only wants

    to wear old t-shirts and jeans.He argues with his mother eachtime she cooks something newfor dinner and refuses to eat. Hethrows tantrums at the mall.Shopping for clothes is almostimpossible. When playing withother children, Ronnie isbossy. Ronnie is happiestwhen he is following his rou-tine. He dislikes surprises.When his mother and father

    hug him, he stiffens. He loveshis German shepherd and willlie on the floor with her forhours. Ronnies mother wonderswhat she is doing wrong.

    In school, Ronnie hasbeen sent to the principals of-fice several times for hittingother children. He sometimesrefuses to participate in groupactivities. During arts and crafts,Ronnie usually asks to go to the

    bathroom.During his six year oldcheckup, Ronnies mother de-scribed these observations to

    her pediatrician who recom-mended an occupational ther-apy evaluation. The evaluationrevealed that Ronnie has a sen-

    sory processing disorder, specifi-cally, a sensory modulationdisorder (SMD). This meansthat Ronnies nervous sys-tem over responds to every-day sensations.

    A child with a modu-lation disorder can appear tohave behavior problems.Refusal to participate in ac-tivities, rigid adherence toroutines and pushing others

    away are actually strategiesused to avoid what thechilds nervous system regis-ters as a threat. Picture your-self in the kitchen atmidnight. The lights areturned off and you thinkeveryone is asleep. Suddenlysomeone taps your shoulder. Inthis situation, your nervous sys-tem reacts with a fight orflight response. You jump, your

    pulse races, you feel afraid. For achild with SMD, the fight orflight response is triggered byeveryday stimulation including

    the feel of a new shirt or glue ontheir hands. Being bumped byanother child in line may be in-terpreted as being punched.

    Over reaction to one event isonly the beginning of the prob-lem for a child with SMD.

    Often they are unable to calmtheir nervous system and thesensory events build up over thecourse of the day. Even if a child

    has strategies to calm them-selves, these may not appear so-cially acceptable or mayinterfere with life skills, as when

    Ronnie spends arts and craftstime in the bathroom. So whatcan be done?

    A qualified occupational

    therapist uses a detailed parentand child interview as well asstandardized assessments to de-

    termine the nature of a sen-sory processing disorder.There are several types of sen-sory processing disorders andthey can vary in degree andpresentation. Once the spe-cific nature of the disorder isuncovered, the family needsto understand what is going

    on. In almost all cases, theparent and child are relievedto hear that there are reasonsbehind the behaviors and thathelp is readily available.

    Therapy for SMD consistsof age appropriate, motivatingplay oriented activities that

    aim to regulate the childs nerv-ous system. Various types of sen-sory rich activities are usedincluding: movement, deep

    pressure and resistance whichstimulate the sensory receptorsin the muscles and joints, tac-tile, visual and auditory input.

    Foods and scents can also beused. As the child responds pos-itively in therapy, a sensory dietis developed for home and

    school. This diet incorporatesactivities and modifies chal-lenges to enable the child to in-crease their abilities to not onlytolerate, but to enjoy and learnfrom sensations they previouslyavoided. For Ronnie, the vac-uum cleaner turned out to beone of his favorite toys. Thevibration helps to regulate hisnervous system! His mother, un-derstanding his challenges,

    washes new clothes severaltimes before Ronnie wears themso that they felt softer and smelllike his other clothes. She hasfound that Ronnie prefers rawcarrots and string beans to thecooked ones that she had beentrying to convince him to eat.Most importantly, she learnedthat Ronnie enjoys bear hugs aslong as she does not stroke hishair. Ronnie and his teacher are

    pleased that when he stands atthe end of the line to avoidbeing bumped, he no longerpushes or hits his peers. She

    moved his desk so that he sits atthe end of the row. Ronnie is al-lowed to use a special brush toscrub his hands before and afterfinger painting.

    Working with his occupa-

    tional therapist helped Ronnieto learn what he needs to do tofeel just right. He can now usehis strategies before going tobirthday parties and play dates.He is making friends and is ahappier child.

    Middle ear infection, orotitis media, affects two-thirds of children by two yearsof age, making it the mostcommon medical diagnosis inchildren. Approximately athird of all children will ex-

    prience at least six episodes of this disorder before enteringschool. Some children haveup to twelve episodes in a sin-gle year. Several bouts of re-occurring infections canresult in permanent damageto the inner ear.

    Typical medical treat-ment of otitis media involvesthe prescription of oral an-tibiotics, commonly amoxi-cillin. However, researchpublished in the Journal of the American Medical Asso-ciation (Dec /91) reveals notonly is amoxicillin ineffective

    for otitis media, it actuallyworsens the condition. Thereis also the problem of re-peated use of antibiotics andthe evolution of superbugs,those growing numbers of an-tibiotic-resistant bacteria.

