Since the last presentation to the Police Commission, on, the August 8,2018, the Firearm Discharge Review Board reviewed the following completed OIS investigations:
• 01S15-009
• 01S16-003
• OIS 17-003
• OIS 17-004

Officer Involved Shooting 15-009
Date & Time: November 11, 2015—Approximately 16:15 Hours
Location: 3000 Block of Cesar Chavez Street
Numerous officers responded to a construction site on the 3000 block of Cesar Chavez
Street. Workers reported a man with two guns had made his way to the sixth floor of the site. Upon their arrival, the officers took up positions to get an eye on the suspect. They observed the suspect holding a "long gun" and saw him looking around as if he were looking for potential targets. Seeing that the suspect was in a high position of advantage, and was armed with a long gun, the officers believed the suspect posed a deadly threat to themselves and the civilians in the area. Three officers fired at the suspect, fatally striking him.
Recommendation to COP: Use of Firearm—In Policy (in effect at time of incident);

Officer Involved Shooting 16-003
Date & Time: October 14, 2016—Approximately 20:06 Hours Location: 28th Avenue & Wawona Street
Two uniformed officers assigned to Ingleside Station were among the officers responding Citywide regarding an officer shot in the Sunset District. The suspect was contained within a perimeter in the area of Sigmund Stern Grove. The two officers were on an edge of this perimeter, near 28th Avenue and Wawona Street, when the suspect emerged from nearby cover and opened fire. The officers returned fire and mortally wounded the suspect.
Recommendation to COP: Use of Firearm—In Policy (in effect at time of incident);

Officer Involved Shooting 17-003
Date & Time: May 3, 2017—Approximately 11:22 Hours
Location: 900 Block of Market Street
Bystanders directed officers to a disturbance at a restaurant on the 900 block of Market Street. The officers saw the suspect actively stabbing an employee. The employee attempted to flee, with the suspect in close pursuit, still wielding his knife. An officer fired at the suspect, mortally wounding him. No one else was injured.

Officer Involved Shooting 17-004
Date & Time: May 21, 2017—Approximately 22:56 Hours Location: 2138 Quesada Avenue
Officers responded to a report of a home invasion robbery by armed suspects.
Arriving officers set up a perimeter, then attempted to make contact with the subjects in the residence. Armed suspects rain out of the back door, where perimeter officers ordered them to stop. A suspect fired multiple rounds at the officers; one officer returned fire. No one else was hit and the suspects escaped from the immediate area.

The 2nd Quarter 2018 OIS Investigation Report was presented to the Police Commission on
August 8, 2018. Since that
• Four (4) OIS investigations were presented to FDRB and completed (01S 15-009, OIS 16-
003, OIS 17-003 and OIS 17-004). These investigations are complete.
• Seven (7) OIS cases have active criminal investigations with the SFPD and/or DA's office
(17-001, 17-005, 17-006, 17-007, 18-002, 18-003, and 18-005).
One (1) case has an open SF ME investigations (18-002).
• Three (3) cases are open administratively (OIS 16-001, OIS 18-001, OIS 18-004).
