    LifeNet Case Study

    LifeNet helicopter ran out of fuel a mile short of its destination and crashed, killing the pilot and 3 others in August 2011:

    Pilot began to prepare for medical flight. Pilot began a private text conversation making dinner plans

    during flight preparation. Pilot missed 2 opportunities to detect insufficient fuel for

    mission. Pilot realized fuel was low half-way through first leg of flight.

    LifeNet Case Study

    Pilot arrived at the hospital, picked up the patient, and looked for a closer destination to refuel.

    Pilot was on ground for 13 minutes and during the stop had another private text conversationanother private text conversation.

    Pilot takes off after miscalculating that he could reach his destination.

    Pilot continues to text in the air up until 19 minutes before crash.

    LifeNet helicopter ran out of fuel a mile short of the destination. LifeNet helicopter crashed into a pasture killing the pilot, the

    patient, and two medical personnel.

    LifeNet Case Study

    Crash blamed on fatigue, training, and distracted texting. Engine ran out of fuel, but the gauges were working properly. Pilot radioed from the hospital that he had 45 minutes worth of

    fuel when he only had 30 minutes worth of fuel. Pilot and his friend exchanged 85 private text messages during

    a 12-hour shift. Investigators believe the pilot wanted to avoid revealing that he

    took off without enough fuel. Autorotation training the pilot received was not representative

    of an actual engine failure at cruise speed.