    Eventually, despite fail-ure of the antibiotics, thebodys own immune systemtakes care of the infectionand all is well. Or so it ap-pears, until the next reoccur-rance. After several episodesof this, it is recommendedyour child has tubes im-planted in their ears, a proce-dure called tympanostomy

    T he term polydipsiarefers to excessivethirst. We see this asan increase in the amount of water being drank, which inturn usually leads to polyuria,which means your cat is uri-nating more often and alarger volume.

    These changes may bevery subtle and slowly pro-gressive or quite obvious. Youmay observe an increase inthe amount of wet litter in acats litter box or even acci-dents outside the litter box.Some cats may begin drinkingfrom a dripping faucet in thesink or from an open toiletbowl. However, if you want todetermine exactly how muchyour cat is drinking, allowhim only one source of waterand subtract the amount leftin the bowl after 24 hoursfrom the amount you put inoriginally. On average, catstake in about 2 cups perday for a 10 pound cat. Thisincludes any water that istaken in from other sources,such as, from canned food.

    If you determine thatyour pet is drinking exces-sively or just think its morethan usual, make an appoint-ment with your veterinarian

    because polyuria and polydip-sia (drinking and urinatingmore) can be an early sign of a serious disease. These caninclude: kidney failure, dia-betes mellitus, hyperthy-roidism (overactive thyroid),liver disease, electrolyte ab-normalities, pyometra (uter-ine infection), other lesscommon endocrine disordersand diabetes insipidus.

    Several diagnostic tests

    may be needed to determinethe cause of your catspolyuria and polydipsia be-cause many different diseasescan cause these signs. Any

    visit to your vet should beginwith a complete medical his-tory and thorough physicalexamination. Initial testingmay include: measurement of water intake at home, com-plete blood count (CBC),blood chemistry tests (includ-ing electrolytes), urinalysis,urine culture and sensitivity,thyroid hormone (T4) con-centration and abdominal x-rays or ultrasound.

    In older cats, the threemost common disease condi-tions seen in practice thatcause polyuria and polydipsiaare kidney failure, diabetesmellitus and hyperthyroidism.

    Diabetes mellitus diag-nosis is based on high bloodglucose concentration andthe presence of glucose, andoften incompletely brokendown fat products called ke-tones in the urine. This dis-ease is very treatable longterm. Initial treatment de-pends on the severity andmay include hospitalizationwith intensive fluid and elec-trolyte therapy in conjunc-tion with administration of short-acting insulin or outpa-tient management with long-acting insulin.

    Kidney insufficiency orfailures diagnosis is based onserum biochemistry tests andurinalysis. Treatment is basedon rehydrating the cat, tryingto slow the progression of thekidney disease by dietarymodification and treatingcomplications such as hyper-

    tension, nauseaand urinary tractinfection.

    H y p e r t h y -roidisms diagno-sis is based onmeasurement of thyroid hormoneconcentration viabloodwork. Diffi-cult cases may require special-ized blood tests or imaging.Treatment consists of drugtherapy or radiation or lesscommonly surgery. This dis-ease is also very treatable longterm. Treatment with ra-dioactive iodine (I-131)will require referral to a spe-cialty clinic or teaching insti-tution in your area.

    Optimal treatment of any persistent medical condi-tion depends on establishingthe correct diagnosis. Thesymptom group polyuria andpolydipsia has many differentpotential causes, and it is im-portant to identify the under-lying cause before beginningtreatment.     notice any changes becausewith an early diagnosis youoften get a better outcome.

    A s one of the most com-monly injured struc-tures in the knee, themeniscus is unique because of itsblood supply. For both the me-dial and lateral menisci, the

    blood supply is from the outsidein. The closer to the center of the knee, the less of a blood sup-ply there is to the meniscus.From the treatment and healingpoint of view, the blood supply

    anatomy is critical. The outerone third of the meniscus has agood blood supply where heal-ing can occur, while the innertwo thirds has a poor blood sup-ply where healing almost never

    occurs. In orthopedic surgerytextbooks, we call the outerone-third the red zone, and theinner two-thirds the white zone.

    Whereas most fracturesand many ligament tears will

    heal on their own, tears of themeniscus in the inner two-thirds have a very difficult timehealing back because of thepoor blood supply. Especially inpatients who have complex

    tears, or tears that have multi-ple little tears in the region, pa-tients are often fighting a losingbattle as they wait for healing tooccur. Although we typicallytry a course of physical therapy

    in patients with meniscus tearsbefore considering surgery, wealways advise patients that theymay feel less pain but meniscustears often remain. Further-more, these tears may get biggerover time, and may cause adja-cent cartilage damage with itsunstable or irregular torn sur-face.

    The surgical treatment of meniscus tears is performedarthroscopically, or with mini-

    mally invasive technique. Thetwo most common proceduresare repair and partial resection.

    Meniscus transplanta-tions are performed in therare patients who meetthe strict criteria. Onlypatients who have tearsin the outer one third of the meniscus are candi-dates for a meniscal re-pair. In the nineties, theorthopedic communitytried repairs in the inner

    two thirds with low successrates. The poor blood supply inthe inner two thirds meant that,despite a strong repair construct,the tear did not heal back. Sec-ond operations were oftenneeded, and there were a lot of grumpy patients.

    There are different menis-cal repair techniques out there,but among the most common isthe suture technique. Wearthoscopically introduce a su-ture with tiny plastic hooks ontheir ends. The plastic hookscapture the capsule tissue be-hind the meniscus and the su-ture is then tied down to repairthe tear. Especially for tearsthat are 1 cm or less, the suturerepair technique has reliable

    success rates. For larger tears,such as bucket handle tears, thesuccess rates drop and we aremore hesitant to do a repair inthese patients.

    More commonly, pa-tients often have tears in theinner two thirds of the menis-cus, or the white zone. Thesepatients are treated with apartial resection. Using a mo-torized shaver, we resect outthe damaged meniscus tissue,while leaving the healthy rimcomponent. Its a fine bal-ancing act between trying topreserve as much meniscustissue as possible while creat-

    ing a smooth remaining sur-face to prevent further menis-cus tears.

    After surgery, we mini-mize the meniscus repair pa-tients from zero to 90 degreesof knee bending, 6 weeks aftersurgery. For the meniscuspartial resection patients, wedont have any strong restric-tions after surgery except toavoid going too wild. Someof my West Milford patientshave gone back to their con-struction jobs the day after orgone motorcycling that week-end. But the West Milfordpatients are a hearty bunchwith excellent genetics.

    A.W. Allard, a Colorado vet-erinarian, examined numerousvariations in dog urine and theeffects on several commonlawn grasses. He found fescueto be the most nitrogen-resist-ant. Yet, even that grass has itssaturation point and canbrown.Who is the biggest offender?

    Dog owners sometimesthink that female dogs havedifferent urine chemistry be-cause they tend to do the mostdamage to the lawn. The factis, both male and female dogs

    can do damage, it just comesdown to the way urine is ap-plied. Male dogs oftentimesmark their territories or spraya small amount of urine to dif-ferent parts of the landscape.These small concentrationsmay not damage the lawnmuch. However, a female dogtends to squat and apply theurine in one spot at a highconcentration. That liquidseeps into the lawn and can dodamage. Male dogs that do notlift their legs and prefer tosquat can do the same level of damage as females.

    Cats that use the out-doors instead of a litter box

    may also create brown spots,particularly if they visit thesame spot over and over.

    Fixing the problemApart from diluting the

    urine, which can cause its ownissues, the best way to alleviatebrown spots is to walk the dogaround the neighborhood toother appropriate spots. Other-wise, a dog-only area can be setup in the yard. Then pet own-ers must train their dogs to usethat specific area. Laying downpea gravel or mulch can be away to absorb the urine with-out having it damage the lawn.Camouflage, like bushes or afence, can set the dog area

    apart from the rest of the back-yard as well.

    It can take up to twoweeks or more to train an olderdog to take to the new pottyarea. Puppies may adapt a lit-tle faster. Collecting urine andfeces and placing it in the dogsarea can help set up a scentmark that may make it easierfor dogs to learn thats wheretheyre supposed to go. For thetraining period, pet ownersshould accompany their dogsto the new area on a leash; donot leave the pet unattendedin the yard. He or she may re-vert back to the normal spotbeing used for relief.

    W ith so many peoplelooking for alterna-tives to costly holi-day fare, newspapers, magazinesand online cookingsites are overflowingwith great ideas. Butthe key to hosting asuccessful seasonalmeal is not simplyfinding interestingrecipes and purchasinglower-cost foods, butin creating and organ-izing a menu thatworks from start to finish.     The point is: you needto know your EFC, and real-

    ize it will be the same regard-less of the school; so why waitto find out and further delayplanning. The only way topay less than your EFC is if your student goes to a schoolthat cost less than your EFC;for example, a communitycollege. Once you determineyour EFC, it is time to deter-

    mine how you will pay thatamount, regardless of theschool. Need based aid ismeant to help cover your fi-nancial need, not your EFC.Therefore, you will pay yourEFC from either, savings, in-vestments, your income, or byborrowing. Many families donot have enough savings to

    cover their EFC for the firstyear, let alone four or moreyears. Therefore, most fami-lies will be borrowing to covera portion of their EFC.

    Once you determinehow much you are expectedto pay (your EFC and unmetneed), it is important to de-termine how much you can

    pay, or more importantly howmuch you should pay. It iscrucial to determine howmuch you should pay earlyon, because if you dont, youcould paint yourself into acorner financially. Youshould know how much youmay have to borrow and whatthat means in monthly pay-
